0427582386 | 2015-09-09 | UrbanProduction Wells | No Violations | I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a 4.5 plunger lift assembly. The tank battery consists of a 210 barrel production tank, a 100 barrel brine tank, and a mechanical separator. There is a dike around the tank battery. There is an identification sign posted at the well; however, no identification at the tank battery. There are locked, chain link fences surrounding the tank battery and well. The lease road is in stable condition with a locked gate at the entrance. The well was placed into production prior to House Bill 278. The well shares the tank battery with permit number 2-3040. I called and instructed Bill Kenney to post an identification sign at the tank battery. |

1119451900 | 2005-06-06 | Production Wells | Violations Noted | On June 6, I received a call from John Fleming with Great Lakes reporting a salesline leak that occurred on Sunday June 5. Their 3 salesline crossing the Sea Lakes campground (owned by Geauga Lake) ruptured and released natural gas and oil to the environment. The release impacted a nearby quarry pond, several trees and ground vegetation. The amount product lost was estimated at less than one barrel. The Aurora Fire Department received the first call and in turn notified OEPA and Great Lakes. Reggie Brown with OEPA-DERR responded. Geauga Lake contacted Safety Kleen who in turn contracted with Enviroserve to initiate the clean up. The cleanup consisted of removing several impacted trees and placing a containment boom on the quarry pond. The cleanup was well underway when I arrived on Monday afternoon. The line will be repaired. |
1119836543 | 2005-06-24 | Production Wells | Violations Noted | Met on site with John Fleming and Woody McDaniels with Great Lakes to discuss the spill remediation at this site and their natural gas gathering system. Also on site was a crew from W. Pol Contracting to repair the salesline. According to information and maps provided by Great Lakes, the 3 pipeline originates near the Portage/Geauga County line, collects gas from the Sea World #1 (P-2918) and Sea World #5 (P-3054) wells, turns southeast to the north side of Treat Road, collects gas from Sea Lakes #1 (P-3039) and Sea Lakes #2 (P-3040) and continues east along the north side of Treat Road to a Great Lakes compressor station then to a Dominion East Ohio meter (designated A-520) located on the north side of Treat Road, at the entrance to the Twinsburg Development sand and gravel pit. Between the point of origin and the Dominion East Ohio meter, the natural gas is controlled by Great Lakes Energy. |
1444751380 | 2015-09-22 | Temporary Abandoned Annular Disposal | No Violations | I was on location to check for the use of annular disposal at this wellhead. My inspection found the well operating with the aid of a 4 plunger lift system. The wellhead was fenced and the proper ID was posted on the fence. This well produces to a diked and fenced tank battery to the south that contains a horizontal separator, one (1) 100 barrel tank, and one (1) 210 barrel tank. The proper ID signage is posted at the tank battery. The access road gate is locked. I found no evidence of annular disposal being used at this time. No violations. |
