1236273555 2009-03-04 Drill / Deepen / Reopen Violations Noted - Received phone call from Wes Drake of the Ohio EPA that brine water was over flowing from a frac tank. Mr. Poling son had place a small portable water pump on the south side of the pit and was pumping water from the pit to the water tank. He left the well site with the water pump running. The pump had overflowed the tank and the water was running down the slight grade back toward the pit.

- While I was on site two (2) brine water trucks arrived on site to haul the water away. The water was being taken to Junction City for disposal. Photos taken. Mr. Poling will have the site cleaned, pit pumped out and filled in. - Contacted Mr. Poling on 3/05/2009 at 9:35 A.M. by telephone and told him that a NOV was issued.

1236862753 2009-03-10 Drill / Deepen / Reopen No Violations -On site for NOV follow up inspection, NOV issued 3/05/2009. Operator has removed water tank, the pit has been closed and the area has been rough graded. Operator has installed an electric pole near the well head. Final grading in the near future, the land will revert back to agricultural farm land.
