Production valves were in the open position. The production equipment consisted of twelve 400 barrel tanks within steel line containment, six sand traps, and six gas production units. The sand traps were flowing back into a 400 barrel gas buster tank. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Two Myers trucks were hauling produced fluids from the tank battery during inspection.

1293588227 2018-09-18 Production Wells No Violations The Wehr pad has a total of 7 wells, 6 production wells, and 1 well that has not been completed. The purpose of my inspection was a status check. The well was equipped with a production tree style wellhead. Production valves were in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of twelve 400-barrel tanks, six sand traps, and six gas production units. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Myers Trucking was hauling produced fluids from the tank battery.
1301653415 2016-11-23 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 was drilling ahead with an 8.75 inch diameter bit at 8319 feet on 11.0 pound per gallon (ppg) mud simulating 11.6 ppg. Frontier Drilling Rig #32 is planning on landing the curve at 8675 feet with a projected total depth of 19500 feet. I completed walk around with Antero Consultant Brandon Jeffcoat revealing proper containment was in place and the 5.5 inch production casing was being offloaded.
1473691086 2016-09-08 Casing String Witnessed No Violations I arrived on location to witness Express Energy Services set conductor casing on the SLASH UNIT 2H. Prior to my arrival Inspector Conrad had approved them to set the conductor to 80 feet. We set the conductor using 257 sacks of class A cement with 2% NaCl and about a half a pound of flake. The weight of the cement was 15.6 pounds and had a 1.18 yield with a 32.19 mix. We used a total of 43.35 barrels of fluid for displacement. When we finished setting the casing we had a 10 foot shoe. After we had finished setting the conductor we had about 15 barrels of cement come back to surface and 55 barrels of water come to surface giving us a total of 70 barrels total to surface.
1479248331 2016-11-15 Casing String Witnessed No Violations On the day of my inspection I witnessed that Frontier Drilling Rig #32 had drilled a 17.5 inch diameter hole to the depth of 1049 feet. Wyoming Casing Crew ran 1039 feet of 13.375 inch 54.5 pound per foot J-55 casing. Frontier Drilling Rig #32 had to wash down the last 2 joints of casing. C & J Energy Services was on site and gained circulation with 231 barrels of fresh water, pumped 25 barrels of gel, 10 barrels of fresh water and cemented casing with 980 sacks of Class A cement weighing 15.6 pounds per gallon and a yield of 1.19 cubic foot per sack cement. Cement additives included 2% calcium chloride and .25 pound per sack of cello-<31 parts per million, pH 7 and 60 degrees.
1480254462 2016-11-27 Drill / Deepen / Reopen No Violations I received a call on 11/26/16 at 2235 from Antero Consultant Chad Francois stating that at 2035 while tripping back in the wellbore they experienced a kick and drilling mud made it to their flare stack resulting less than a barrel release and that crews were in place cleaning up. Upon arrival on 11/27/16 at 0915 I met with Consultant Brandon Jeffcoat who stated that they had tripped out the wellbore to repair their directional tool. They were down approximately 18 hours for repairs and they experienced the kick with 8000 units of gas forcing diesel based drilling mud past their separator into the flare stack. They were at 13600 feet reaming the wellbore down to 14337 feet with 11.1 pounds per gallon mud, simulating 12.4 pounds per gallon. The flare stack is on containment with a calculated 11.9 gallons inside, 1.8 gallons surrounding the stack and .3 gallons outside the berm. Inspection revealed they were currently drilling on 11.3 pounds per gallon mud, simulating 12.0 pounds per gallon at depth of 15702 feet with a planned total depth of 19566 feet. Minimal drilling mud residue was found near the flare stack. Antero will complete the clean-up today with additional scraping around the flare stack and micro-blaze being applied in the general area.
1488837393 2017-03-06 Preliminary Restoration No Violations Preliminary Restoration status check inspection accompanied by ODNR County Inspector Austin Gutridge. The area affected by the well pad construction meets ORC 1509.072 with vegetation established and erosion control in place. The lease road is stoned and in good repair. Signage is posted at the well pad entrance. Halliburton crews are on site preparing to begin the well stimulation 5.5 diameter production casing toe preparations.
