1172603642 2007-02-26 Production Wells No Violations - Received phone call on from adjacent landowner that storage tanks were over flowing, also received phone call from Wes Drake of Ohio EPA on this mater. Arrived at location and James Deal of Pickaway County Emergency response was on location. Well pump jack had been turn off before arriving to site. Kilbarger Company, Larry Howdyshell operator was cleaning oil from dike area, about 45 barrel of oil was contained in dike area. A some amount of oil mist was on the top and outslope of the dike about a five foot area. Kilbarger had more people coming to help with clean up.

The wind blew a mist onto the ground outside of the dike. The ground outside of the dike area has been cleaned up and straw placed on the ground. Requested operator to remove soil from inside the dike area because it is saturated with oil from the last two spills. The inside of the dike area hasnt been clean at the time of the inspection, talked to Bob Atha and he said Killbarger was in charge of removing the material and that they completed this as soon as possible.

1618630757 2019-01-03 Production Wells No Violations I received a call from Mr. Zack Sminner at 9:43 AM. Mr. Zimmer stated when he arrived at this well he noticed the fitting that was plumbed into the wellhead on the tubing tee had leaked. I called the owner of the well, Mr. Bob Masters at 9:52 AM and left him a voicemail message to return my call. I arrived on site at 10:45 AM and found the oil had ran away from the well in two directions. This oil reached approximately 60 long by an average of 6 feet wide in the two directions it traveled. The oil stopped in standing water. The standing water is approximately 20 feet from an unnamed stream. I used two absorbent booms and absorbent diapers to ensure no oil entered into the stream. Kilbarger Construction on site at 1:08 PM. They have brought a 20 yard roll off container with a liner to load the contaminated soil into. This soil will be taken to the Hocking Athens landfill for disposal. At 1:30 PM Mr. Jim King arrived with a mini excavator, straw, seed and absorbent pads and booms. AT 2:50 PM Kilbarger Construction arrived with a vacuum truck. The contractor dug a hole began moving the oil to the sump that was dug. The vacuum truck was used to vacuum the oil from the sump. The contractors removed approximately 95 percent of the oil. They used absorbent pads to soak up the remainder of the oil. I will return on 1.4.19 to ensure compliance.
