0068231459 2017-02-08 Administrative Inspection No Violations I was on location to check the status of this drilled, horizontal Utica Shale well. My inspection today found the lease road gate present and locked. The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The pad is becoming inundated with weeds and small diameter trees/brush approximately 1 in diameter and less. The 8H well has never been completed or produced. The wellhead is surrounded by a liquid-tight cellar with a two-piece grate covering the cellar and tubular steel gates surrounding the wellhead. The wellhead consists of a 5.5 (flanged) production casing wellhead with double wing valves on each side. There is no Christmas tree and the top of the wellhead is fitted with a night cap. No gauges are present to check annular and wellhead pressures and the cellar is full of water and frozen. There is no surface equipment or tank battery on the well pad. Chesapeake has applied for, and been issued, a plugging permit for this well that will expire on 5/11/2017. Chesapeake drilled a 398 water well on 2/10/2012 to use for completion operations and is located on the south side of the lease road at coordinates: 41.21888, -81.16999 (NAD83 +/-9). The water well will need to be properly sealed and abandoned under the rules outlined in 3701-28 of the Ohio Administrative Code prior to abandoning the location. I will speak with Chesapeake about their plans to plug and abandon the well.
0087362419 2017-05-21 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), and Weatherford to finish plugging this Point Pleasant, horizontal well. The fluid level inside the 9.625 intermediate casing held overnight and NWP ran 24 joints 0f 2.375 tubing to the top of the 5.5 casing stub at 780. They rigged up and circulated 120 barrels of brine at five (5) barrels/minute (bpm) to clean the hole. They then mixed and pumped a 10 barrel sweep consisting of 15 gallons of S-417, 15 gallons of P-32, and 9 gallons of diesel at 2bpm to clean the inside of the casing prior to cementing. They followed the sweep with 65 barrels of water and pulled nine (9) joints to a depth of 489 to set the final plug. The final plug was set in two (2) stages and good cement returns were achieved at the surface. The tubing was pulled and the hole was topped off. I will check the integrity of the top plug tomorrow (5/22/17). See plugging report for more details on this job. No violations.

0468967450 2018-01-22 Final Restoration No Violations I was on location to check the reclamation status of this Utica Shale well pad. I met with Derick from Woodford Construction and he told that they have completed all of the required dirt work on the pad. He told me that between 12,000 and 15,000 tons of limestone was removed from the pad during reclamation. My inspection found that the stone has been removed from the pad and back to the pad entrance as allowed by the landowner waiver (Form 7). The stockpiled topsoil that was located on the southwest side of the pad has been spread and the pad location has been filled and graded. Silt fence has been installed around the reclaimed area of the pad and is backed and supported by chain link fencing and metal poles. The track-hoe was removed from location during my inspection and the two (2) dump trucks and bull dozer will be removed tomorrow. The area will need to be mulched and seeded. As soon as a good seedbed is established, the silt fence may be removed and final restoration will be reviewed for approval. No violations.

0774193046 2018-05-02 Final Restoration No Violations I conducted a status check inspection of this reclaimed, shale well pad as it progresses towards final restoration status. My inspection found the pad stable and mulched and seeded. The seed is beginning to sprout across the affected area and binding the soil. The super silt fence remains in place around the perimeter of the pad and is in good condition. I will check back in June to determine if the seeding is sufficient and whether or not the silt fence can be removed. No violations.

