0158437265 2013-02-13 Urban Drill / Deepen / Reopen No Violations I was on location to check the status of the oil spill clean-up and reconstruction of the dike and tank battery. All affected materials have been removed and the location is clean. The weld on the 210 barrel tank has been repaired. The dike has been recontructed in the excavated area of the the old battery and is below grade in a good, clay rich soil. A heavy duty liner has been placed inside the dike and covered with gravel. The 210 and 100 barrel tanks have been spotted back in the diked area and the flow lines are being constructed. The siphon dam remains on the south side of the battery and has no residual oil in it. No violations.

Pumper Mike Schrock was across the road collecting absorbent pads that had been used to collect the oil that had got off location. The site was in good condition, the dike and tank battery were completely rebuilt and all oil had been cleaned up on the location. No violations.

All affected soil and materials have been removed from the location. The dike and tank battery have been rebuilt and plumbed to the pumping unit. The siphon dam has been removed and there are no signs of free oil on the ground or seeping from the location. The clean-up of the ditch and drainage continues under the supervision of the Ohio EPA. No violations.
1280386316 2013-02-05 UrbanProduction Wells No Violations I was on location to check the clean-up status of an oil spill. Don Caproletti (contractor) was on location and had constructed a sump with a siphon dam on the south side of the tank pad. The sump and dam were working properly and oil was being collected in the sump. The oil was being removed from the sump with absorbent pads. The affected soil on the west side of the lease road had been removed and hauled away. The remaining soil and rock from the tank battery will be properly disposed of today. The clean-up is progressing and no oil or sheen can be found on the lease road or Lake Rockwell Road. No violations.
1395727716 2013-01-31 UrbanProduction Wells No Violations I received a call the previous evening from Northwood Energy about an oil spill at this well. I was informed that a barrel of fluid got off the location, down the lease road, and was contained. I arrived on location and found oil ponded inside the tank battery. There was oil ponded immediately outside the south side of the dike and I followed a trail of oil in a low berm along the west side of the lease road for about 200 feet. Absorbent pads had been placed along the berm and around the tank battery and dike. I walked down to Lake Rockwell Road and there was a noticeable sheen running out onto the road and entering a roadside ditch to the east of the lease road. There was a boom in the ditch with pads that had caught some free oil. I followed the ditch to the east for 200 and found two more areas with booms and pads that had absorbed oil. Several feet past the last boom, there was a culvert carrying drainage to the south under Lake Roackwell Road. I went back up to the well location and met with Wade Balser (Ohio EPA) to discuss the spill. I explained to Wade what I had found and we walked back down the road to examine the ditch. We crossed the road, followed the drainage to the south, and found a boom and pads in front of some cattails and there was obvious oil staining on the cattails and absorbents. It appears the cattails impeded the free oil from continuing down the drainage ditch, but a sheen from the oil was noticeable past here. We continued to the south where the ditch emptied into a small creek and found another boom and some pads. We followed the sheen for another 500 to the east to where there was no noticeable sheen on the water. There were several more booms placed along this leg of the creek to absorb the sheen. We returned to the well location and I called Bill with Northwood to inform him that he needed to report this to the OEPA spill line even though they were already aware of the spill. 1485589275 2013-02-07 UrbanProduction Wells No Violations I was on location to check the clean-up status of this oil spill. Sutton Pump was on location steam cleaning the 210 barrel stock tank so that it could be repaired. The affected soil and rock had been removed from the location. I spoke with Don Caproletti (contractor) and he was going to leave the sump and siphon dam in place to contain any residual oil that may seep out of the affected area. The well location looked good and there was no visible oil or sheen on the lease road or migrating off the location. No violations.
1743013449 2013-02-04 UrbanProduction Wells No Violations I was on location to check the status of the oil spill clean-up at this location. Don Caproletti (contractor) was on location and digging out the contaminated soil. About 3 of soil and rock had been removed from the tank battery area and stockpiled at the northeast corner of the pad for hauling. I told Don to build a sump with a siphon dam on the south side of the dike to catch the oil leaching from the pile and from the dike. We discussed how to properly rebuild the dike. I noticed that the soil beneath the dike was a clay and suggested he use that for the new dike. The excavated area formed a natural berm that could be used as the new tank pad. I was satisfied with the progress of the clean-up. No violations.
1912624232 2013-02-01 Administrative Inspection No Violations I spoke with Chris Schofield (Northwood) this morning about the clean-up status of this oil spill. Chris informed me that when they dug the sump hole to steam the oil out of the tank battery it was saturated. I told him that they would need to remove all of the affected soil and haul it to an approved landfill. He told me they would start excavating later today and start hauling on Monday.
