During my inspection the well was equipped with 4.5 inch diameter production casing with a tubing head, 1.5 inch diameter tubing, and a plunger lift system. The production valves were in the closed position and no production lines were attached. There was no production equipment on location. The well remains equipped in a manner that is incapable of production. Northwood Energy reported Zero for production since the year 2010. Original violation still exists.

1273437403 2016-07-19 Temporary Abandoned Annular Disposal No Violations My inspection today finds the annular disposal is not in use. The well is equipped with 4.5 casing and 1.5 tubing. The flow line is disconnected from the wellhead. The well has reported zero (0) oil or gas production since 2010. No production equipment could be found. Additionally, the lease road is in poor condition. Northwood Energy shall submit a written plugging schedule to Eric Conrad at 5880 Memory Road Zanesville, Ohio 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Northwood Energy will be notified if the plugging schedule is approved or denied.
1537288225 2018-02-01 Production Wells No Violations The purpose of my inspection was to follow up on compliance notice numbered 1273437403. During my inspection the well was equipped with 4.5 inch diameter production casing with a tubing head, 1.5 inch diameter tubing, and a plunger lift system. The production valves were in the closed position and no production lines were attached. A sour gas odor can be smelled at the wellhead but the Tango Tx1 hydrogen sulfide gas detector detected zero readings of hydrogen sulfide. There was not any production equipment on location. Northwood Energy reported zeros for production since the year 2010.

1755829752 2019-04-11 Production Wells No Violations The purpose of my inspection was to follow up on a previously issued compliance notice. On the day of my inspection I witnessed the well was equipped with 8.625-inch diameter surface casing, 4.5-inch diameter production casing, with 1.5-inch diameter tubing utilizing a plunger lift system as means of production. At the time of my inspection the flow line had been disconnected and the well appeared to be idle. It appeared that production area was shared with 34121235820000 and consisted of a 100-barrel steel production fluid storage tank, 2 horizontal separators, 2-inch diameter meter run with chart recorder, the chart in the chart recorder was not up to date. Identification was present on the production equipment and production was reported as 0 since 2011.
