We discussed the options that needed to be done to make the liner liquid tight. Joe does not want to replace the entire liner due to cost. His plain is to take a twelve (12) foot 30 mil liner and place it from the top of the dike in to the flat area of the dike area and use silicon caulk (three beads) to join the existing liner to the new twelve (12) foot section.

He told me that they did not have capacity in the tank for the water. I advised that they need to develop a plan to keep the dike free of fluids.

0986138662 2014-04-10 Salt Water Injection Well No Violations Witnessed the release of the packer and the removal of one sub joint of tubing (4.10). Circulated fresh water and inhibitor then set the packer at 2426 with 34,000 lbs tension. Tested the annulus 5 times with a steady loss in pressure despite attempts to pull more tension, release air from the annulus. Company decided to trip tubing and packer out of well. Packer rubber had multiple cuts. Replaced packer with a rebuilt Halliburton R-4 (lined). Witnessed the packer being ran back in the well. On the 2nd joint of tubing the service rig elevators opened and the tubing (2joints) and packer were dropped into the well. Company plans to fish out the tubing and packer on 4-11-14.

- On site, tallie tubing and place rotary bit on 2 3/8 tubing to drill out cement plug in casing and to check for open well. Circulate fresh wash in the well, pull tubing from well.

1307627503 2011-05-20 Salt Water Injection Well No Violations Producer Well Service pump 1000gal of fresh water with 20% acid into well. Tallie tubing and install new R-4 Halliburton packer (2 pins removed) in well to a depth of 2,444.09 feet. Measurement was to the center of packer. Pump 5 gallon of inhibitor and 1,000 gallons of fresh water into well and circulate to the annulus. Set packer 14,000 string weight and pull 28,000 psi, packer slip in well, try test but failed, Operator try to release packer but failed. Decision was to wait to Monday 5/23/2011 and try to release packer and pull from well for inspection.

Packer set at 2430. Circulated fresh water and inhibitor to surface prior to setting the packer. Packer set at 22,000 over string weight. Tested annulus at 890 psi for 15 minutes with no loss in pressure.

Attempted 7 total tests while pulling joints out of the well. Reseating the packer. All attempts resulted in the same drop in pressure while testing. Company plans on tripping out of the well and evaluating the tubing and packer.

Rig crew found pieces of plastic that were in the collar threads on a few joints of tubing. There were no visible areas on the packer to indicate a problem. Tubing is stacked at well site. A Pump Off Plug set at 3,000 lbs was installed on the bottom of the tail joint. Halliburton R-4 packer (lined) was rebuilt. Ran used 2/38 lined tubing. Tubing was tested at ten joint intervals during installation. Found one defective joint of tubing. Attempted multiple times at increasing pressure to release the pump off plug. Attempts were not successful. Shut down operations for night.

They found tubing liner inserts that were broken and a few that were swollen due to pressure. The pump off plug was half way out. The 4 tail joint was full of flakes and sediment. Witnessed tubing and packer (rebuilt Halliburton R-4 lined packer) being ran back into well. Circulate fresh water (25.50 barrels with 2 gallons inhibitor) prior to setting packer. Packer set at 2429, (24,000 over string weight). Tested at 900 psi for 15 minutes with no loss.
