0960887762 2016-10-25 Final Restoration No Violations I inspected this location at the request of Regulatory Compliance Specialist, Linda Ray (Range Resources), to assist in determining if this well has been plugged and abandoned so it may be removed from Ranges performance bond. The Division of Oil & Gas currently has the well listed as producing in the RBDMS database and the last commercial production was reported in 1996 by Lomak Operating Company. Lomak was issued a plugging permit on 12/14/1995 and the permit expired on 12/13/1996. There are no plugging records on file with Division and this well was transferred to Range Resources-Appalachia, LLC on 6/27/2000. I spoke with the Mahoning County Inspector, John Fleming, concerning this well, as he was employed by Lomak Operating during this time period. John told me that this well was plugged and abandoned in 1996 and he remembers being on the plugging job. This land was originally a farm and today the location is a residential, apartment complex. I reviewed historical imagery on the Portage County Auditors website and was able to determine the location of the former tank battery and lease road. The lease road ran north to south across the property approximately 350 east and parallel to the west parcel boundary. The tank battery would have been in the green space between the first two (2) apartment buildings to the north. The wellhead would have been located in the green space behind the last apartment building at the southwest corner of the parcel. My inspection today found no physical evidence of the existence of the former wellhead, production equipment, or lease road. Based on site review and collected evidence, this well was plugged and abandoned in 1996 by the Lomak Operating Company under the issued permit. No further action will be required to be performed by Range Resources-Appalachia, LLC. Final Restoration is approved.
