KEN MET WITH USEPA INSPECTOR JEFF KIMBLE TO REVIEW WELL AND OIL SPILL SITE ON 5/28/2009. OIL ENTERED A FIELD TILE, AND IT FLOWED INTO PONDS ON THE BRUSH WELLMAN PROPERTY. THE WELL WAS LOCATED BY FOLLOWING THE OIL IN THE FIELD TILE TO NEAR THE WELL SITE. EXCAVATION AT THE SITE EXPOSED 8-, 6-, AND 2-INCH PIPES IN THE WELL APPROXIMATELY 5 FEET BELOW GROUND. A 12-INCH PVC PIPE WAS PLACED OVER THE WELL, AND IT WAS DRIVEN APPROXIMATELY 8 FEET INTO THE GROUND. ANOTHER 20-INCH PVC PIPE WAS PLACED OVER THE 12-INCH TO PROVIDE EXTRA STORAGE CAPACITY. OIL WAS NOTED IN THE WELL CASING, BUT IT WAS NOT IN THE PVC PIPE. ABOUT 500 GALLONS OF OIL DISCHARGED FROM THE WELL. USEPA AND OEPA COORDINATED CLEAN-UP OF THE OIL. 1244122230 2009-06-01 Orphan No Violations Oil discharged from a previously cut off abandoned well in a farm field owned by Dennis Widman. Met with USEPA inspector Jeff Kimble to review well and oil spill site which occurred on 5/28/09. The oil entered a field tile and flowed into ponds on the Brush Wellman property. The well was located by following the oil in the field tile to near the well site. Excavation at the site exposed 8, 6 and 2 inch pipes in the well approximately 5 feet below ground. A 12 inch PVC pipe was placed over the well and driven approximately 8 feet into the ground. Another 20 inch PVC pipe was placed over the 12 inch to provide extra storage capacity. Oil was noted in the well casing but not in the PVC pipe. Approximately 500 gallons of oil discharged from the well. USEPA and OEPA coordinated clean up of the oil. The well will be submitted to the Orphan Well program for emergency plugging with US Coast Guard funding. Spoke to ODNR Wildlife officer John Waltose during inspection. Checked for oil discharge in Sugar Creek and Portage River. No signs of oil. No reported impact to wildlife.
1262637854 2010-01-04 Plug / Plug Back No Violations Open well with 8 inch casing. Operator had to spear tubing to remove from well. Rods and tubing removed and well cleaned out to depth of 1250 feet.. Pumped 125 sacks cement for bottom plug into well through 2 inch tubing and circulated oil and water to tank.
1262711206 2010-01-05 Plug / Plug Back No Violations Cement in well set up at 250 feet. Operator poured 9 sack grout into well and displaced fluids into tank through reverse grout method. Cement added to well after tubing removed with cement noted at top of well. Well will be cut off 4 feet below ground and ID plate attached. Returned later same day. Well cut off 4 feet below ground and cement to top of pipe. ID plate attached and pit backfilled/graded.

Checked treeline between S.R. 590 and well site for possible additional well locations. No signs of wells but area has heavy vegetative cover in most places. Noted gas bubbling between 6 inch and 2 inch pipes. Fluid is still contained in well casing.
