0021169229 | 2015-12-28 | Production Wells | No Violations | My inspection today finds the well is in production. The well is still being produced into temporary tanks while work on the North slope of the pad is on going. |
0206529245 | 2019-01-31 | Production Wells | No Violations | On the day of my inspection accompanied by ODNR inspector Bill Stolarik I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place on the 9.625-inch diameter deep intermediate casing, and 13.375-inch diameter intermediate casing. Production equipment included a Gas Processing Unit, as well as 8- 400-barrel steel production fluid storage tanks, and a combustor that was shared with the other producing wells on the pad. There was secondary containment around the production fluid tanks, and the Gas Processing Units. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. There was a midstream valve site on the west side of the pad. As well as a water impoundment to the south, right side of the lease road as you enter the pad. Identification tags were present, and production was up to date. |
0470900792 | 2018-03-13 | Production Wells | No Violations | The purpose of my inspection was a status check. The pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of eight gas production units, eight 400 barrel tanks, two vapor recovery towers, and two combustors. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current. |
0566944897 | 2015-06-01 | Drill / Deepen / Reopen | No Violations | My inspection today finds the drilling contractor, Patterson Drilling 343, is drilling an 8.75 borehole at a depth of 14449. The anticipated total depth is 18,123. |
0952232980 | 2019-01-31 | Production Wells | No Violations | On site inspection along with ODNR Inspector Ryan Armstrong. On this day we were conducting field training and visited the well pad. The pad consists of 8 producing wells. The tank battery sits on the left side of the pad as you enter it and consists of (8) 400 barrel tanks. Identification is present on each well head. See Ryans reports for more information. |
1003596752 | 2016-05-10 | Production Wells | No Violations | My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set inside of a plastic line steel containment system. On the opposite side of the pad there are eight production units, also set in containment. Several pieces of construction equipment are parked on the location and are being used for the reclamation of the adjacent fresh water impoundment. |
1022090260 | 2017-03-17 | Production Wells | No Violations | The pad is named Wilson and it consists of 8 wells. The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. Antero Resources was on location preforming maintenance on a gas production unit. |
1071129035 | 2018-02-02 | Production Wells | No Violations | Status check following a nearby pipeline mechanical issue that is not owned by Antero. This pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. The pipeline tie in was located on the West side of the pad with no issues present. The identification was located at the wellhead and the production records were current. |
1128825137 | 2015-01-20 | Drill / Deepen / Reopen | No Violations | My inspection today finds the conductor has been cemented in place. I can see cement at surface. |
1285003789 | 2015-02-04 | Casing String Witnessed | No Violations | I am onsite to witness the surface casing installation and cementing operations. The drilling rig, Sidewinder Drilling Rig 59, drilled 425 of 24 borehole. Wyoming Casing ran 412 of 18.625 J-55 casing. CalFrac cemented the casing in place with 662 sacks of class A cement with 3% CaCl and .25 pounds per sack of Celloflake. I witnessed 37 barrels of cement circulate to surface. |
1291083822 | 2017-10-24 | Production Wells | No Violations | The purpose of my inspection was a status check on a production well. The pad is named Wilson and it consists of 8 wells. The well was equipped with a production tree style wellhead. Production valves were in the open position. A gauge present on the production casing read 380 pounds per square inch and the gauge on the production tubing read 175 pounds per square inch. Annular pressure monitoring was present. The production equipment consisted of eight gas production units, eight 400 barrel tanks, and two combustors. Identification was present and production records were current. The pipeline tie in was located on the West side of the pad. The identification was located at the wellhead and the production records were current. |
