0153426231 2018-11-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 0 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 12- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a midstream valve site on the northwest side of the pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
0653388225 2017-03-15 Production Wells No Violations The pad name is JR Byler and it consists of 7 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges present read 420 pounds per square inch on the casing and 160 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consisted of seven gas production units, twelve 400 barrel tanks, three compressors, and two combustors. Identification was present and production records were current.
1164043278 2014-07-15 Casing String Witnessed No Violations My inspection to day finds Express Energy Services has drilled a 32 hole to a depth of 100. They then ran 100 of 26 98 pound casing. Allied Oil and Gas Services cemented the casing in place with 330 sacks of class A cement. I witnessed good cement returns to surface.
1211640762 2017-10-17 Production Wells No Violations The purpose of my inspection was a status check on a production well. The pad name is JR Byler and it consists of 7 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. Gauges present on the annulus read 0 pounds per square inch. The gauge on the production casing read 350 pounds per square inch and the gauge on the production tubing read 170 pounds per square inch. The production equipment consisted of twelve 400 barrel tanks within steel line containment and seven gas production units. A vapor recovery system to capture tank vapors consisted of one compressor and two combustors. The pipeline tie in was located on the North side of the pad. Pipeline contractors were in the process of launching a pig during inspection. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.
1287043149 2014-07-14 Casing String Witnessed No Violations I am onsite to witness the cementing operations of the conductor casing string. Express energy ran 100 of twenty-six inch weld casing. Express also cemented the casing in place with 330 sacks of class A cement. I witnessed twenty barrels of cement return to surface.

1408578897 2014-08-20 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. I tested the mix water with a low range Quantab which indicated the total chlorides were at 37 ppm. Dallas Morris 24 ran 371 of 18.625 casing. Allied Oil and Gas Services circulated 730 sacks of class A cement into place. The casing was pressured up to 800 pounds and lost only five percent.
