During my inspection I found that all the steal flow line had not been removed yet and the separator was still on location. I talked with Zack Poling and he said he would get it taken care of.

0527215561 2015-09-10 Plug / Plug Back No Violations I was onsite to witness the rest of the plugging on this well. When I arrived on location Kenny was pulling the tubing back out of the well. He set down at 1200ft when running the tubing down to set the plugs. We ran the tools down to see if we could knock the bridge out but had no luck. We then ran the tubing back down and hooked up Formation to circulate the hole clean. We were able to clean up the hole and trip the tubing down to set the Clinton plug from 3240-3040. We then pulled up to set the Lime plug from 2440-2240. We pulled the tubing out of the well then hooked onto the 5.5. They pulled out 2 joints then set the Berea plug from 1125-965. The 5.5 was pulled up to the bottom of the surface to set a 200ft bottom of surface plug. We then topped the well off with a 200ft surface plug.

1231166049 2013-09-04 Production Wells Violations Noted The permit to plug this well has expired. I went out to check the status of this well. It is still out of production. I talked with Mr. Poling and he told me he is thinking of putting it back in production. If he does not he will renew the permit to plug the well. The company plans on putting well back in production. I notified the company and told them to have it back in production by January or they will have to apply for a plugging permit and plug the well.

He is going to attempt to put the well back into production. If the well does not show any signs of capable production he plans on plugging the well.
1441717881 2015-09-08 Plug / Plug Back No Violations Wrights well service on site. They have pulled out the 3 casing and left the 5.5 casing. A 35 bottom hole plug was set thru tubing. The plug did not circulate.
1441806062 2015-09-09 Plug / Plug Back No Violations I was onsite today to witness the plugging of this well. Kenny was unable to find a free spot in the 5.5 to rip the pipe the day before. So today Appalachian is on site to run a bond log to find free pipe. The bond log showed the pipe has cave in all the way up to 1200ft. I had Appalachian run a 10 shot 10ft gun down to the Clinton so we could set a plug there. They ran back in the well to perforate the Lime. Then Appalachian ran a cutter down to 1200ft to cut the 5.5 off. Kenny picked up on the casing to make sure it was free then set it back down. The 1.5 tubing will be ran in the well to set the Clinton plug. Formation cementing will be set up for tomorrow.

1950353075 2014-03-05 Production Wells No Violations I talked with Mr. Poling about this well just to remind him the well will need to be put back in production or plugged. He let me know that as soon as the weather is better and it is dry they would be servicing the well. If they did not like the results they would plug the well while the rig was still on location.
