0284286559 2016-02-25 Production Wells No Violations My inspection today finds the well properly equipped to be capable of production. The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by four other wells on the pad. The rainwater drainage sumps are open at the time of inspection.

The well was equipped with a production tree style wellhead. Production valves were in the open position. Pressure monitoring was present on all casing strings. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. The identification was located at the wellhead and the production records were current.
0587006866 2018-11-26 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.375-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 200 pounds per square inch, the 9.625-inch diameter casing had 900 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The Gas Processing Units and midstream valve site were on a secondary pad to the southeast of the well pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and two combustors. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.
2063135418 2014-06-13 Casing String Witnessed No Violations I am onsite to witness the conductor cementing operations. Express Energy Services drilled a 32 hole to a depth of 100. They then ran 100 of 26 casing. Express also provided the cementing service by circulating 330 sacks of cement in place. I witnessed twenty barrels of cement return to surface.
