I documented that the well is producing into a Dominion/East Ohio Gas line and took pictures inside the meter run showing production and that the chart was changed out by Tri State Measurement on 10/01/13. The operator has not reported production since 2009. The ID sign on the tank does not meet urban production well requirements and indicates Alsid Oil & Gas as the owner of the well. The well is a swab well and there is a 100 barrel tank on location inside a dike and the separator has been removed from the location. |

There is an obvious odor of natural gas inside the meter shack. The sixteen day chart was changed seven (7) days ago and the well is being produced in violation of Chiefs Order 2017-441. The required ID is not posted at the wellhead or at the tank battery at this time. |

The required ID is not posted at the wellhead or at the tank battery at this time. I will ask Mr. Svetlak to post the required identification immediately. |
