1032233995 2013-12-10 UrbanProduction Wells No Violations I inspected this well to follow up on Chiefs Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insurance as required by ORC 1509.07(A) and has been found in material and substantial violation for failing to maintain coverage. My inspection found the wellhead equipped with a 4x2 swedge with a ball valve. The well is currently shut in and produces to a 300 barrel tank. The tank battery contains a brine vault that is not in use and the dike around the battery does not meet SPCC requirements. I did not find a separator and there is no ID on any of the equipment.
1494951494 2017-05-16 UrbanProduction Wells No Violations I inspected this well location as a follow-up to Chiefs Order # 2016-171. At the time of this inspection, I observed the following: a wellhead with 4 to 2 reduction swage which had a ball valve that was closed. No other flow-line or supply-line appurtenances were in place. There was an associated storage tank battery located adjacent to the wellhead. The battery was comprised of one 210-barrel steel, stock tank. The areas surrounding the well and battery were heavily overgrown with vegetation and trees, with tree limbs and other vegetation intertwined within the battery making approach and inspection difficult. The visible elements were in disrepair. I did not observe any other signs of leak, spills, or impact to the surrounding areas. The gravel-paved access road that extends west from the sand and gravel operation was overgrown in many areas and in poor condition with numerous wash-outs. There has not been an annual production report filed since 2003.
1653870242 2007-04-24 Production Wells No Violations The last reported production from this well was in 1992. Svetlack acquired the well in 2005. A NOV will be issued if the well is not placed back in production or plugged and abandoned in the next six months.