0776080275 2018-02-01 Final Restoration No Violations I conducted a status check inspection for Final Restoration. Reclamation of the pad has progressed over the past month. My inspection today found that the reclaimed area has been mulched and seeded. The silt fence will remain until the seed bed is well established. The cap on the water well that was damaged during the reclaiming process still needs to be replaced. The access gate and lease road will remain per the landowner waiver. I will check back in late spring to check the ground cover growth and soil stabilization. No violations.
0809960058 2017-05-20 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), Weatherford, KLX Wireline, and Energy Fishing Services (EFS) to continue with plugging operations on this Utica/Point Pleasant, horizontal shale well. NWP rigged up and set three (3) plugs inside the 5.5 production casing to isolate the Clinton Sandstone, the top of the Big Lime, and the intermediate casing shoe. The tubing was pulled and KLX ran in the hole and shot off the 5.5 casing at a depth of 780. EFS rigged up the casing jacks and unseated the slips and casing with a pull of 140,000lbs. TRKs power tongs were rigged up and 17 joints (plus two (2) stubs and one (1) pup) of 5.5, 20lb/ft., range-3 casing were recovered from the hole. The hole was loaded with water and fitted with a night cap. Operations will continue in the morning (5/21/17). See plugging report for more details on this job. No violations.
0902714573 2018-06-15 Final Restoration No Violations Status check inspection for Final Restoration approval. The entire well pad has been seeded and the seed bed is well established. There are several small, low, wet areas where moisture has prevented the seed bed from establishing and will likely establish as pockets of wetland. A landowner waiver (Form 5) was received by Columbus on 6/7/18 and waives the removal of the access road and gate and a portion of the road past the gate. Ownership of the water well, drilled for operations, was transferred to the landowner. The water well cap, damaged during reclamation, was replaced as requested. I am approving the removal of the silt fence around the perimeter of the location and Final Restoration will be approved when the erosion controls are removed. No violations.
0915999548 2017-05-22 Plug / Plug Back No Violations I returned to location to verify the depth of the top plug in this recently abandoned Point Pleasant well. Evolution Energy Services (EES) was continuing to clean the cellar and the crew from Nicks Well Plugging (NWP) was waiting to rig down. I spoke with Chesapeakes EH&S coordinator on location about getting the service rig out of the way and he agreed that it would be best to do that at this time. NWP began rigging down and the EES crew began cleaning NWPs circulating tank so that it could be loaded and removed today. I was able to verify the top of the cement plug with a weight and measuring tape at a depth of five (5) feet inside the 9.625 intermediate casing string. There was no observable leakage or gassing and I told NWP that the ID plate may be attached. NWP loaded out all their equipment and EES began cleaning out the first of two (2) 500 barrel frac tanks on location. No violations.

The super-silt fence is in place and functioning properly. I will check back in mid-June before approving the removal of the fence and passing Final Restoration status. No violations.
1320778437 2011-11-05 Drill / Deepen / Reopen No Violations I met with Shane Green, Chesapeake Drilling Superintendent about water seeping from the ground on the southwest side on the berm area. I visually inspected and then tested the blackish fluid. The water had a smell to it, but no oily sheen. The pH test strip indicated a pH of 6 and the chlorine level on the Quantab read 159 PPM indicating that the fluid is not oil field related. I called Shane to tell him of my findings. There were no violations at this time.

1321902148 2011-11-17 Drill / Deepen / Reopen No Violations I performed a rig inspection on NOMAC Rig 14. The rig was down for repairs. The drilling depth was still 305. This inspection satisfies 1501:9-9-03. There were no violations at this time.
1322677751 2011-11-29 Plug / Plug Back No Violations I witnessed the plugging back of the Trenton formation. This is a 7.875 hole and the total depth is 6520. The bottom of the drill pipe was set at 6510 and the bottom plug was from 6510 to 6010. Halliburton pumped a 22 barrel spacer which was followed by 173 sacks of Class H cement (yield 1.26, weight 17.2#). The NOMAC Rig 14 pulled up the hole to 5850, circulated the hole and ran back into the hole to 6000. Halliburton pumped 242 sacks of Class H cement (yield 1.26, weight 17.2#), displaced with 56 barrels of mud and shut down. Job complete.

According to the rig operator, they will case below the deepest water well in the area and the consulting engineering firm will monitor the private wells for any drawdown once this well becomes operational.

