0415154949 | 2014-03-31 | Production Wells | No Violations | My inspection today finds the well appears to capable of production. The tank battery consists of eighteen 400 bbl tanks. Identification is posted in accordance with 1501.9-9-05(A)10. The lease road is in good repair. Production records are current. |
1023616323 | 2017-08-10 | Production Wells | No Violations | The purpose of my inspection was a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of three gas production units, six 400 barrel tanks, and one combustor. Identification was present and production records were current. Construction equipment was parked on the east side of the pad. |
1238495263 | 2012-11-13 | Drill / Deepen / Reopen | No Violations | I stopped at the well site to check the status of the well. The rig is being moved. |
1350659969 | 2012-10-18 | Drill / Deepen / Reopen | No Violations | On site to witness 13.375 surface string cementing. The pipe was run to a depth of 1192 with a guide shoe on bottom. 45 bbl of returns to surface. The job was delayed by a top drive malfunction. |
1371356584 | 2012-11-06 | Drill / Deepen / Reopen | No Violations | My inspection today finds the drilling rig crew has ran 5.5 casing to a depth of 12080. Universal is on site to cement the well. |
1409020029 | 2013-11-01 | Preliminary Restoration | No Violations | The well site meets the preliminary restoration requirements. |
1473105371 | 2017-01-05 | Production Wells | No Violations | On the day of inspection accompanied by ODNR Senior Health Physicist Paul Carder, The well was equipped with a production tree with valves in the open position and a gauge present read 400 pounds per square inch on the casing. Production equipment included a sand trap, a line heater and a GP Unit with a 4 inch sales meter with a 1.5 inch orifice and chart recorder. Also present a vapor tower and 6-400 barrel tanks all within containment and a gas combustor all shared with the Noble Unit 1H and Guernsey Unit 1H. A fenced in water impoundment with life preservers is located south west of the well pad. |
1546882222 | 2019-01-07 | Preliminary Restoration | No Violations | On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing and pressure was at 215 pounds per square inch. The 13.375-inch diameter casing was stubbed up to surface with a 2-inch diameter valve that was cracked at the time of my inspection. Production equipment included a Gas Processing Unit, as well as 6- 400-barrel steel production fluid tanks, and a combustor that was shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a Mark West midstream valve site near located on the northwest corner of the pad. Identification tags were present, and production was up to date. It appeared that vegetation had been established around the location and preliminary restoration has passed according to Ohio Revised Code 1509.72 (A). |
1554154715 | 2012-10-28 | Drill / Deepen / Reopen | No Violations | On site to witness the second stage of the 9.625 cement job. Universal Well Services pumped cement back to surface. 5 bbl of cement returns. |
1611766857 | 2016-05-16 | Production Wells | No Violations | My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of six 400 barrel tanks set in a plastic lined steel containment system. Also set in the same containment, are three production units. Mark West personnel are on site to run a pig through the pipe line. |
