0250679144 2009-11-23 Drill / Deepen / Reopen No Violations I received a call from Sheila Freedman at 380 Chadwick Drive in Aurora (330-562-0507 or 216-978-8762). Her allotment is located southwest of the Beljon Unit 2 well. She said that the drilling did not bother them but since the drilling rig moved off, her house has been shaking. Her home is approximately 1500 from the well and no complaints from other homes in the allotment have been received. This inspection of the well and the neighborhood did not indicate any problems.
0447209939 2017-10-25 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Moore Well Services was the plugging contractor, Petroset Cementing was the cement contractor, and Appalachian Well Surveys provided wireline service. Plugs were set across the Berea Sandstone/Ohio Shale interval and the Glacial Deposits. A cast iron bridge plug was set inside the 8.625 surface casing at a depth of 225. See plugging report for more details on this job. No violations.
0497849358 2014-03-06 UrbanProduction Wells No Violations I conducted a status check inspection of this Clinton well. I found the tank battery was fenced and diked and the gate was locked. The battery contained 1-100 barrel tank, a 50 barrel tank, and a horizontal separator. The proper ID was posted on the fence of the tank battery. I inspected the wellhead and found it producing with the aid of an electric pumping unit. The proper ID was posted at the wellhead. I found no violations at this time.
0793029453 2018-04-09 Final Restoration No Violations Status check inspection for Final Restoration of this Clinton Sandstone well that was plugged and abandoned on 10/25/2017. My inspection today found that the area of the former wellhead has been graded and all ancillary equipment related to the well has been removed. The area of the tank battery to the north has been incorporated into the Beljon Estates Subdivision and has been overprinted by development. The lease road and flow line have not been removed and an executed Landowner Waiver (Form 5) from Pulte Homes of Ohio has been submitted, waiving the removal of the lease road and flow line, and will be removed during Phase 2 of the development by the landowner. Final Restoration is approved.
0922773402 2009-11-13 Drill / Deepen / Reopen No Violations I met on site with Tim Cosma and Joe Lang (Cedar Valley) and Tom Mills (excavator) to discuss the drainage concerns along the new lease road. The neighboring property owners are worried about the road acting as a conduit forcing more surface water onto their properties. The decision was made to move a nearby topsoil stockpile and to build up the road after frac to retain water south of the neighbors. The site will be revisited after the work has been completed.

1258390621 2009-11-04 Drill / Deepen / Reopen No Violations I met on site with two neighboring property owners along Pioneer Trail to discuss drainage concerns they have since the lease road was installed. The homes are located north of the lease road. Chuck Horn at 572 Pioneer Trail (330-995-6694) requested the meeting. The area in question has always been a low spot and the property owners installed a crock with a sump pump to remove accumulated water and transfer it to the street. Their concern is that the new lease road will add to the amount of water directed to this area. I told them that I would meet with Cedar Valley to discuss options for retaining water on the south side of the lease road.
1277151918 2010-06-02 Preliminary Restoration No Violations Preliminary restoration is approved. The area around the wellhead is planted in corn. The tank battery is fenced and the well is in production.
1280007010 2010-01-25 Production Wells No Violations Status check inspection found that the well has been turned on but does not have the required security, safety equipment and identification on the tank battery. I called Tim Cosma and told him to shut the well in until these issues are resolved.
1508949596 2017-10-24 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Moore Well Services was the plugging contractor and Petroset Cementing was the cement contractor. Plugs were emplaced across the Clinton Sandstone and at the top of the Big Lime. The 4.5 production casing was ripped at a depth of 2590 and 32 joints were recovered prior to power tong failure. Operations will continue tomorrow (10/25/17). See plugging report for more details on this job. No violations.
1509027032 2017-10-26 Plug / Plug Back No Violations Status check inspection of the plugged and abandoned Clinton Sandstone well. I was on location to check the top plug in this well. My inspection found the plug solid and one (1) foot below the top of the casing head. I found no indication that the plug was leaking or lacked integrity. Moore Well Services was currently removing the pumping unit from location and will be cutting off the casing and attaching the ID plate later today. No violations.
1934877443 2017-12-18 Final Restoration No Violations Status check inspection for Final Restoration of this plugged and abandoned Clinton Sandstone well. The well was plugged on 10/25/2017 and final restoration is due by the end January of 2018. This site is being quickly developed as a residential subdivision. My inspection found that all the surface production equipment has been removed from the location of the wellhead and tank battery. The lease road remains and a Landowner Waiver (Form 5) will need to be submitted to leave the road and the flow and sales (disconnected) lines buried for Final Restoration to be passed. No violations.
