0117654956 2014-11-03 Plug / Plug Back No Violations At the time of the inspection the well was cut off three feet below grade with cement observed at that level inside the 4.5 casing. No gas in any levels was detected at the well or the surrounding area. The identification plate was placed on the well and the remaining hole was filled in. The area was then graded and prepared for seeding.

There was no change in the status of the well from previous inspections and the Hydrogen Sulfide levels were measured at 93 ppm in winds over 15 mph.

At the time of the inspection there was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at any levels in the work area. There was no change in the condition of the well from when it was uncovered on 10-16-2014. A ring of DO NOT ENTER tape was left around the area until plugging operations continue.
0417213226 2014-04-30 Orphan No Violations This inspection was done at the request of Glen Davis of Glen Davis Well Service to discuss the logistics of plugging the well for the Orphan Well Program. At the time of the inspection the well was still bubbling trace amounts of Hydrogen Sulfide gas. There was a strong odor downwind from the well but no Hydrogen Sulfide gas levels were detected due to high winds. The well is located in a 10 by 10 sinkhole in an area of small brush ~100 south of State Route 2.
0663889673 2014-10-28 Orphan No Violations This well is under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the crew moved the rig and equipment to the location to start the cleanout of that well. A crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. There was 5 ppm Hydrogen Sulfide at the wellhead and levels around the well during the wellhead preparation stayed between zero and 15 ppm of Hydrogen Sulfide. The 6casing was cut off about three feet below grade and a 4.5 collar was welded to the 6 to allow for use of the 4.5 swage and oil saver. The inside of the casing was filled with solid debris. Drilling with a 4 cable tool bit started at 12:30 pm and continued till 4:30 pm and a depth of about 30. A ring of DO NOT ENTER tape was put around the area until cleanout continues.
0815360509 2012-03-21 Production Wells No Violations Contacted by Ottawa Co. Health Dept. to investigate a well located along SR 2. Well has strong septic odor. Well head casing has rusted off due to elevated H2S concentrations. Gas meter readings H2S are 173 ppm, LEL was 40 %, CH4 was 0.55 ppm. Health department sent a letter to landowner identifying well and requested completion of idle and orphan application.

At the time of the inspection the well was plugged from a Total Depth of 35 to the surface with 150 sacks of 50/50 poz mix from PetroSet Cementing. A crew from OBriens Safety Services was on location to monitor gas levels. There were readings of 73 ppm Hydrogen Sulfide at the wellhead during the cleanout and levels around the well and work area during plugging stayed between zero and 15 ppm of Hydrogen Sulfide.

1413476314 2014-10-16 Orphan No Violations This well under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection a backhoe was brought on location by the plugging contractor to begin site preparation. A path was cleared through brush to the well location. The area was excavated and an approximately 6 diameter casing was uncovered. There was a distinct smell of Hydrogen Sulfide at the location but no gas was detected at any levels in the work area. A ring of DO NOT ENTER tape was put around the area until plugging operations continue.
1415027298 2014-10-29 Orphan No Violations This well is under contract to be plugged for the Orphan Well Program by Glenn Davis Well Service. At the time of the inspection the crew from OBriens Safety Services was on location to monitor gas levels during the cleanout process. There was 73 ppm Hydrogen Sulfide at the wellhead before cleanout continued and levels around the well during drilling stayed between zero and 15 ppm of Hydrogen Sulfide. Drilling with a 4 cable tool bit started at 9:30 am and continued till 12:30 pm and a depth of 32. At that time the drilling slowed considerably and it was determined this was the bottom of the well. The decision was made to plug the well at that depth on 10-30-14. A ring of DO NOT ENTER tape was put around the area until the well is plugged..
