0473303542 2018-11-01 Production Wells No Violations This inspection was conducted as a follow up to Complaint # 5999. When I arrived on location Moore Well Service had previously dismantled the tank battery and placed the 100 barrel tank on containment and started excavating underneath the tanks prior location. The tank was steamed out and the fluids were removed by a vacuum truck from Duck Creek Energy. Moore Well Services (MWS) dug an area 15 X 20 X 18 to remove all the contaminated material. The excavation was landed in the yellow clay, a member of the Hiram Till. The excavated material was placed on plastic liner and encapsulated to prevent migration. I requested that the excavation stay open to allow any seepage out of the stone tank pad. I will re-inspect to assess the level of remediation.
0983213605 2018-11-05 Production Wells No Violations I conducted a follow up inspection to assess the clean up after a valve failed and released tank fluids on the tank pad. The excavation, below where the faulty valve was located, was full of water with just a sheen of oil. The weeds were cut around the pumping unit and on the dike surrounding the tank battery. I spoke with Brooks Yarborogh, who indicated that the water will be pumped out and the excavation filled with 57 sized gravel and the tank battery built.
1541445609 2018-10-31 Production Wells No Violations I responded to complaint (# 5999) regarding oil and gas leaking out of a tank at the tank battery for the Viall-Newhouse # 2. Upon my arrival I met with Fred Romeo ( Emergency Response DOGRM ) and Jim Viall ( complainant ). The three (3) load line valve had broken on one (1) of the 100 barrel stock tanks and allowed some of the tanks contents to leak on the stone tank pad. Brooks Yaraborogh also arrived on the location. Romeo estimated that the amount of release was less than five (5) gallons from the spatial extent of the contaminated stone. I looked into the tank and could not see the fluid level. I ordered that the tank be removed and set on plastic and the area underneath the tank be excavated until clean material was found. All contaminated soil and gravel was to be placed on plastic and encapsulate for containment until disposal.. The overgrown vegetation at the tank battery and wellhead was to be cut and maintained. Brooks said, that Moore Well Service has been contacted and would be on location tomorrow. I will follow up and observe the remedial work tomorrow
2044973516 2018-12-04 Production Wells No Violations The tank battery was still disassembled and the removed tank pad material was encapsulate in plastic. The Operator was waiting for more favorable weather to re-construct the tank battery. Called Operator to request a time frame for completion, but had to leave a message for a return call.
