I was shown by the facility operator that the water in the diked tank battery is pumped by sump pump and garden hose to a floor drain in the pump house which flows into a concrete vault next to the off-loading pad. The water in the concrete vault is then pumped into a storage tank. There are no violations at this time.

0905386242 2016-05-12 Salt Water Injection Well No Violations I conducted a quarterly status check inspection of this active injection well and the associated surface facilities with Inspector John Fleming. The pump was currently running and a gauge on the injection tubing indicated 800psi. The facility was currently manned by one (1) Stallion employee. A gauge on the annulus between the 4.5 casing and injection tubing showed the annulus to have no pressure (0psi). The gauge was changed out by the employee and still registered 0psi. The permit for this well requires a minimum of 200psi be maintained on the annulus and monitored monthly to help detect leaks under OAC 1501:9-03-07(F). Inspector Fleming requested that Stallion schedule a pressure test for the annulus and that the Division be notified prior to conducting the test. I will wait to here from Stallion.

The area has received recent significant rainfall so the decision was made to leave the south wall of the containment dike open to allow for rainwater drainage. The open dike will also allow access to the containment area for placing the storage tanks. Before placing the tanks, the liner will be covered with six inches of sand and several inches of gravel. After the tanks have been set, the south end of the liner will be rolled back and the containment wall will be built and the liner placed on top.

1174070217 2007-03-01 Production Wells No Violations Status check inspection was completed after the owner applied to covert this production well into a salt water disposal well. The well is located in a soybean field approximately 550 west of SR 88 and adjacent to the Geauga County line. The surface equipment at the wellhead has been removed with rods and tubing remaining. The tank battery is located 75 west of SR 88 at the Geauga County line. There is no evidence of lakes, ponds or streams in the immediate vicinity of the well or the existing tank battery. Several private residences (with water wells) are located east and south of the site. The North Coast disposal well (PN 3343) and equipment yard are located 1000 east of the Groselle #2 well.

1181083204 2007-06-04 Salt Water Injection Well No Violations The purpose of this inspection was to observe the conversion of a production well into a salt water injection well. The first objective was to set a bottom cement plug in the Clinton Sandstone formation. Appalachian Well Surveys set a retainer in the 4-1/2 diameter casing at the depth of 4000. Bubs Well Service ran 2-3/8 diameter tubing with a retainer stinger to bottom. Prior to stinging into the retainer, I pressure tested the 4-1/2 casing. 1540 psi was held on the casing for 15 minutes with zero pressure loss. The service rig stung into the retainer and Universal Well Services mixed and pumped a 29 sack bottom plug thru the tubing. Displacement pressure reached 1000 psi. We waited 10 minutes and tried to pump again. The formation pressured up to 2500 psi. The bottom plug was set. The service rig stung out of the retainer; Universal reverse circulated to flush out any residual cement that was in the casing; then the tubing was pulled. A minimum of 300 of cement is required to be above the top of the Newburg formation for injection requirements. Appalachian Well Surveys ran a cement bond log across the Newburg formation to determine the top of cement. The top of cement was located at 3634. A cement squeeze job was necessary. Appalachian perforated with 3 shots at 3614. Universal pumped on the 4-1/2 casing and established circulation out of the 8-5/8 casing valve. The rate was 3-4 barrel per minute at 1200 psi. Universal mixed and pumped 2 barrels of cement slurry, then blew a hydraulic line. The 2 barrels of cement slurry was circulated out of the 4-1/2 casing. A replacement hydraulic line was brought in from Wooster. Universal mixed and pumped cement a second time; the slurry weight was insufficient; the cement slurry was circulated out of the casing; Universal was out of bulk cement. Another cement truck and bulk truck were called out to the job. I left the location at 0215 hrs on 6-5-2007. I called Craig Norton (330-697-2373), who was overseeing the job for M &
1182177138 2007-06-15 Salt Water Injection Well No Violations Bubs Well Service, Appalachian Survey and Superior were on site to complete the conversion of this Clinton production well into a Newburg salt water disposal well. The cement retainer and Clinton plug was set on June 4, 2007. On June 15, Appalachian perforated with 29 shots from 3898 to 3870, with 45 shots from 3856 to 3814 and with 43 shots from 3798 to 3756. After perforating, the hole was swabbed down and a Baker Lockset packer was run in the hole with 2-7/8 tubing to a depth of 3900. Superior spotted 500 gallons of 28% HCl. The packer was pulled up to 3737, 50 bbl of inhibitor was mixed and circulated and the packer was set. The annular space was pressured to 1000 psi. After five minutes, the pressure was 990 psi and after 15 minutes the pressure was 960 psi, for a total loss of 4%. The MIT passed. The well was then treated with 2500 more gallons of 28% HCl and 1000 bbls of salt water. The formation broke at 1582 psi. The average treating pressure was 2931 psi with initial shut in pressure of 264 psi. The 5 minute shut in pressure was on vacuum.

A pressure of 1040 psi. was placed on the tubing-casing annulus. During a fifteen minute period, the pressure held without a loss.

1819235146 2013-07-01 Salt Water Injection Well No Violations I was on location to inspect workover operations at this disposal well. Inspectors Steve Ochs, Rick Skidmore and ODNR interns were on location as well. Whipstocks service rig was tripping tubing out of the hole. They thought they had hit an obstruction when running into the hole and Stallion personel decided to run a downhole camera tomorrow to see if there was anything inside the casing. No violations.
1888309800 2013-05-15 Salt Water Injection Well No Violations I witnessed a mechanical integrity test. The annular pressure went from 540 psi. to 260 psi., in fifteen minutes. The well failed the test.

2011417827 2019-02-01 Salt Water Injection Well Violations Noted I found the injection well operating and over the maximum allowable pressure during my inspection. There was a facility operator on location. I instructed the facility operator to reduce the injection well pressure immediately. The facility operator reduced the operating pressure on the injection well. I informed the facility a compliance notice will be issued for operating over pressure. Identification signs are posted. I observed no leaks or spills. A follow-up inspection will be conducted by February 4, 2019.
2049205924 2007-08-10 Salt Water Injection Well No Violations M-J Oil personnel were on site to place eight 210 bbl storage tanks inside the containment area. The liner was protected with a layer of sand. The next action will close the southern containment wall and extend the liner over the dike wall.
2078464824 2015-04-29 Salt Water Injection Well No Violations The well and surface facility were inspected. The well was equipped with a new two inch valve on the casing. There were no violations found at the time of inspection.
2085607264 2013-05-15 Salt Water Injection Well No Violations I was on location with UIC Inspector Steve Ochs to witness a mechanical integrity test (MIT) requested by Stallion personell. Stallion believed that the tubing packer had failed and was leaking into the annulus between the tubing and the 4.5 casing. 550psi was placed on the pipe at 8:19am and at 8:25am, the pressure had dropped to 400psi. By 8:29am, the pressure was down to 340psi and failed the MIT required by OAC 1501:9-3-07 which allows for no more than a 5% decline over a 15 minute testing period. Steve Ochs verbally suspended injection operations and will issue a compliance notice that operations may not commence until the well is repaired and Stallion demonstrates mechanical integrity.
