0090035633 2017-10-18 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well pad name is Cynthia and it consists of 5 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. Annular pressure monitoring was present. The gauge on the production casing read 280 pounds per square inch and the gauge on the production tubing read 170 pounds per square inch. The production equipment consisted of eight 400 barrel tanks within steel line containment and five gas production units. A vapor recovery system consisted of two compressors, one vent scrubber, and three combustors. The pipeline tie in was located on the North side of the pad. A dust suppressant had been recently applied to the lease road. The identification was located at the wellhead and the production records were current.

The tank battery consists of eight 400 barrel tanks set in a plastic lined liquid tight steel containment. The tank battery is shared by four other wells on the pad. Two vapor recovery compressors and three incinerators are located near the entry of the well pad. Heavy rain is falling at the time of inspection.
1692609820 2018-11-27 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing equipped with a plunger lift system. At the time of my inspection annular pressure monitoring was in place, and the 13.375-inch diameter casing had 300 pounds per square inch, the 9.625-inch diameter casing had 0 pounds per square inch. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid tanks, and 2 combustors that were shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. There was a midstream valve site on the northwest side of the pad. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

The tank battery is comprised of eight 400 barrel tanks set inside of a plastic lined, liquid tight steel containment system. There are three vapor recovery units and three incinerators set at the location. Four other wells are located on the pad and share the production equipment. The ground is covered by three inches of snow at the time of inspection.
1993401106 2015-02-04 Drill / Deepen / Reopen No Violations After reviewing the cement bond log that was performed by Renegade Services on 12-30-2014 and conferring with manager Brent Bear the decision was made to require 1000 of separation between the cement top at 7241 and the upper most perforation. At 1:40 PM, I called Steve Self, Completions Manager for Antero Resources, and discussed the 1000 requirement. Mr. Self agreed to the condition that well not be perforated above 8241.
