1406463516 2014-03-11 UrbanProduction Wells No Violations I inspected this well to check the idle status of this Berea Sandstone well. The last commercial production reported from this well was in 1988 by D. A. Waldron & Associates and no commercial production has been reported since ownership was transferred to David A. Waldron in 2007. My inspection found 7 drive pipe just above grade and 4.5 production casing equipped with a 4 X 2 swage and ball valve. The valve was open and a pressure gauge on the on the wellhead read 78psi. A 1 steel line came off the wellhead and was equipped with a plastic flow line for a domestic feed. The 2 flow line was disconnected and there is no ID posted at the wellhead as required by law and no associated production equipment on location. Based on the fact that no commercial production has been reported from this well in 26 years, David A. Waldron will, within 180 days, obtain a plugging permit as required by section 1509:13 of the Ohio Revised Code and properly plug and abandon the well in accordance with section 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action including issuance of a Chiefs Order and bond forefeiture.
1516370159 2018-01-19 UrbanProduction Wells No Violations Follow-up to Chiefs Order #2016-311 for failure to maintain liability insurance and Compliance Notice #1406463516 requiring this well to be plugged, within 180 days, and that the required identification posted. Division records indicate that as of 1/19/2018: 1) the required insurance has not been obtained by David A. Waldron and filed with the Division, and 2) this well has not been transferred to another owner meeting the requirements outlined in the Chiefs Order. The Chiefs Order requires David A. Waldron to immediately suspend all oil and gas operations in the State of Ohio. My inspection today found that the required ID has not been posted and that the well has not been plugged as required by the Compliance Notice. A gauge on the production tee indicates a pressure of 40psi and the two (2) inch flow/sales line is disconnected. The well is currently not in production and the valve on the domestic tap is closed. This violation continues.
