One truck had an Ohio license plate PEW2187 and the other truck had a Pennsylvania license plate AF25709. A call to Tom Tomastik also found that Power Gas did not submit the required plans for modifying the disposal system. These plans were requested on February 23, 2006. Power Gas must shut down the disposal system until the required pland have been provided and approved and the brine trucks have the required identification.

Hi-Range Quantab analysis of the water inside of the dike exceeded the maximum detectable level. Flushing and recovery will continue until the chloride levels are down to acceptable levels.

On this date, Stamm Contracting was pouring the new concrete pad for the three 210 bbl tanks. 22.5 cubic yards of concrete was poured over a prepared bed of bankrun gravel. The mix was 6.5 sacks per yard with one pound per yard of fiber and two pounds per yard of calcium chloride. Underneath the concrete and over the bankrun gravel was a double layer of 20 mil plastic. Expansion joints were placed around the new pad. The violations are corrected.

A gauge on the annulus between the 4.5 casing and injection tubing showed the annulus to be pressured at 450psi. I found no violations at this time.

I spoke with John Firko and asked him to put more pressure (200 psi) on the annulus. The was some pressure but not enough to register on the gauge. He stated that they have a high pressure pump on one of their water trucks that could do the job. Power Gas has also completed some modifications to the containment system which probably should have had prior approval. The changes include 1) segregating ponds 1, 2 and 3 from ponds 4, 5 and 6 by placing a mechanical plug between ponds 3 and 4; 2) constructing a partially curbed concrete pad west of the ponds for use by trucks offloading directly into pond 3; and 3) failing to provide adequate secondary containment around the concrete pad. I told Mr. Firko to contact Tom Tomastik in Columbus to discuss these changes. I will follow up.

Power Gas submitted a proposal to UIC for temporary repair of the system. The proposal was accepted and I am waiting for notification from Power Gas as to when then intend to begin work. The proposal calls for 1) the removal of the three 210 bbl tanks; 2) lining of the containment area around the tanks with clay and a 20 mil liner and 3) resetting two of the 210 tanks inside new containment area. The proposal calls for continued use of the concrete settling ponds.

1078442613 2011-03-09 Salt Water Injection Well No Violations On 3/7, Rick Keppler with EnerVest Operating reported a tank overfill inside of the containment dike associated with this system. The overfill occurred when a sump pump inside of the containment dike was left on over the weekend. The estimated loss was less than 20 bbl of oil /brine/fresh. The entire volume was contained and is being returned to the tanks with a vacuum truck and a steam truck. Recovery was initiated on 3/7, continued on 3/8 and 3/9 with an EnerVest vacuum truck, Sutton Pump steam trucks and EnerVest employees. Company response was prompt and effective.

The premixed inhibitor was pumped down the annulus and up through the 2-7/8 tubing. Once inhibitor circulated to surface, the Halliburton R-4 packer was set in tension and the annulus was pressured up to 900 psi. The pressure began to fall immediately. Several more attempts also failed before Universal was released. Enervest now plans to test the casing at 500 intervals in an attempt to locate the leak.

Exco provided their treating truck to pressurize the annular space between the 2-3/8 tubing and the 4-1/2 casing. The permitted pressure is 860 psi. Based on the response of the state issued Wica pressure gauge, Exco pressured the well up to 920 psi. After 5 minutes the pressure remained the same. After 15 minutes, the pressure remained at 920 psi. The MIT was a success. A MIT will be completed on the injection line as soon as it is ready.

Universal Well Service mixed and displaced inhibitor which was pumped down the tubing and returned through the annulus. Once inhibitor circulated to surface, the packer was set in tension, the wellhead was tightened and the MIT began. The planned MIT pressure was 900 psi. At approximately 500 psi, the 4-1/2 casing started leaking above the wellhead and below the top collar. The test was called off and the pressure released. Enervest will present a corrective action plan to repair or replace the leaking casing as soon as possible.

The tanks are being connected and the pumphouse will be reconfigured prior to placing the new system in operation. The old concrete storage facility will be steam cleaned and pumped out before crushing and backfilling with limestone.
1285704532 2010-09-20 Salt Water Injection Well No Violations After replacing a leaking joint of 4-1/2 casing and several joints of leaking 2-7/8 tubing, Universal Well Service was on site to attempt the third mechanical integrity test. Universal circulated 1650 gallons of inhibitor down the annulus and up the tubing. Whipstock then set the rebuilt Halliburton R-4 packer in tension at 3784. Universal pressured up on the annulus to 900 psi. After five minutes the pressure was 895 psi and after 15 minutes the pressure was 875 psi. The pressure loss of 25 psi was within the allowable pressure loss and the MIT passed.

I was on location with DOGRM District Supervisor Steve Ochs to conduct quarterly status checks for the UIC Program. The 2.875 injection tubing was currently on vacuum. The pressure on the annular gauge between the 2.75 injection tubing and 4.5 casing indicated a pressure of 0psi. The plug on the needle valve was removed, the valve was opened, and it was found that the annulus was on vacuum. A call was made to Mike Burkhardt (Enervest) to inform him of the issue. He said he would get a pump truck to location and pressure up on the annulus to determine if the tubing or packer is leaking. He will inform the Division when the pressure test will occur so it may be witnessed by a Division Inspector. We will wait for notification.

A brine release at the wellhead has impacted an area several hundred feet long and several feet wide. The entire facility needs to be remediated and operated properly.
