0102153771 2011-06-20 Orphan No Violations Meet with EPA and OEPA on location to discuss well and any help with funding plug job. Plastic barrel has been suspended by wires into the brine pool. A garden hose is attached and connected to a valve and another hose that is coiled on the ground. Gas meter measured LEL of 26 on gas in hose.
0145127286 2014-04-15 Orphan No Violations I met on location with Chris Corso and Doug Rennicker from WhipStock and DOGRM Inspectors Kip Thompson and Josh Garland to view the site for a plugging estimate. Because the well is producing so much water, they requested that we accurately meter the water output from the pit. I notified our hydrogeologist in Zanesville and he is locating a Weir meter to obtain these measurements. We discussed plans of where and how to store to fluids from the well and how to best proceed to get the well plugged. After we supply the water output readings, WhipStock will supply us with a scope of work and an cost estimate.

Filmed multiple zones that are leaking gas and brine. Took water sample at GPS N39.01023 / W083.06423. Photos. Talked to neighbor across the street and he showed me the closest water well to this site. Water well GPS N39.01023 / W 083.06423.
0894351818 2010-02-03 Administrative Inspection No Violations Received a return phone call from John Wynn. He has agreed to sign the right of entry form and have the well plugged through the orphan program.
1021233958 2013-11-19 Orphan No Violations My inspection this day with inspectors Morgan Button, Josh Garland, and Pat Shreve. Our goal was to find the location of the wellbore in the pit and gather other information needed to draw up a plugging plan. With the aid of a 20 long PVC pipe, we tested the distance to solid rock and located what we believed was the hole. The pit is 29 x 28 and prolifically bubbling gas. The previous test of the water showed it was brine and we estimated the well flowing at the rate of 50 gal/min.. We discussed options and came up with a plan to proceed. See Wynn write-up 11-19-13 on M/drive under Wynn. I also stopped by the landowners house but he was not home.
1240326390 2009-01-28 Administrative Inspection No Violations Received a call from John Wynn Re: Orphan well on his property in Pike County, Newton Township. Well is producing gas and brine. Brine is flowing into Sunfish Creek, a tributary of the Scioto River. An appointment to meet Mr. Wynn was set for 2-5-09 at 3:30pm.

Four attempts to E-mail John at the above address using different variations (caps, noncaps) each was sent back to me with message of no such account.
1376054318 2013-08-01 Orphan No Violations Accompanied idle and orphan supervisor, engineer, hydrologist and county inspector. Assisted with water sampling. Updated group on site history.
1384955832 2013-11-19 Orphan No Violations Met Cindy Van Dyke, Pat Shreve, and Josh Garland and escorted them to the wellheads original location. After some time spent on looking around the pit the creek and discharge point from the pit into Sunfish Creek, Pat and Josh rigged up 20-feet of small diameter PVC and went around the pit to probe the bottom of the pit to determine if there was any bottom irregularities that might suggest where the original well casing might be located in the pit. Found a couple of areas in the middle that were possibilities. Then Josh and I rigged up a clothes line across the middle of the pit. Pat brought a fishing pole and reel with a feet-meter gauge to measure the length of fishing line deployed from the reel. We then attached a pulley to the clothes line, spooled the fishing line through the pulley, and tied a weight to the line. We then moved the line so the pulley was over the middle of the hole. Then we lowered the weight to sound the bottom where it was suspected the original casing was located. The deepest depth probed was 17-feet after several attempts moving the probe around the middle of the hole. We discussed the next step necessary to determine the location of the well in this pit.
1384974407 2013-11-19 Orphan No Violations Well is washed out with a hole at surface approximately 30 across. Inspectors and I took a 20 section of PVC pipe and ran down every 2 across the edge and across the middle and determined that there is multiple sections of rock. The PVC pipe would go horizontal from south to north only. An Excavator hoe would need brought in and clean out area. The pond of brine is being fed by an aquifer that feeds about 50 gal per minute. If the water is extracted out, then look for the bore hole for replugging.
1385576471 2013-11-19 Orphan No Violations Accompanied Cindy VanDyke, Pat Shreve, and Morgan Button on field inspection at well site. Parameters measured of phenomenon and plugging/remediation options discussed.
1397588760 2014-04-15 Orphan No Violations I met with Chris Corso and Doug Rennicker, they are with Whipstock. Whipstock is looking at bedding on plugging this well. Cindy VanDyke was also on site. I tested the water using a high range quantab. The reading was 9 on the range. It was decided that a flow meter would be needed to obtain a accurate reading of the gallons per minute of flow from the well. I will obtain a flow meter and monitor the flow to obtain a reading.
1397588868 2014-04-15 Orphan No Violations Met with Chris Corso of Whipstock Natural Gas Services to discuss plugging operations and brine management of well.
