0294045752 2009-06-02 Production Wells No Violations The valve on the 8-5/8 casing head is buried but appears to be open. The result has been the release of a small amount of salt water to the surface area around the wellhead. The Quantab test results indicated chloride levels of approximately 6000+ ppm. I contacted Rick Batta with North American and asked him to uncover the casing head and place a pressure gauge on the open valve side to monitor annular pressure. I also asked him to recover the salt water and reseed the affected area.

1571954889 2017-11-20 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 tubing and rods and an electric powered pumping unit. The belts on the pumping unit motor are currently broken. ID is posted on the unit and there is a domestic tap, charged at 14psi, plumbed to the 4.5 production casing port. The well produces to a diked tank battery to the north and contains one (1) 100 barrel tank, a 50 barrel poly tank, and a horizontal separator. A gauge at the separator indicates a pressure of 110psi. The required identification is posted on the 100 barrel tank. No violations.
