0418570940 2012-05-21 Production Wells No Violations I was on location today to inspect the well due to, two adjacent water well complaints. I had the company expose the annulus and asses the pressure. I meet with Tim Cosma from Cedar Valley. Tim exposed the annulus and it was found that the annulus was open. Three weeks ago the operator repaired a flow line leak by clamping it. The operator also tripped out the old pump and replaced it. Adjacent landowners are claiming that there water issues started at the same time. The well has a pumping unit on it. The area around the well has been affected by the work over and the repair of the line. Operator will need to reclaim affected area. The tank battery was in good repair. The tank batterys inventory was: two 210 barrel production tanks and two vertical separators. Tank battery was fenced and the identification sign was posted.
0681129968 2010-04-21 Annular Disposal No Violations Columbus requested this inspection to determine the current annular disposal status. Annular disposal does not appear to be used at this time.
1093118925 2017-01-19 Temporary Abandoned Annular Disposal No Violations I was on location to inspect this well for the use of annular disposal. My inspection found the wellhead equipped with an electric-powered pumping unit. The wellhead and unit are fenced and the required ID is posted on the fencing. A gauge indicates 76psi on the production tubing and there is a domestic tap on the 4.5 production casing. This well produces to a diked tank battery to the south containing two (2) 210 barrel tanks, two (2) vertical separators, and the associated meter run. The tank battery is fenced and locked, and the required ID is posted on a tank. I found no evidence of annular disposal being used at this time. No violations.

1270146520 2010-04-01 Production Wells No Violations I inspected the yard area. Top soil has been spread over the area of the spill. It will be around 2 weeks before Cedar Valley will plant the pine tree and seed the spill area. This complaint continues.
1372859841 2010-03-31 Production Wells No Violations I received a call from Jay Cheslock at 11:43AM about a complaint from Mr. And Mrs. Tom Zenkewicz of 12777 Chamberlain road. The complaint(#2641) was a oil spill in the southeast corner of there front yard. I called Tim Cosma-Field Supt. Cedar Valley (330-853-9565). He was aware of the spill and headed that way. I arrived on the scene at 1:38PM. Tim and I met and looked over the spill. The spill was approximately 1 quart of oil. Bruderly Excavating was called and will be on the location shortly. Bruderly arrived with the trackhoe and proceeded to dig up the spill area and dumped the contaminated soil in his truck. There were 2 lines in the ground. One line was the production line for well number 3405 and the other was a dead line from the other well of 3399 across the road. A patch was installed on the 2 steel dead line. Tim stated that they would flush the old line at a later date. The hole was filled back in. The owners also talked about replacing a pine tree that was 4 feet to the southeast of the leak. Tim stated that they would do that. He wanted to ground to settle out before replacing the pine tree. Top soil will be hauled in tomorrow. This complaint continues.