1428440688 2015-04-07 Orphan No Violations My inspection today was conducted with Bob Lane of Bob Lane Welding. Mr. Lane viewed this well and addressed the issues of plugging it. Methods of draining the pit were discussed as well as locating the well bore and a casing program that would follow. I provided Mr. Lane with the available information on this well and gave him a log of an adjoining well. He will study this information and come up with a plan and a rough cost estimate to plug this well. The well continues to release brine water into the pond, overflowing down and into Sunfish Creek, a tributary of the Scioto River. A stead rate of gas was being released into the air from the pond and nothing seemed to have changed from the last time I visited the location.
1435256895 2015-06-25 Orphan No Violations Accompanied South West Region Geologist on inspection of the site. We collected 4 water samples. 1. upstream of the discharge, 2. at the point of discharge into Sunfish creek. 3. down stream of the discharge. 4. at the brine pool. Samples will be taken to the ODNR Lab for analysis.
1454431166 2016-02-01 Orphan No Violations Inspected site with Inspector Matt Fulk.
1456172015 2016-02-23 Administrative Inspection No Violations In an effort to contact and talk to Mr. John Wynn regarding the plugging of this well, I have tried to phone him numerous times with no response. Each time I have let a message telling him we would like to talk about plugging this well and asking for him to contact me. I have called him at 740-970-0528 on 2/3/16 at 12:10 PM, 2/4/16 at 8:15 AM, 2/5/16 at 8:20 AM, 2/9/16 at 1:45 PM and 2/19/16 at 2:00 PM. I always left a message and have not gotten a response. On 2/23/16 at 11:00 AM, I called and left the message that we would be done on his property on Thursday, 2/25/16, discussing how to plug this well and would like to meet with him @ 11:00-11:30 AM. Hopefully, he will respond to the message and meet with us.
1465100377 2016-08-02 Orphan No Violations On location to show this orphan well to DOGRM Surveyors Rob Lowe and Tom Smith. This location is being evaluated for possible aerial mapping drone testing. No change in the well status was observed. It is discharging large amounts of brine to Sunfish Creek and gas to the atmosphere. Photos were taken.
1502288029 2017-08-08 Orphan No Violations Orphan well field work, accompanied by OWP Manager Gene Chini and ER Section representatives Nate Lane, Jason Blocker, and Trisha Cartmell: The purpose of this site visit is to run a FLIR Camera on the large brine-filled depression in attempt to identify a potential wellbore. Results of running the FLIR camera are inconclusive in identifying a significant gas concentration, however the wellbore is still assumed to be near the center of the depression. Options were discussed for dewatering the depression, finding the wellbore, and prepping the well for plugging.
1502738116 2017-08-10 Orphan No Violations At time of inspection well was discharging water into Sunfish Creek and gas appeared to be venting in several locations in pit. Accompanied Eric Conrad on the inspection.
1502769345 2009-02-05 Administrative Inspection No Violations Received a message from the Jackson office that Mr. Wynn is sick and cannot make our meeting set for 3:30pm.
1768599340 2012-08-16 Orphan No Violations Accompanied by Morgan Button MRI 2. Took pictures and video of the brine pool and the brine flow into Sunfish Creek. Tested brine with High End Quantab results were 6 on the strip. 1480 ppm Cl. Stopped by John Wynns house and left a copy of the Right of Entry form along with my business card. Accompanied by Morgan Button MRI 2.
1977732555 2009-05-21 Orphan No Violations I&O inspection With Robert Stonerock. High end Quantab showed 9 on the strip. That is off the chart on the bottle. Flowing brine into Sunfish Creek. This site would be good for a down hole camera. I will schedule one with Tom Tomastik.
2068527084 2009-02-06 Orphan No Violations I was unable to contact Mr. Wynn by phone. I inspected the well location alone. The inspection revealed that the site has a fence around a 20 foot pool of brine that was bubbling gas at a very steady rate. No casing of any kind was observed. The weather conditions during the inspection were 20 degrees and partially sunny. The brine pool had no ice in it. The brine pool is clear and appears to be deep. You can see down 6 feet with out seeing a bottom. The brine pool is 3.5 feet below the surface and 20 feet in diameter. Brine is discharging at a rate of approximately 500 gallons a minute. This discharge is flowing directly into Sunfish creek a tributary of the Scioto River. The spill way has a 2 foot wide stream flowing over some sand bags that have been placed at the edge of the pool. A sample of the brine at the surface of the pool was collected for field analysis with High End Quantabs. The brine measured 9 on the Quantab. Quantab units on the bottle end at 8.2 which is 6075 ppm Cl. The field sample exceeded this. I was able to find the well card for this well. It is not complete but does have a note on bottom that reads This old well is reported to have been drilled about 1905-06. When uncovered in 1929 the top of the well was marked by a large circular opening probably 20 ft in diameter from which much water and a little gas was being given off. This well is reported to have a depth of 2200 ft. Well to Trenton Limestone still produces gas and Mineral water. Pictures and a video were taken during the inspection.
