0146096985 2011-09-17 Drill / Deepen / Reopen No Violations I performed a rig inspection on Paterson rig #329. The rig crew were helping run into the hole with 13 3/8 casing. This inspection satisfies 1501:9-9-03. There were no violations at this time.
0207200480 2011-11-09 Completion Testing No Violations The workover rig moved on-site and preparing for the frac crew which are due on location on 11/11/11. There were no violations at this time.
0209223859 2017-02-17 UrbanProduction Wells No Violations I was on location to inspect the status of this horizontal, Utica Shale well and well pad. The current status of this well is listed as temporary inactive (1st year) and is incorrect as the inactive status expired on 11/21/2014. A plugging permit was issued on 5/25/2015 and expires on 5/25/2017. The only commercial production reported for this well was from flow-back operations during the 4th quarter of 2011. My inspection today found the lease road and well pad in stable condition. The well pad is fenced and secured. Vegetation and small brush are beginning to take root on the pad, but does not present a serious issue at this time. The wellhead is secured with tubular steel fencing and two (2) steel grates cover the cellar. The main and wing gate valves are closed, chained & locked, and the well is shut-in. I will speak with Chesapeake about their plans to plug the well as the permit will expire in just over three (3) months. No violations.
0210543536 2013-11-06 UrbanProduction Wells No Violations I inspected this well at the request of Columbus in regards to an application for temporary inactive status (1st year) for this horizontal well. My inspection found that identification is posted at the lease road entrance, at the pad gate, and at the wellhead. Access road gate is currently open and being used by the farmer. Entire well pad is secured with chain link fencing and topped with three strands of barbed wire. The lease road and pad are in good condition and constructed of limestone. Access gates to the pad are locked. Completion operations were conducted on the well (perforating/fracing) in November of 2011, and during flow back operations, produced 583 barrels of oil, 0 mcf of natural gas and 1796 barrels of brine. Continued testing in 2012 did not yield any commercial quantities of gas or oil and 7207 barrels of brine. The wellhead is shut in, surrounded by a gate, and contained within a liquid tight cellar that is covered by metal panels. The gate valves on the wellhead are chained and locked. There is no production equipment or tank battery on the location. Security cameras are located at the entrance to the pad. No violations.
0225283247 2017-05-22 Plug / Plug Back No Violations I was on location to verify the integrity and level of the top plug in this recently abandoned Point Pleasant well. I tagged the top of the cement plug inside the 9.625 intermediate casing with a weight and measuring tape at a depth of two (2) feet below the top of the casing. I did not observe any leaks or gas bubbling through the fluid on top of the cement. I notified Jason Nicholas (Nicks Well Plugging) that the casing may be cut off, the ID plate attached, and that the tin horn can be removed. No violations.
0239605548 2011-10-31 UrbanPreliminary Restoration No Violations Most of the rig equipment is gone. The rest of the equipment will be gone today. There were no drilling pits. Good vegetation growth on the berms with a chain link fence around the drilling site. There were no violations at this time.
0555235434 2017-05-12 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), Weatherford, and Energy Fishing Services (EFS) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. EFS used the casing jacks to unseat the casing slips and recover the 5.5 productions casing. 492 of 20lb/ft., P-110 casing was recovered during he job and had to be chop-sawed at the casing collars while coming out of the hole. The inside of the 9.625 casing is coated with dehydrated drilling mud which will need to be removed and cleaned prior to setting plugs in the casing. EFS will bring out a 9.625 casing scraper on Monday morning (5/15/17). See plugging report for more details on this job. No violations.
0775400352 2017-06-05 Plug / Plug Back No Violations I was on location to inspect this pad following plugging and abandonment operations that concluded on 5/17/17. My inspection found that all equipment has been removed, the tin horn (cellar) has been removed, and the wellhead has been cut off below grade and buried. There is a large pile of stone on the east side of the pad. Final Restoration is due in August 2017 (3 months). No violations.
0777546231 2017-05-10 Plug / Plug Back No Violations I was on location today with Chesapeake (CHK) field representative Wallace Bossert and Nicks Well Plugging (NWP) to continue plugging operations on this Point Pleasant well. NWP ran 2.375 tubing to a depth of 4992 and tagged the top of the cast iron bridge plug. They then rigged up, established circulation, and then spotted a 200 plug on top of the bridge from 4992-4792. The cement was allowed to cure for 7 hours and NWP tagged the top of the bottom plug at 4770 with a sinker bar on the sand line. The tubing was spotted at 4510 and NWP mixed and pumped a 300 plug to isolate the Clinton Sandstone inside the 5.5 production casing. See plugging report for more details on this job. No violations.
0850301554 2017-05-17 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), Weatherford, and Nine Energy Services (NES) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. NES rigged up and ran inside the 9.625 casing and set a cast iron bridge plug at a depth of 450. Negative and positive pressure tests were performed on the plug and passed. The annular gas has been shut-off and plugging can continue. NWP spotted tubing at the top of the bridge plug and filled the 9.625 casing with cement from 450 to surface in two (2) stages. The hole was topped off and NWP began rigging down. See plugging report for more details on this job. No violations.
