2018-07-18 | Final Restoration | No Violations | The purpose of my inspection was a follow up on a final restoration. During my inspection today, the surface was graded, mulched, and seeded with thick vegetation in place. All debris and equipment have been removed. The location has met the requirements for final restoration.
2018-01-19 | Plug / Plug Back | No Violations | Bishop tripped 1972 feet of 1.5 inch diameter tubing within the 7.875 inch diameter open hole and Formations cementing set a 200 foot plug. The plugged consisted of 60 sacks of class A cement. The 1.5 inch diameter tubing was pulled up to 200 feet and Formation pumped a 60 sack plug to surface. The tubing was tripped out of hole and the well was topped off with cement and the cement stayed at surface.
2018-01-18 | Plug / Plug Back | No Violations | Bishop tripped 1.5 inch diameter tubing to 5882 feet in depth. Formations Cementing arrived on location and pumped a 400 foot bottom plug through 1.5 inch diameter tubing. The plugged consisted of 30 sacks of class A cement with a yield of 1.18. 13.5 barrels of fresh water were pumped for displacement. Bishop pulled the 1.5 inch tubing up to 150 feet below the top of the Big Lime and Formations pumped a 150 foot big lime plug. The plugged consisted of 45 sacks of class A cement with a yield of 1.18. Bishop tripped the 1.5 inch diameter tubing out of hole and tripped within the 4.5 inch production casing with the ripping tool. Telegraph from the ripping tool was received at 3300 feet in depth and the casing was successfully ripped. The 4.5 inch diameter casing was tripped out of hole.
2006-10-12 | Annular Disposal | No Violations | A.D. not hoked up well is equipped with pump jack and tubing and rods and 1-210 bbl tank that will need dike work preformed when SPCC rules go into effect.
2015-08-10 | Production Wells | No Violations | My inspection today finds the well is equipped with tubing and a plunger lift system. The tank battery consists of one fifty barrel tank, a separator and meter with a current chart. All production equipment is set in an earthen dike.
2016-12-28 | Temporary Abandoned Annular Disposal | No Violations | The purpose of my inspection today is a status check on a temporarily abandoned annular disposal well. During my inspection the well was equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, and a plunger lift system. The production valve is open with a production line attached. This well is not disposing brine through the annular disposal method. There is not an annular disposal connection plumbed up to the annulus. The production equipment consists of one 50 barrel tank, one chart meter, and a vertical separator. The production records are current, the identification is located on the 50 barrel tank, and there are no violations present.
2018-01-16 | Plug / Plug Back | No Violations | I met with Buckeye Oil Producing on location to discuss plugging procedures. The well has known hydrogen sulfide issues. During my inspection Bishop Well Services showed up on location with their service rig. The well was equipped with 4.5 inch diameter production casing, 1.5 inch diameter tubing, and a plunger lift system.
2012-03-13 | Annular Disposal | No Violations | Inspection found AD not hooked up or in use and well shut in, equipment at well 4.5 production casing -1.5 tubing, The well is set up to be produced by means of a plunger lift system.
2018-01-17 | Plug / Plug Back | No Violations | The purpose of my inspection was to witness plugging operations of this well with a known hydrogen sulfide gas problem at surface. This well was completed through 4.5 inch diameter production casing to produce from the Clinton and Medina formations with 40 perforations from 5792 feet to 5917 feet in depth. The well was equipped with 8.625 inch diameter surface casing set at 1862 feet and 4.5 inch diameter production casing set at 6017 feet in depth. Bishop well service was rigged up on location with their derrick service rig. Bishop began to trip the 1.5 inch diameter production tubing from the well and experienced 250 parts per million of hydrogen sulfide gas. A brine truck arrived on location and loaded the 4.5 inch casing with 70 barrels of brine and discontinued the flow of the hydrogen sulfide gas. The entire string of 1.5 inch diameter tubing was tripped from hole. Timco Well Surveys arrived on location and rigged up with 10 shots in a 10 foot span. Timco tripped in hole and perforated from 4326 to 4336 feet in depth.
2018-03-26 | Final Restoration | No Violations | The purpose of my inspection was a status check on a final restoration. During my inspection the pit had been filled and the site had been graded. One 100 barrel tank was located near the plugged well. Once the tank is removed the site will then meet final restoration requirements written under 1509.072 (B). Heavy logging activity was currently taking place on the well site and lease road. Harvested trees were being stored near the lease road entry and where the recently removed tank battery was located.
