0477175250 | 2007-11-06 | Drill / Deepen / Reopen | No Violations | Called out for surface cement job. Arrived on site they were running the 85/8 in the hole. They cross threaded the 6th and 7th joints collar and then tightened it. When they released the slips they lost 6 joints down the hole. Surface job was cancled. |
0702156576 | 2010-03-08 | Administrative Inspection | No Violations | Received call from Bill Mitton. 3:20pm Re: will get identification on wells this week. Will attempt to produce and work with the well this spring. Will be bringing a rig down. If not successful will plug the well while rig is in area. |
0715455708 | 2016-02-15 | Final Restoration | No Violations | I walked to the location of the GPS coordinates and found that they were in a field that has been used for farming of crops. At this time I passed the final restoration of this location in accordance with 1509.072 |
1045331078 | 2008-06-05 | Preliminary Restoration | No Violations | Well site has been reclaimed and pits filled in. Land is now being used for agriculture. Production equipment has been installed. Well needs identification. |
1368805676 | 2013-05-17 | Plug / Plug Back | No Violations | Witnessed plugging operations. Packer removed from well and bottom plug spotted through tubing from 1122-338. Top of surface plug set from 102-0. |
1369229796 | 2013-05-16 | Plug / Plug Back | No Violations | Attended the pre plug prep for its eventual plugging the next day (Friday, 05-17-2013). Operator contacted me this morning that they were mobilizing today to start the plugging process on this well. The plugging of this well was delayed originally because the landowner was contemplating taking over ownership of this well. Operator showed up on location with service rig and a truck with flatbed trailer containing a small dozer. When I showed up on location they were removing the production tubing and submersible pump out of the well and getting rigged up to push the bridge plug down hole that was installed between the current producing zone, Berea, and the lower and original producing zone, the Ohio Shale. After pounding on the plug for an hour, the plug released and traveled with gas, water and the apparatus that was used to pound the plug open to the surface. Water and gas shot above the crown sheaves on the service mast. What was considered a bridge plug was in fact a retrievable plug. The company vented gas after they remounted the wellhead. Gas pressure started to ebb and they shut-in the well at approximately 1640 hours. They secured their equipment and left the site. |
1370609212 | 2013-05-31 | Final Restoration | No Violations | Contacted the landowners where this site was located concerning restoration after plugging the well. I talked to Mr. and Mrs. Howard (Shirley) Childers and asked them if they had any concerns about the restoration that Knox Energy performed at the well site. Mr. Child indicated to me that he had no problems with the present condition of the site. He had already plowed and planted soy beans in the area where the well was located. I went to the site to verify the conditions. It appears that the surface where the well was located is in good shape and there was no remnants of a well in the field, or any activity related to the plugging. All material was removed and pit was dewatered and filled in. I asked Knox Energy to submit a waiver form to the landowner for their approval of the restoration since this was not a typical reclamation situation. This field will continue to be used for farming crops on this location. |
1967388369 | 2008-01-11 | Production Wells | No Violations | Production equipment has been installed. Electric service is in place and operating. Electric lines are either on poles or buried in conduit. Whole operation looks good. Well is not connected to production lines yet. They are in a testing phase right now. |
2066541035 | 2007-11-07 | Drill / Deepen / Reopen | No Violations | They recovered all 6 joints of 85/8 that had been dropped. Spud date 11-5-07. TD 618 42 conductor. Ran 600 of 85/8 and cemented to surface. Good circulation and cement returns to the surface. Cement did drop back after pump stopped. They are going to grout it in during the production cement job. |