0939976266 2011-10-14 Drill / Deepen / Reopen No Violations Paterson is drilling ahead on the horizontal leg of this well. They anticipate completing the well by the first of next week. No violations at this time.
0952668256 2017-05-15 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), Stallion, and Energy Fishing Services (EFS) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. EFS rigged up the 9.625 casing scraper and NWP picked up the tool and tripped into the 9.625 casing. The tool made it to a depth of 75 and would no long move downhole. NWP brought in a swivel and they were able to wash 10 joints into the hole and circulate the mud out of the casing. The solids in the mud were filling up the cellar and caused operations to cease until 5/16/17. Wallace had a super-sucker clean out the cellar later in the afternoon. See plugging report for more details on this job. No violations.
1015793989 2011-11-09 UrbanPreliminary Restoration No Violations The chain link fence was installed and vegetation was growing on the berms around the drilling location. This well PASSES PRELIMINARY RESTORATION.

The wellhead is surrounded by a liquid-tight cellar with a two-piece grate covering the cellar and tubular steel gates surrounding the wellhead. The wellhead consists of a 5.5 (flanged) production casing wellhead with double wing gate valves on each side. The christmas tree consists of two (2) main gate valves with double, tubing wing gate valves on each side, and a swab gate valve on top of the tree. The well is shut-in and gauges on the casing and tubing valves read 0psi. The gate valves are chained and locked. There is no surface equipment or tank battery on the well pad. Chesapeake has applied for, and been issued, a plugging permit for this well that will expire on 5/25/2017. Video surveillance monitoring has been removed from the location. Note: Temporary Inactive status for this well expired in November of 2014.
1461268082 2017-05-09 Plug / Plug Back No Violations I was on location today with Chesapeake (CHK) field representative Wallace Bossert, Nicks Well Plugging (NWP), and Nine Energy Services (NES) to begin plugging operations on this Point Pleasant, horizontal well. NWP pulled the remaining tubing from the well and NES rigged up and ran a junk basket and gauge ring to 5050 to verify drift diameter. The tools were pulled and NES ran in and set a cast iron bridge plug (CIBP) at a depth of 4992. The CIBP was tested at 5000psi for fifteen minutes with no leak-off and operations continued. NWP began tripping 2.375 tubing back into the 5.5 production casing and will begin setting cement plugs in the morning (5/10/17). See plugging report for more details on this job. No violations.
1496165983 2017-05-11 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert and Nicks Well Plugging (NWP) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. NWP spotted plugs inside the 5.5 production casing across the top of the Big Lime and at the 9.625 intermediate casing shoe. The tubing was pulled and the crew was waiting on the casing shooters. I left location for the Murvay 8H (24423). See plugging report for more details on this job. No violations.
1496775976 2017-05-11 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company men Wallace Bossert and Blake Riley, Nicks Well Plugging (NWP), and Nine Energy Services (NES) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. NES ran into the hole and shot the 5.5 production casing off at a depth of 492. A casing spear and grapple was set in the 5.5 casing and NWP pulled 75,000lbs on the casing and could not unseat the casing slips. It was decided to bring out casing jacks in the morning and attempt to recover the pipe. See plugging report for more details on this job. No violations.
1693950177 2017-05-16 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), Weatherford, and Energy Fishing Services (EFS) to continue with plugging operations on this horizontal, Utica-Point Pleasant well. EFS and NWP continued to circulate and scrape the inside of the 9.625 casing. The 5.5 casing stub was tagged at 492 and the hole was swept with S-410, P-32, and diesel fuel to remove the oil residue from the casing walls. The tool was pulled and Weatherford ran a cement bond log (CBL). The CBL indicated no recoverable 9.625 casing and the remaining plugs will be s inside this casing. The annulus between the 5.5 casing and 9.625 casing continued to release low pressure gas and it was decided to set a cast iron bridge plug inside the 9.625 casing tomorrow morning (5/17/17). See plugging report for more details on this job. No violations.
1703284766 2011-11-09 UrbanPreliminary Restoration No Violations The vegetation was growing on the berms around the drilling location. The chain link fence was around the drilling location. This well PASSES PRELIMINARY RESTORATION.
1829272889 2017-07-21 Final Restoration No Violations I was on location to conduct a Final Restoration inspection of this plugged and abandoned Utica/Point Pleasant well that was plugged on 5/17/2017. The landowner, Mr. Edward Hosey, has executed the Landowner Waiver (Form 5) and has waived removal of the chain link fencing, removal of the entire access road, and removal of the entire well pad. My inspection today found that all equipment has been removed. The wellhead and cellar were cut off and buried following plugging operations and the area has been rough graded. A flow line, sales line, and tank battery were never installed or constructed for this well. The well pad is limestone and the establishment of vegetative cover is not required. Final Restoration is approved.
