0253529723 2014-04-30 Preliminary Restoration No Violations My inspection today finds the well meets the preliminary restoration requirements. The site has been seeded and the roadway restored. The tank battery consists of two 100 bbl tanks, a separator and a meter.
0358918223 2014-10-29 Production Wells No Violations At 6:58 PM I received a telephone call from Bruce Carpenter, EH&S Director, for Eclipse Resources. Mr. Carpenter indicated that a release of crude oil was discovered at the wellhead. Upon my arrival I witnessed nine personnel of Eclipse Resources onsite with two spill response trailers and heavy equipment. The spill was contained on location and all standing oil had been removed. The company estimated the spill at 10 barrels. The affected area measured 30x40 around the well with a narrow strip of oil extending 150 down a tire depression in the lease road. The spill appears to have been caused by the accidental closure of a valve at the tank battery by a contractor for the land owner who was installing a domestic gas tap. The pressure caused the diaphragm to fail on a Big Joe regulator at the wellhead allowing oil to be released under pressure.

1347540629 2012-09-12 Drill / Deepen / Reopen No Violations My inspection today finds the rig is running 7 casing. The well bore has been drilled with a 8.875 bit to a depth of 1340. I witnessed the rig crew run 1272 of 7 casing in the well. Formation cementing cemented the pipe with 215 sks of class A cement, 600 lb of accelerator, 200 lbs of cotton seed hulls, and 25 lbs of lcm flake. Good cement returns to surface.
1347892694 2012-09-12 Drill / Deepen / Reopen No Violations Returned to the rig for the surface pipe pressure test. The well was pressured up to 375 psi at 1:15 pm and had 370 psi at 1:45 pm.
1813819481 2013-08-26 Brine Hauler No Violations I received a call from Elaine Grissom. Ms. Grissom stated that water trucks illegally withdrawing water from a local stream. I met Isaiah Levan (driver for Harmon Well Service) at the location, he stated that they had verbal permission to haul from that point. I asked him to get written permission before hauling any more. He said that they were done anyway. He and another truck hauled 15 loads total for the day.

All pipe was properly marked. I witnessed Formation Cementing pump 65sks of class A cement and circulate it to surface.
