1249111141 2015-05-05 UrbanProduction Wells No Violations I inspected this well and tank battery at the request of Chris Lachman (CSL Consulting) for safety compliance of the above ground storage tanks as part of the HUD review process in expansion of the Anna Maria Nursing Facility. The wellhead is secured by chain link fencing topped with three strands of barbed wire and the man door is locked. The well is operating with the aid of a plunger lift system and is equipped with a Kimray motor valve and BT-2000 logic controls. The valve on the annulus between the 8.625 surface casing and 4.5 production casing is open. The proper identification is posted on the fence. The tank battery contains two (2) 140 barrel tanks, a 50 barrel poly tank, and a horizontal separator. The tank battery is fenced and screened on the north and east sides with board on board fencing and both man doors are locked. The dike surrounding the tank battery measures 51(L) x 21(W) x 3(H) and exceeds SPCC requirements. The valves on the tanks are locked, the tank hatch lids are closed, and the vent stacks are plumbed to a 42 gallon charcoal filter equipped with a low-pressure relief valve. Both tanks are equipped with lightning arrestors and the equipment is properly spaced. The proper ID is posted on the fencing and well #24352 also produces to this tank battery. This facility meets the safety requirements outlined in Chapter 1509.021 and 1509.23 of the Ohio Revised Code and Chapter 1501:9-9-05 of the Ohio Administrative Code.
