0960724872 2007-10-28 Plug / Plug Back No Violations Bowman has excavated out around the hole and is building it up with large stones. No water is coming out of the well at this time. Drill pipe is still in the hole. Conductor is still in the hole. Pit has been filled in does not look like the pit was pumped out before it was filled in.
1194445027 2007-10-26 Drill / Deepen / Reopen No Violations Spud date 10-25-2007. Air Rotary only ran 42 of conductor. Permit called for 60 min. if on air. TD 383 at 375 hit a large amount of water, The water filled up the 7 flow line. Some water came up on the back side of the conductor. Site started to subside. Driller cut off the drill pipe in the hole and skidded the rig away from the subsidence. A 40 area around the well site dropped 24-30. The pit water measured 1000ppm on a High End Quantab. They have to fill in the subsidence with large rocks. This has to be done before a rig can move on to the site.
1204124612 2008-02-27 Administrative Inspection No Violations I had received a call from Mark Bailey Paxtons Operations manager Re: has not been able to talk to Mr. Brust. Mark leaves messages and no one returns the call. I called the Brusts at 9:45am on 2-27-08 and talked to Mrs. Brust and gave her an update on their complaint. I told her that the electric to the well is shut off so it is not a hazard to their live stock. I told her about the restoration plan and the 9 month time frame to complete it. I informed her Paxtons plan to bury the electric line at the well head during restoration. I told her that the gravel on the road to the water tower is temporary and that Mark told me they will fix the road back to original condition during the restoration. I asked Mrs. Brust to please give this message to Mr. Brust and that they should call Mark soon so that the issue with the dead cow can be resolved. I gave Mrs. Brust Mark Baileys Cell phone number and My cell phone number. I told her to call me if they have any questions or problems in the future.
1213624063 2008-06-05 Final Restoration No Violations Restoration has not been completed yet. Site is saturated and so is the field leading to the site. I have contacted Paxton Madison and they are waiting for the ground to dry out. Then they are going to finish both the 20091 and the 20093. They are going to finish them at the same time. Plastic pit liner pieces are all over the ground.
1222890300 2008-10-01 Final Restoration Violations Noted Final restoration is not done. Pit liner pieces are sticking out of a pile of soil and drill cuttings. The disturbed area is 170 feet by 145 feet. 5 gallon buckets are partially buried on location. The top soil is in a pile. A soil sample was taken of the disturbed site. I received a call from the land owner Chris Brust on 9-30-08 he wanted to meet with me on location. I had a meeting with Mr. Brust on 10-1-08 to discuss the restoration violations. Mr. Brust is upset and wants take the companys bond. I explained that he cannot take the bond him self. Mr. Brust wants to approve of the contractors before they start the work. Mr. Brust also stated his concern for the access road to the water tower. The road has been recently resurfaced and is in good shape.
1227102903 2008-10-06 Final Restoration No Violations Meeting on location with Dale Stewart (contractor for Paxton Madison) We went over the scope of the work to be done. Remove the contaminated soil and replace it with good soil from a permitted location. Seed, lime, fertilize and mulch the site. I called Chris Brust (landowner) before the meeting and after the meeting to keep him informed on the progress.
1313871779 2008-10-24 Final Restoration No Violations Site graded. Seed and mulch applied. All debris removed from location and taken to landfill. Access road looks good. Took pictures of site.

1629240166 2007-10-30 Plug / Plug Back No Violations Plugging lost hole. Tried to run 2 tubing down the hole. Could only get 2.5 joints down the hole. Getting hung up right bellow the conductor. Had to use 4 drill pipe to place the plugs. Set 3 plugs 100 each total of 300. Could not tell where the top of cement was at with the drill pipe and decided to stop plugging and wait on the service rig to finish the plugging.
1780412241 2007-11-20 Plug / Plug Back No Violations Ran tubing down and TD top of cement plug at 50. Circulated hole and pumped cement through tubing. Cement stayed at the top.
1820343846 2007-11-19 Plug / Plug Back No Violations I arrived on location and the service rig was set up on the Brust 1-A well completing the production casing cement job. I talked to the rig hands about the method they planed to use to finish the plug job on the #1. Dick Pulling said that he did not want to TD top of the cement and to just fill it with cement from the top. I said no they need to find the top of the cement and go from there. I told him that it should be 80 to the top of the cement if no fill up occurred. We compromised by using the 2 tubing to find the top of the plug. They did not have enough 2 tubing on site to do the job. We rescheduled for the next day. I suggested they bring a total of 120 of tubing to allow for any loss that could have occurred.
