0641963773 2016-09-29 Plug / Plug Back No Violations G-R Contracting used their service rig to attempt to get a hold of the 1.5 tubing setting 4 from the top of the 4.5 casing wellhead. The overshot tool was unable to retrieve the pipe. Appalachian Wireline Surveys attempted to run a collar locator tool down the tubing, but only made it to a depth of 3036.
0648290986 2016-10-13 Plug / Plug Back No Violations During my inspection today Formations Cementing set a 200 foot isolation plug with Class A cement in the 7.875 inch diameter open hole from 3940 feet to 3740 feet. G-R contracting pulled 51 joints of the 4.5 inch diameter casing to 1967 feet. Formations set a 200 foot bottom of surface plug with Class A cement from 1967 feet to 1776 feet. G-R pulled the 4.5 inch diameter casing to 200 foot. Formations cementing set a 200 foot surface plug with Class A cement with 2% calcium. G-R pulled the remaining 4.5 inch casing from the hole. The surface plug dropped and Formations set an additional 125 foot surface plug with Class A cement with 2% calcium. Cement stayed at surface.
0726455424 2016-07-19 Temporary Abandoned Annular Disposal No Violations My inspection today finds the annular disposal is not in use. The well is equipped with 4.5 casing and 1.5 tubing. Both valves on the 4.5 by 1.5 tubing head have rotted off and are laying at the base of the well. Also, the wellhead is very badly corroded, which is consistent with being affected by H2S. The 1.5 tubing and plunger lift lubricator have broken off at the top of the wellhead. There are no means of well control on the 1.5 tubing or the 4.5 casing. A sulfur smell can be detected in the vicinity of the well, but no H2S was noted on the gas detector. A ten foot diameter area, around the well, has been affected to the point that no vegetation is growing. The tank battery is comprised of one 210 barrel tank, a rusted out and collapsed 50 barrel steel tank and a production unit. No identification could be found at the wellhead or the tank battery. G-R Contracting Inc. shall submit a written plugging schedule to Eric Conrad at 5880 Memory Road Zanesville, Ohio 43701 within 15 days, per OAC 1501:9-11-05(B), and plug the well within 90 days. G-R Contracting Inc. will be notified if the plugging schedule is approved or denied.
0821050425 2016-10-12 Plug / Plug Back No Violations The purpose of my inspection today is to witness Appalachian Well Surveys run a bond log and shoot the 4.5 inch diameter casing off. During my inspection Appalachian logged the well and discovered we had free 4.5 inch casing to 4000 feet and the casing appears to be in good condition. Appalachian ran a casing cutter down to 4000 feet and shot the casing off. G-R contracting pulled two joints of the 4.5 inch casing.
0842877345 2016-08-08 Production Wells No Violations My inspection today finds the status of the well has not changed. The wellhead is badly deteriorated. The tubing has broken off. Valves on the 4.5 casing head have rusted off and the well is open to atmosphere. No repairs have been made to the well. I called and spoke to Jordan Jones with G-R Contracting and reminded him that a written plan of action is required for the well. Mr. Jones asked to be allowed to send the plan via email and I agreed.
1123035080 2016-09-14 Production Wells No Violations My inspection today finds the well owner has moved a dozer on location in preparation for plugging. Jordan Jones stated in a telephone conversation that the company intends to place one to two loads of brine in the well to quell any gas flow.
1420408808 2016-10-11 Plug / Plug Back No Violations The purpose of my inspection today is to witness Formations cementing set a plug within the Big Lime formation. During my inspection Appalachian Well Surveys shot the 1.5 inch diameter tubing off at 4525 feet. G-R Contracting pulled two joints of the 1.5 inch tubing and formations set a 300 foot Class A cement plug from 4525 to 4225 feet. G-R Contracting removed all of the 1.5 inch diameter tubing from the hole.
1475700745 2016-10-05 Plug / Plug Back No Violations The purpose of my inspection today is to witness Appalachian Well Surveys shoot the 1.5 inch diameter tubing at 5200 feet in order to get a proper bottom plug. During my inspection Appalachian ran in hole with their tools and they could only make it to 2800 feet. Appalachian pulled their tools back out of the hole and G-R Contraction ran a broach down hole to try and clear a column within the tubing 1.5 inch for Appalachians tools. Once G-R tripped back out of the hole Appalachian ran back in with their tools but could only get down to 3064 feet with this attempt. The depth we need to reach still has not been achieved. Appalachian tripped their tools back out of the hole. When the tools came to surface they attached a heavier sinker bar to the bottom of the casing cutter. Appalachian tripped back into the hole, got down 5194 feet, and successfully cut the 1.5 inch tubing. Appalachian tripped their tools back out of the hole. G-R rigged back up to the 1.5 inch diameter tubing to pull 2 joints up and the rig was not able to pull the tubing. Operations will resume tomorrow.
1680590625 2016-09-20 Production Wells No Violations My inspection today finds the well operator has dug around the wellhead to expose the 8.625 by 4.5 casing head with a back hoe. While digging the 4.5 by 1.5 tubing head fell off. Also, a G-R Contracting employee stated that his personal protection H2S monitor alarmed at 18 parts per million.
1855330907 2016-11-22 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection the location has been graded and seeded. Vegetation is growing on location and there is not any erosion issues. There is no evidence of an oil and gas well left behind. Location has met 1509.072 B requirements.
