A plugging permit was issued on 5/25/2015 and expires on 5/25/2017. The only commercial production reported for this well was from flow-back operations during the 4th quarter of 2011. My inspection today found the lease road and well pad in stable condition. The well pad is fenced and secured. Vegetation and small brush are beginning to take root on the pad, but does not present a serious issue at this time. The wellhead is secured with tubular steel fencing and two (2) steel grates cover the cellar. The main and wing gate valves are closed, chained & locked, and the well is shut-in. I will speak with Chesapeake about their plans to plug the well as the permit will expire in just over three (3) months. No violations. |

My inspection found that identification is posted at the lease road entrance, at the pad gate, and at the wellhead. Access road gate is currently open and being used by the farmer. Entire well pad is secured with chain link fencing and topped with three strands of barbed wire. The lease road and pad are in good condition and constructed of limestone. Access gates to the pad are locked. Completion operations were conducted on the well (perforating/fracing) in November of 2011, and during flow back operations, produced 583 barrels of oil, 0 mcf of natural gas and 1796 barrels of brine. Continued testing in 2012 did not yield any commercial quantities of gas or oil and 7207 barrels of brine. The wellhead is shut in, surrounded by a gate, and contained within a liquid tight cellar that is covered by metal panels. The gate valves on the wellhead are chained and locked. There is no production equipment or tank battery on the location. Security cameras are located at the entrance to the pad. No violations. |

The tubing was spotted at 4510 and NWP mixed and pumped a 300 plug to isolate the Clinton Sandstone inside the 5.5 production casing. See plugging report for more details on this job. The plug back footage totaled 2100 divided into three equal stages of 700. Halliburton mixed 310 sacks of Densecem cement for each stage. Densecem System is a Class H based blend with a weight of 17.2 ppg and a yield of 0.98 cf/sack. The plugs were spotted using open ended 5 drill pipe. Halliburton broke and maintained circulation throughout each stage. |

My inspection found the lease road gate present and open (this is access to the farmers field). The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The wellhead is surrounded by a liquid-tight cellar with a two-piece grate covering the cellar and tubular steel gates surrounding the wellhead. The wellhead consists of a 5.5 (flanged) production casing wellhead with double wing gate valves on each side. The christmas tree consists of two (2) main gate valves with double, tubing wing gate valves on each side, and a swab gate valve on top of the tree. The well is shut-in and gauges on the casing and tubing valves read 0psi. The gate valves are chained and locked. There is no surface equipment or tank battery on the well pad. Chesapeake has applied for, and been issued, a plugging permit for this well that will expire on 5/25/2017. Video surveillance monitoring has been removed from the location. Note: Temporary Inactive status for this well expired in November of 2014. |

NWP pulled the remaining tubing from the well and NES rigged up and ran a junk basket and gauge ring to 5050 to verify drift diameter. The tools were pulled and NES ran in and set a cast iron bridge plug (CIBP) at a depth of 4992. The CIBP was tested at 5000psi for fifteen minutes with no leak-off and operations continued. NWP began tripping 2.375 tubing back into the 5.5 production casing and will begin setting cement plugs in the morning (5/10/17). See plugging report for more details on this job. No violations. |

The 5.5 casing stub was tagged at 492 and the hole was swept with S-410, P-32, and diesel fuel to remove the oil residue from the casing walls. The tool was pulled and Weatherford ran a cement bond log (CBL). The CBL indicated no recoverable 9.625 casing and the remaining plugs will be s inside this casing. The annulus between the 5.5 casing and 9.625 casing continued to release low pressure gas and it was decided to set a cast iron bridge plug inside the 9.625 casing tomorrow morning (5/17/17). The landowner, Mr. Edward Hosey, has executed the Landowner Waiver (Form 5) and has waived removal of the chain link fencing, removal of the entire access road, and removal of the entire well pad. My inspection today found that all equipment has been removed. The wellhead and cellar were cut off and buried following plugging operations and the area has been rough graded. A flow line, sales line, and tank battery were never installed or constructed for this well. The well pad is limestone and the establishment of vegetative cover is not required. Final Restoration is approved. |
