0021355561 2015-06-19 Plug / Plug Back No Violations I was on location with Inspector John Fleming, Hagen Well Services, and Appalachian Well Surveys to continue with plugging operations. Appalachian ran a cement bond log (CBL) and gamma ray tool to check for free pipe below the Salina Salt Group. The CBL indicated that the salt had not squeezed the 4.5 casing and was free to the point where the casing could be shot off to avoid perforating and squeezing a plug across the Clinton Sandstone. The casing was shot off at a depth of 4,550 where the gamma ray signal indicated the presence of the competent Medina Sandstone. The pipe was free and Hagen pulled a 6 pup joint and one (1) 44 joint of casing. The weight indicator showed 53,000lbs of pullback coming out of the hole and agrees with the actual string weight of the 11.6lb/ft., 4.5 casing of 52,780lbs. The casing was set in the slips, packed off, and shut-in. Operations will resume on 6/22/15. No violations.
0095123146 2015-06-22 Plug / Plug Back No Violations I was on location with Hagen Well Services, Petroset Cementing, and Northeast Oilfield Services to conduct plugging operations. Plugs were set across the Clinton Sandstone, the Big Lime, the Ohio Shale/Berea Sandstone interval, and the Glacial Deposits. Good circulation was achieved on all plugs and cement circulated and held at the surface on the final plug. 103 joints of 11.6lb/ft., 4.5 casing was recovered during the job. See plug report for more details on this job. No violations.

0402644849 2015-07-16 Final Restoration No Violations I was on location to check the status of final restoration. This well was plugged back from the Rose Run Sandstone to the Trenton Limestone on 1/9/2015. The Trenton was stimulated with acid and subsequent testing proved the zone to be non-commercial. The well was plugged and abandoned on 6/22/2015 and final restoration is due by 9/22/2015. My inspection today found that the tank battery has been dismantled and removed. The location has been graded and needs to be seeded. There is trac-hoe parked on the lease road. I spoke with the landowner about the lease road and he would like it to remain for access to his bean field. A landowner waiver will be required for the lease road and flow or sales lines left in the ground. I will check back later on the establishment of vegetation. No violations
0549401144 2015-06-18 Plug / Plug Back No Violations I was on location with Steve Hagen Well Services and Petroset Cementing to witness the setting of the plug across the Trenton Limestone. This well was plugged back from the Rose Run Sandstone on 1/12/15 and the Trenton Limestone was tested for production and found to be a dry hole. Hagen swabbed the hole dry this morning and Petroset rigged up to the 4.5 casing. A 30 sack plug was bullheaded to the perforations with 50lbs of cottonseed hulls. The plug landed, the well pressured up to 600psi, and the valve on the casing was shut. Full displacement was achieved to a depth of 5932. No violations. See plug report for more details on this job.
0834554355 2015-01-12 Plug / Plug Back No Violations I was on location to check on the plugging back of this well to the Trenton Limestone. Mike Weinsz (M&M) was on location with Chipco and Appalachian Well Surveys to conduct operations. Appalachian ran in the hole and tagged the top of the cement plug set on Friday (1/9/15) at a depth of 6338. They set a steel bridge plug at 6337 and ran a cement bond log coming out of the hole. The DV tool (2-stage cement tool) run when the well was drilled was found at 6250. The bond log verified the top of the cement at 5582 and satifies the requirements outlined in OAC 1501:9-1-08(M)(7)(a)(i) that requires 500 of annular coverage above the top perforation which is 6123. Appalachian perforated the casing in 3 zones: 6123-6144, 6202-6208, and 6216-6224. M & M Royalty will be testing these zones. No violations.
0919130892 2015-05-19 Preliminary Restoration No Violations I received a call from Mike Weinsz (M&M) concerning the plugging requirements for this recently plugged back well. The section perforated and acidized in the Trenton Limestone was non productive and preliminary restoration is due from the plug back. I inspected location today and there is currently no activity at the well. The wellhead is currently equipped with a Demco gate valve and a 2.375 tubing head. The gate valve is closed. The pit used for plug back operations is closed and the area is graded. There is no drilling equipment on location. Preliminary restoration is approved. No violations.

The average rate was 19bpm with a pressure of 3502psi. ISIP was 3451psi. 5 minutes was 3284psi. The total gallons used were 138,500 gallons and 759 sacks of sand. There were no violations at this time.
1306935648 2011-05-18 Urban Drill / Deepen / Reopen No Violations On May 10, 2011, during the primary 4-1/2 casing cement job, the multi stage cementer packer failed to open and the second stage was cancelled. On May 18, prior to drilling out the trip bomb, Universal pressured up on the packer in a last attempt to open the ports. At 2100 psi the ports opened and circulation was established. Universal then mixed and displaced 150 sacks of 50/50 pozmix with 2% gel and 10% salt. Circulation was maintained throughout the job with the plug down at 17:06 on May 18.

1353093911 2012-11-16 UrbanPreliminary Restoration No Violations I inspected this location to check the status of preliminary restoration which was due 7/30/2011. During my inspection I found that the drilling pits had been closed and that the site had been graded and seeded. There were no drilling, completion, or related equipment on location. The access road was gated and in good condition. The wellhead and tank battery were both diked and fenced with locks on both gates. The well was equipped with a pumping unit and the tank battery contained two vertical separators, 2-210 barrel tanks, and 1-210 barrel, horizontal tank. The meter run was located inside the tank battery. Preliminary restoration is approved. No violations.

1684491505 2014-01-09 Plug / Plug Back No Violations I was on location to witness the spotting of the bottom plug across the Rose Run Sandstone in the process of plugging the well back to the Trenton Limestone for testing. Mike Weinsz (M&M) was on location to oversee operations. Chipco spotted 2.375 tubing below the bottom perforation at a depth of 7,063. Universal Well Services rigged up and then mixed and pumped 11.5 barrels (50 sacks) of 50/50 Pozmix cement slurry. The slurry was displaced to 6,325 with 24 barrels of water. Chipco pulled 1000 of tubing and Universal circulated the tubing clean with 40 barrels of water. Operations will continue on Monday (1/12/15). No violations.
1761777468 2016-09-15 Final Restoration No Violations I conducted a Final Restoration inspection on the former location of this Rose Run Sandstone well. This well was plugged and abandoned in accordance with Chapter 1501:9-11 of the Ohio Administrative Code on 6/21/2016. My inspection today found that all of the surface and production equipment has been removed and the site has been graded. Native vegetation is well established on location and the topsoil is well bound to prevent substantial erosion. The operator has met the restoration requirements outlined in Chapter 1509.072 of the Ohio Revised Code and has submitted the required Landowner Waiver (Form 5) to leave the access road in place for the landowners use. Final Restoration is approved.
