My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. A gauge on the 4.5 production casing flow line indicates a pressure of 110psi. This well produces to a diked tank battery to the west and is shared by the Ebie #s 2 & 4 (PN#s 3208 & 4060). This well and the #4 produce through a horizontal separator and into three (3) 210 barrel tanks. The chart at the meter run indicates a differential production pressure of 70psi. The required ID is posted at the wellhead and at the tank battery. This well was drilled prior to the implementation of HB 278, and due to its remote location in a cornfield and the presence of a locked access gate, urban requirements will not be enforced at this time. No violations. |

The tank left the tank battery and traveled approximately 100 feet to the southeast. The resulting fire was contained in the tank battery by the Randolph Fire Department. Inspector Carl Roberts responded to the site. On this date, PEP contractors were continuing with the cleanup. The damaged 210 bbl tanks and separators were removed from the containment area and will be scrapped. The small scattered pockets of paraffin are being collected and removed. The majority of the cleanup, other than the equipment removal should be completed by the end of today. |

Randolph Co. Fire Dept. received a call from a landowner across the street at around 10:30 AM, 9/29/09. The landowner(Craig Klodt) later stated that he had heard it thundering earlier in the morning. The pumper was called, Steve Pero. Steve called Mark Smith. The tank battery was owned by PEP Company. Bob Whorstall called me, Carl Roberts at 12:30PM to make the call to investigate the incident. Bob was in Columbus for a meeting. I arrived on the location at 1:00PM. The fire department had extinguished the fire with foam. There were 4 stock tanks going north to south. Lightening had hit the 3rd stock tank. The stock tank was launched some 120 feet southeast of the tank battery area. A chain reaction occurred to the other 2 stock tanks and the separator. The separator ball valve failed and the sales line gas came back in and set the separator on fire. There were 15 barrels of fluid that was released from inside the 3rd stock tank that ended up inside the dike. The whole 15 barrels were recovered. The clean up started immediately. There were 2 water trunks and a track hoe. |

| 1611278268 | 2019-03-18 | Production Wells | No Violations | I was notified by Mark Smith (PEP) on 3/13/19 that approximately 15-20 gallons of oil had leaked out of the stuffing box on this Rose Run Sandstone well. He told me that Scott Miller Contracting was on location to scrape up the affected material and was going to haul it to landfill. My inspection today found a 20 x 20 area has been scraped on the west side of the wellhead and a secondary containment dike has been constructed around the wellhead. I found no evidence of any free oil on the ground. The well is in production and is equipped with 2-3/8 tubing and rods and an electric powered pumping unit. A gauge on the tubing flow line indicates a pressure of 120psi. This well produces to a diked tank battery on location and shares equipment with the Ebie #4 (PN#24060). The equipment consists of three (3) 210 barrel tanks, a horizontal separator, and an associated meter run. The chart indicates a differential production pressure of 70psi and there is a domestic tap on the sales line. The required ID is posted at the wellhead and at the tank battery. No violations. |
