0231013712 2015-08-13 Preliminary Restoration No Violations I am accompanied by Inspector Brian Sefcik on my inspection today. The well site meets the preliminary restoration requirements of 1509.072(A).
0651733752 2018-07-27 Production Wells No Violations Status check inspection found the well in production and flowing through a production tree into a 3 sales line with 200 psi currently showing on the production flow line. There is an emergency shut down device on the production flow line with multiple control points on the well pad. Annular pressure monitoring is in place on the 13.375 and 9.625 well head sections with 250 pounds per square inch pressure showing between the 5.5 diameter production casing and 9.625 diameter intermediate casing. There is 750 psi between the 9.625 casing and 13.375 casing strings. There are 3 active wells on the site with proper well identification posted on each well head. The production equipment consists of 3 gas processing units along with 4 steel construction 400 barrel capacity production tanks situated inside a polymer lined steel containment dike area.
0731856085 2017-06-07 Production Wells No Violations The purpose of my inspection was a status check on a producing well. The pad is named Brown and there are a total of three wells on this pad. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. Gauges present read 800 pounds per square inch on the casing and 500 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of one 50 barrel tank, four 400 barrel tanks within a lined steel containment, three gas production units, three sand traps, and two combustors. The pipeline tie in is located on the east side of the pad. The production records are current, the identification was located at the wellhead, and there were no violations present.
0902886199 2017-02-14 Production Wells No Violations The purpose of my inspection is a status check. During my inspection the well was equipped with a production tree with valves in the open position. Gauges present read 1400 pounds per square inch on the casing and 700 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment included three gas production units, three sand traps, two gas combustors, one 50 barrel tank, and four 400 barrel tanks within containment. All production equipment is shared with 3412124550, 3412124551, and 3412124552. Identification was present and production records were current. I was accompanied by ODNR inspector Doug Bennett.
0988151073 2015-06-01 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig, Patterson 257, has reached a total depth of 7334. I witnessed the rig running 9.625 casing and installing bow spring centralizers. The planned landing depth of the casing is 7306.
1361300456 2014-11-26 Casing String Witnessed No Violations I am onsite to witness the conductor cementing operations. Lightning Services drilled 103 of 36 hole. Lightning then ran 103 of 26 weld casing in the well. Stingray Cementing LLC. cemented the casing in place by circulating 401 sacks of class A cement with 3% CaCl and .25 pounds per sack Celluseal around the casing to surface. I witnessed 25 barrels of cement circulate to surface. One foot of cement was left in the base of the cellar to provide a liquid tight seal.

1480225616 2016-08-05 Production Wells No Violations On August 4, at 9:27 AM, I received a telephone call from George Muller who stated that he lived adjacent to the Brown well pad. Mr. Muller stated that he could see a fifty to sixty foot flare burning on the pad and he was concerned about his safety. At the time of my inspection I found the pad in the initial production phase. A flare no more than ten feet tall was burning at the flare stack. While speaking to the Gulfport representative I was able to determine that the cause for the large flare on the previous day was high line pressure due to a Mark West facility mechanical problem. For now, the company is producing the well trough their flow back equipment due to high pressures and flow rates. The company representative stated that he did not anticipate the flare stack being utilized for more than two weeks.
1537274722 2018-09-18 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The production equipment consisted of three sand traps, three gas production units, and four 400-barrel tanks. Two combustors are decommissioned laying on the side of the pad. Gulfport Production Superintendent Robert Coots notified ODNR of a small wellhead leak on the Brown 1A. Gulfport is working on a plan for remediation.
1656649916 2015-06-10 Drill / Deepen / Reopen No Violations My inspection today finds the drilling contractor, Patterson Drilling, has reached a total depth of 16550. The drilling rig is beginning to pull the drill pipe out of the 8.75 well bore. 16548 is the anticipated casing point for the well. A contingent from ODOT is touring the site at the time of inspection.
