0124361290 2016-11-14 Production Wells No Violations Follow up to Incident # NR11051600000765 that occurred on 11/5/2016. I inspected the location today and found the excavated hole secured with safety fencing. There is six (6) feet of water in the excavation and several small pockets of free oil are present on top of the water. The siphon dams were found clean and a minor amount of sheen was found at the siphon dam at the down-stream side of the culvert. The ditch was clean and the absorbent pads need to be removed. I met with contract pumper Derek Bryant and asked him to put some absorbent pads in the excavated hole to collect the free oil. I will continue to monitor location.
0832472407 2016-11-08 Production Wells No Violations Follow up to Incident # NR11051600000765 that occurred on 11/5/2016. Scott Miller Contracting (SMC) continued with clean-up operations. Water and oil that had seeped into the excavation overnight was removed. The excavated hole was enlarged to the north and no oil was found. The flow line was fitted with a clamp and a siphon dam was constructed between the excavation and the west side of the culvert pipe. I walked the ditch and pads had soaked up some residual oil along the course. There was no free oil found in the ditch or the eastern-most siphon dam. The remaining three (3) tandem loads of affected material were removed from location by Alderman Trucking and Northeast Oilfield Services. The material was disposed of at the Kimball landfill in Dover. I will continue to monitor the location.
1478626717 2016-11-07 Production Wells No Violations Follow up to Incident # NR11051600000765 that occurred on 11/5/2016. I was notified by Matt Pettigrew (Contract Field Supervisor), at 12:57pm, that a 2 flow line leaked approximately three (3) barrels of crude oil into a farm field and the oil migrated into a dry ditch. He said Northeast Oilfield Services was on location vacuuming the free product out of the ditch for disposal and that Scott Miller Contracting (SMC) would be out to remove any impacted soil. I was on location this morning with Emergency Operations Coordinator Fred Romeo to assess the extent and impact of the spill. We found that the spill had migrated much further than originally reported. Oil had migrated east of the release point and through a 65 concrete culvert under the lease road. The oil traveled 650-700 ESE of the culvert and stained the grass and weeds in the dry ditch. SMC constructed a sump and siphon dam at the furthest extent of impact as a precaution and another on the east side of the culvert pipe. The entire length of ditch was scraped and the affected material was stockpiled near the lease road. Emergency Operations Manager Terry Whitehair arrived on location to assist in coordinating the clean-up efforts. The flow line was excavated below where the leak emerged at the ground surface. The flow line was found five (5) feet below grade just north of the dry ditch. The excavation consisted of 2-3 of topsoil and silty loam and five (5) feet of glacial till and boulders. Oil and water were seeping into the excavation from the north in the layer of boulders. SMC excavated a hole 15 north of the flow line to check for oil migration and none was found. The excavated hole was 15 long (east-west) and 6-8 wide (north-south). The impacted soil, till, and rock was stockpiled for removal. It was decided to stop excavating and leave the hole open overnight and let the forecasted rain flush any residual oil into the excavation. SMC then flushed the 65 culvert pipe with 250 gallons
1483634582 2016-12-09 Production Wells No Violations Follow-up to incident # 201600215 concerning an oil release that occurred 11/5/2016. My inspection today found the excavated hole has been closed and the area has been graded. The siphon dams on each side of the culvert have been removed. I observed a minor amount of sheen at the east end of the culvert. I walked the ditch and found no issues. The eastern siphon dam has been left in place and is clean. Pin Oak will need to remove the corrugated culvert pipe used for the siphon dams, the trash bag with soiled pads, and the remaining pads along the ditch. I informed Emergency Operations Manager Terry Whitehair of my findings.
1931571134 2016-11-17 Production Wells No Violations Follow up to Incident # NR11051600000765 that occurred on 11/5/2016. My inspection today found that the excavated hole remains open and secured with fencing. There is a small amount of oil on the water in the hole (< one (1) quart). I walked the ditch and found no pollution or contamination issues. The siphon dam sumps were all clean and functioning properly. The pads need to be removed from the ditch. I will ask Matt Pettigrew (Contract Supervisor) to remove the pads and collect the oil from the excavated hole. I updated Emergency Operations Manager Terry Whitehair on the site status. I will continue to monitor the location until it is restored.
