0313424924 2014-02-10 Completion Testing No Violations My inspection today finds Halliburton is pumping one bbl per minute due to a screen out. The pump pressure is averaging 9000 pounds. This is the forth stage of the well.
0342515702 2013-04-27 Casing String Witnessed No Violations My inspection today finds the drilling contractor has drilled the well to a depth of 347. The rig ran 322 of 20 j-55 casing and cemented it in place with 566 sks of class A cement.
0439894221 2013-04-24 Casing String Witnessed No Violations My inspection today finds the drilling contractor ran 30 conductor to a depth of 126. Baker Hughes cemented the casing in place with 312 sks of class A cement.
0456090048 2016-03-21 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery is comprised of eight 400 barrel tanks set in a plastic lined steel containment system. A large compressor is set on location and is in operation at the time of inspection.
0517361761 2013-03-22 Drill / Deepen / Reopen No Violations My inspection today finds the well site is currently under construction. All erosion controls are in place as per the pre-site review.
0584758754 2017-04-06 Production Wells No Violations The pad is named NBL 19 and it consists of 3 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges present read 620 pounds per square inch on the casing and 330 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consisted of three sand traps, three gas production units, eight 400 barrel tanks, and one combustor. Identification was present and production records were current.
0636906515 2018-03-08 Production Wells No Violations The pad is named NBL 19 and it consists of 3 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of three sand traps, three gas production units, one vertical heater treater, one reboiler, seven 400 barrel tanks, 2 vapor recovery towers, and one combustor. The pipeline tie in was located on the south side of the pad.
0951115460 2013-07-31 Drill / Deepen / Reopen No Violations The drilling rig has drilled a 12.25 hole to a depth of 7300. They are preparing to run 7258 of 9.625 casing. The stage cement tool will be set at 4500.
1151823910 2015-04-07 Production Wells No Violations At the time of my inspection, Sunpro, Force, and TK Gas Services are providing cleanup services. Nearly all free product has been removed from the inside the containment. A revised volume of the release will be forthcoming as displacement for tanks and pea gravel were not taken into account in the initial calculation. One foot of water has been placed into the containment to float any product trapped in the pea gravel. A crew is using booms to sweep the remaining product to one end of containment where a Force vac. Truck is skimming it off of the water. Once nothing is left but a sheen, an oil degrader will be added and allowed time to work. There is no sign of hydrocarbons outside of the containment.

1376915803 2013-08-12 Drill / Deepen / Reopen No Violations My inspection today finds the well is at a depth of 11,190. The operator is currently drilling ahead to an estimated depth of 17925.
1459403398 2019-7-30 0:00: PL No Violations 0.50
1468511625 2014-05-19 Preliminary Restoration No Violations I am accompanied by Austin Gutridge. Our inspection today finds the well site meets the preliminary restoration requirements of 1509.072.
1474635741 2015-04-06 Production Wells No Violations At 6:32pm I received a telephone call from Jon Andrusik indicating that a 100 barrel condensate release took place at the NBL 19 production facility and that all condensate was in secondary containment. My inspection finds the source of the spill was an open valve on the loading manifold. The site is under video surveillance and a more definitive explanation of how the valve was left open will be forthcoming. Sunpro is onsite with a vac. Truck and two additional trucks in route. The plan moving forward is to remove all free product with the vac truck and then begin bringing fresh water in and flush the inside of containment. All water recovered during the process will be put back into the production tanks and later hauled to an injection well. I have requested that CNX sample the area around the lower end of containment and continue monitor the soil around the containment for signs of hydrocarbons. At this time, there is no reason to believe the that the product has left the secondary containment. All wells are shut in and the clean-up has been suspended until an electrical storm in the area has cleared.
1514599109 2019-1-3 0:00:0 PW No Violations 0.50
1705946074 2014-08-06 Preliminary Restoration No Violations My inspection today finds the well site meets the preliminary restoration requirements. The well is currently in production. The tank battery is comprised of eight 410 barrel tanks.
