0015379141 2002-05-09 Orphan No Violations 5-6-2002 - Received a phone call from Jason Hatfield (H.A.D.) concerning a 10 dia. Csg found in Annevar Development, a new real estate development. Inspector Claus located the casing within the foundation footer of a new home being built at 1035 Brittingham Drive. Inspector Claus questioned the workers who were laying cement block at the site. They said they looked into the casing and it was bubbling, so they filled it in with concrete to seal it off. Joe Petit (330-819-3939) of Mosley Building Corporation (330-633-7502) arrived at the site and discussed the situation with Inspector Claus. Inspector Claus informed Joe Petit that the casing should not be covered up and that the foundation footer should be moved. Inspector Claus located the well on the ODNR well spot map. It appeared to be a plugged well with the permit # 34-133-2-1133-0000 located in lot 4 South of Ravenna Township. 5-8-2002 - Joe Petit called Inspector Claus and informed him that the casing was bubbling again and that he was able to ignite the gas. 5-9-2002 - Inspectors Claus and Patrick inspected the casing and took a GPS reading; Lat. N 41.14329, Long W 081.26116. It was determined that the well was in fact the plugged well, Willard #1, permit # 1133. Inspector Patrick took digital photos of the site, the casing, and the casing location. Inspector Claus verified that there was gas bubbling up through the cement on the inside of the 10 dia. Casing. Inspector Claus informed Joe Petit of the inspection findings. Joe Petit stated that construction would be stopped and that Mosley Building Corporation would not be building on that lot. Inspector Claus will conduct additional research to determine the future plugging responsibilities for this abandoned well.

1039009967 2002-12-03 Orphan No Violations This leaking plugged well was going to be placed on the orphan well list. The current property owner, (Carter Lumber, 601 Tallmadge Rd., Kent, OH 44240; contact Howard Kline 330-673-6100 Ext. 433), optioned to vault and vent the well at Carter Lumbers expense. On 12-03-2002, Doug Gonzalez and his crew saw cut and removed the footer of the partially constructed garage; dug down an additional 4 feet; cut off the 10 dia. casing; constructed a vault, vented it, and cemented it in place; leaving a 2 dia. threaded riser vent pipe extending up 5 feet from the ground level at the north side of the remaining footer. Inspector Claus witnessed the entire procedure. THE SITUATION HAS BEEN RESOLVED. GPS coordinates for the well N 41.14329, W 081.26109.
