0531222711 2018-08-09 Salt Water Injection Well Violations Noted While walking to the injection well for quarterly UIC inspection a scald area was observed extending North West of the well (GPS N39.62360/W083.03069) 132 feet by 45 feet which then enters an intermittent drainage that is 4 feet by 84 feet, A second scald area 18 feet by 16 feet at (GPS N39.62352/W083.03024) the intermittent drainage extends another 57 feet where a third scald area is 117 feet by 36 feet. (GPS N39.62316/W083.03007). Field tests of the soil with High Range Quantabs revealed Greater than 6298 ppm Cl. A 2 inch union on the plastic to high pressure hose connection appears to be new stainless steel. Bob Atha was notified of the scald area and the leaking bull plug on the injection tubing at 11:49 am by phone. Bob came to the location and shut in the injection pump. I called ODNR Spill/Response on call and reported the incident at 11:57 am.

The wellhead slip ring has inhibitor staining that extends down the 4.5 inch casing. The annulus has zero pressure. Bob Atha was notified that he needs to do a mini test on the annulus of at least 300 psi for 15 minutes. Bob stated that the annulus froze and pushed fluid around the wellhead slips. Bob stated that he has tightened the wellhead slip ring.

Identification posted at the well and facility. The 2 steel injection line is leaking at the area outside of the containment dike where the steel injection line goes into the ground. Discolored soil and salt residue were observed. A field test of the soil revealed over 6215 ppm Cl (Max out high range Quantab). I contacted Bob Atha and he sent over a crew to meet with me on site. Contaminated soil is to be removed and taken to an EPA approved land fill. The injection operations are shut down until repairs are made. Injection line test scheduled for 6-30-16.

- Producers Service Corp. pumped 5 gallons of inhibitor and 300 barrels of fresh water down back side of tubing. Pressure well to 1100 PSI and tested for 15 minutes with no pressure lost, pressure was bleed down to about 810 PSI. Well test passed.

A brine drip was noticed on the 2 hammer union on the injection line at the wellhead. I informed Bob Atha about the issue and he responded by coming to the well site and replacing that section of injection line with a high pressure hose. Bob tested the line and called me. I returned to the well and witnessed the line being used at normal pump pressure with no leaks observed.

With less than 5% loss in pressure. I advised Dave and Bob to shut down the well and to do the test. I also advised them to remark the ID sign at the storage tanks, and replace the inline flow meter. Well is to be tested 10/18/02. 10/18/2002 Tested the annulus at 500 P.S.I. for fifteen (15) minutes. No loss in pressure. Well passed.
