0000PL6203 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
0669958848 2005-11-09 Production Wells Violations Noted I received a call from Jeff Moore with Moore Well Service reporting an oil spill on the south side of Calvin Road directly east of the Silver Creek overpass. Other agencies contacted included Edinburg Fire, Ohio EPA (Jim Erwin responded) and West Branch State Park (Officer Corl responded). The spill resulted after the separator for this well failed, putting oil in the salesline. The plastic salesline then pressured up and split releasing the contained oil. The oil ran downgradient to the road ditch on the south side of Calvin and into a low area adjacent to the creek.. Moore estimated the release to be three to four barrels. The release impacted approximately 200 feet of road ditch and several isolated portions of the low area. It does not appear that any oil reached the creek. Moores initial response included shutting in the wells, constructing a siphon dam in the road ditch and recovering oil with a vacuum truck. Secondary response included placing booms across Silver Creek, placing sorbent pads in the impacted low area and constructing one additional siphon dam. By the end of the day, the spill had been stabilized. Planned activity for the next several days includes leak repair, product recovery and pressure washing the road ditch.
0867618409 2005-11-14 Production Wells Violations Noted The majority of the site remediation has occurred. I contacted Jeff Moore and requested that additional peat be applied to the area below the last siphon dam. I also asked him to contact Jim Erwin with OEPA before removing the booms across Silver Creek.
1104912961 2005-11-18 Production Wells Violations Noted Follow up inspection. The affected area has been stabilitzed and additional peat has been applied to the low area.
1538232451 2005-12-28 Production Wells Violations Noted The violation has been corrected.
1663243739 2005-11-10 Production Wells Violations Noted On this day, Moore personnel were on site with a minihoe and steamer to clean out the ditch, install another siphon dam and collect oil.
