Inspector John Fleming took possession of the valve following the spill and it was secured at the Uniontown Field Office until this morning. I met with Charles Guinther, Jr. whose employer, Scientific Expert Analysis (SEA) has been retained by the operators insurance company to analyze and determine the cause of the valve failure. I transferred custody of the valve to Mr. Guinther and signed the chain of custody form documenting the transfer. I will continue to monitor progress at this location. |

I received and reviewed the preliminary reports from Hull & Associates, on 10/21/15, pertaining to the soil and water testing performed at this location over the last month. In the last round of testing, all samples came back non-detect or tested below the applicable standard allowed. I talked with Mark Smith (PEP), and gave him permission to begin putting the site back together. My inspection today found that the excavated area has been filled and compacted and there is one pile of topsoil remaining to be spread. A 500 barrel frac tank has been emptied and is on containment. The site is dry and in good condition. No violations. |

I received a call at 9:20 hours from Mark Smith contract supervisor for Pep Drilling Co. Mr. Smith reported a 50 barrel crude oil spill release which occurred on September 8, 2015. Mark explained that at 14:30 hours on September 8, 2015 a water truck pulled brine water off the Pep Drilling Co. Lumber #7-#8-#9 wells common 210 barrel storage tank, at 16:30 hours the well operator showed up on site and discovered free oil inside the earthen dike area of the tank battery. Mr. Smith said he then contacted Scott Miller Contracting to initiate clean-up. Mr. Smith reported 30 barrel of crude oil recovered from clean-up effort on September 8th. I contacted Terry Whitehair North East Emergency Response Coordinator and told him of the release he stated that he would meet me at the location. Mark Smith notified the OHEPA. Scott Miller Construction started digging interceptor trenches to find the leading edge of the oil. Pep Drilling is making arrangements for roll-offs and disposal and testing of the contaminated soil. Personal and a water truck will be on location over night to monitor the trenches. |

The release occurred at the tank battery for this well and the Lumber #s 8 & 9 (PN#s 23901 & 23902 respectively). The tank battery was dismantled and the equipment was moved to the north for temporary storage.. An area measuring 100 X 100 and 10-15 deep has been excavated to remove the contaminated soil. The contaminated soil is currently stockpiled on containment on the north side of the location. Unaffected material has been stockpiled to the east and is being tested. Scott Miller Contracting is on location and is overseeing the excavation and hauling of the affected materials. There are 15 lined and covered roll-off boxes in the field to the south that contain affected soil and has been tested for TENORM (Technologically Enhanced Naturally Occurring Radioactive Material) and cleared to be disposed at an approved landfill. Hull and Associates personnel are conducting soil and water tests to confirm that all the contaminated material has been removed and has not migrated outside of the excavated area. I will continue to monitor progress on this location. |

My inspection found the well equipped with a natural gas-powered pumping unit. The wellhead and unit are fenced with tubular cattle gates and the proper ID is posted at the wellhead. The well is currently shut-in as the tank battery is being rebuilt following an oil spill that occurred on 9/8/2015. The rebuilt tank battery is diked and lined with a heavy duty liner. The battery contains one (1) 210 barrel tank and two (2) horizontal separators. This well and the Lumber #8 (PN#23901) produce through one separator and the Lumber #9 (PN#23902) produces through the other separator. There is a domestic hook-up on the # 7 & 8 separator. The tank battery is currently not in use. No violations. |
