0005206456 2014-10-07 Plug / Plug Back No Violations I was onsite to witness the plugging of this well. The tubing was set at 1550ft for the first plug. We were unable to get the tubing back down to the Lime formation. After the pipe was shot off the hole caved in down the 5.5. We tried to wash the tubing down for 2days but only gained 120ft. We set a 250ft plug at 1550 then pulled out 250ft of pipe and set another 200ft plug to cover the Berea. The tubing was then pulled up to the bottom of the surface casing and a 200ft plug was set. The tubing was then pulled up to set the 200ft surface plug. The cement stayed at the surface with no signs of settling. The well is now plug.

1412339824 2014-10-03 Plug / Plug Back No Violations I was onsite to witness the plugging of this well. The rig crew had to run the tubing back down to the Lime to set the plug. They were unable to get the tubing down to the lime so they had Formation Cementing hook up to the tubing and try to wash the pipe down. The tubing would not wash down. They are shutting down for the day and will try to clean up the hole on Monday. We talked about possibly bringing out a mud pump and power swivel.
1412774879 2014-10-07 Plug / Plug Back No Violations I received a request from inspector Justin Smith. I was asked to cover this plug job. Formation Cementing on location setting the plugs. I witnessed the lime/isolation plug and the bottom of surface plugs. Inspector Smith will fill out the plugging report.
