0020101202 2017-02-14 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice 1310311233 and I was accompanied by ODNR inspector Doug Bennett. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, and 1.5 inch diameter tubing. All valves are in the closed position. There are no production lines attached to the well. There was a light sulfur smell upon arriving at the wellsite. Utilizing the ODNR Sperian gas detector, Honeywell PHD6 gas detector, and Tango TX1 Hydrogen Sulfide Detector, no gas or hydrogen sulfide was detected. There has been no action taken since the last inspection on 6/16/2016. Pictures were taken to assist this inspection.

0154610417 2019-03-04 Production Wells No Violations The purpose of my inspection was to follow up on a previously issued compliance notice. On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, with a 4.5-inch diameter to 2-inch diameter swage nipple with a 2 inch diameter valve. The well was no longer hooked up to any production equipment or flow lines. There was a 100-barrel steel production fluid storage tank near the well head that was no longer in use, and what appeared to be a 1.25-inch diameter transmission line ran next to the well head. Production area was shared with 34121215890000 and consisted of a 100-barrel steel production fluid storage tank, and 2 horizontal separators. Identification tags were present on the production equipment however it was no longer legible, all valves were shut and the well appeared to be idle, and production was reported as 0 since 2014.
0369259422 2015-09-29 Administrative Inspection No Violations At 2:29 pm I received a call from Scott Davis in response to a voice mail I left. Mr. Davis and I discussed the companys plans for the well. Mr. Davis stated that he was going plug the well. I reminded Mr. Davis that the well has been in violation for more that four years.

During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a tubing, and 1.5 inch diameter tubing. All valves are in the closed position. There are no production lines attached to the well. There was a light sulfur smell at the wellsite. While using Sperian and Tango TX1 Hydrogen Sulfide Gas Detectors no gas or hydrogen sulfide was detected. The well was not legibly identified in a conspicuous place on or near the wellhead. There has been no action taken to gain compliance and all original violations still exists.

During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, and 1.5-inch diameter tubing. All valves were in the closed position. There were no production lines attached to the well. While utilizing the Ventis Pro4 gas monitor zero Hydrogen Sulfide was detected. While standing directly beside the wellhead 10 LEL was recorded. The production equipment consisted of two 100-barrel tanks and two horizontal separators. Tank battery is shared with 3412121589. Green Gas Company reported zero for production for the years 2014, 2016, and 2017.

The well was not legibly identified in a conspicuous place on or near the wellhead. Green Gas Company reported zero for production for the years 2014 and 2016. Pictures were taken to assist this inspection.
1310311233 2011-07-07 Production Wells Violations Noted Upon inspecting site at request of land owner Larry Woodford found well to be idle and no production has been reported on well since it was drilled in 1983,all valves at well head were shut on the day of my inspection and no domestic hook ups were noted.

PLUG OR PRODUCE WELL OR FILE FOR INACTIVE STATUS. FAILURE TO COMPLY MAY RESULT IN BOND FORFIETURE.

1426533595 2015-09-24 Production Wells No Violations I am accompanied today by Inspector Sefcik, our inspection finds the well is shut in. A brass ball valve on the casing head is black, indicating that H2S may be present. A 100 barrel tank is set on location, but it is not connected to any pipelines. Photos were taken.