1449378573 2013-10-09 Preliminary Restoration No Violations I was on location to inspect the status of preliminary restoration. Identification is posted at the lease road entrance. The access gate is locked and the entire well pad is secured with chain link fencing and topped with three strands of barbed wire. The lease road and pad are in good condition and constructed of limestone. Access gates to the pad are locked. Total depth of the horizontal leg was reached in December of 2011 and production casing was run and cemented. No completion operations have been conducted on the well (perforating/fracing). The wellhead is shut in, surrounded by a gate, and contained within a liquid tight cellar that is covered by metal panels. There is no production equipment or tank battery on the location. Security cameras are located at the entrance to the pad. Vegetation is well established around the perimeter of the pad and there is no sign of erosion. Preliminary restoration is approved. No violations.
1495804454 2017-05-22 Plug / Plug Back No Violations I was on location to check the status of this plugged and abandoned Point Pleasant well. Evolution Energy Services was on location with a super-sucker and cleaning mud and debris out of the tin horn (cellar) around the wellhead. All recovered material was being sucked into staged roll-off boxes spotted south of the wellhead. The service rig was still spotted over the wellhead and the open circulating tank and two (2) 500 barrel frac tanks remained on location to be cleaned out. I will return later to check the top of the cement inside the wellhead. No violations.
1496672315 2017-06-05 Plug / Plug Back No Violations I was on location to inspect this pad following plugging and abandonment operations that concluded on 5/21/17. My inspection found that all equipment has been removed, the tin horn (cellar) has been removed, and the wellhead has been cut off below grade and buried. There is a five (5) gallon bucket on location full of water, and a 4x4 piece of matting on location. The Weir boxes on the north and east sides of the pad are nearly full and the drain valves are closed. Final restoration is due in November (6 months) 2017. No violations.
1500319090 2017-07-17 Final Restoration No Violations I was on location to inspect the pad and surrounding areas prior to approval of the reclamation plan by Division engineering. I took photographs and video of points of interest. Restoration activities have not been undertaken as of this date. No violations.
1504621510 2017-09-05 Final Restoration No Violations Status check inspection of this plugged and abandoned Utica/Point Pleasant well pad. The well was plugged and abandoned on 5/21/2017 and Final Restoration is due by the third week of November. No reclamation activity has taken place since the plugging of the well. Reclamation plans were submitted by Chesapeake on 7/13/17 and comments were returned by DOGRM Engineer Paul Logan on 7/17/17. I spoke with Paul Logan today and Chesapeake has yet to address the submitted comments on the reclamation plan. I left a message with Jody Jones (Chesapeake) for a status update on this project. I will wait to hear back from Jody on the status of this well pad. No violations.
1506891523 2017-05-19 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), and Weatherford to continue with plugging operations on this Utica/Point Pleasant, horizontal shale well. Weatherford uninstalled the night cap and prepared the wellhead for plugging operations. The cast iron bridge plug (CIBP) was pressure tested and the test passed. NWP ran 182 joints of 2.375 tubing into the hole and tagged the CIBP at 5914. They then mixed and pumped 22 sacks of Class A cement slurry and emplaced a 200 plug on top of the cast iron bridge plug. The tubing was pulled above the cement and reversed out. The tubing was then pulled to 4550 and operations were shut down for the evening. See plugging report for more details on this job. No violations.
1515596161 2018-01-10 Final Restoration No Violations I was on location to check the progress of Woodford Excavating reclaiming this horizontal well pad. They have undertaken the removal of the rock channels on the northeast and east sides of the pad and are stockpiling the material for removal. I spoke with DOGRM Engineer Paul Logan and he informed that the final reclamation plans were approved on 11/27/2017 and that the required 48 hour notice to begin reclamation was given on December 4th,, 2017. DOGRM Engineering Tech Gerald Smith has been monitoring reclamation. No violations.

1733735056 2014-05-15 Production Wells No Violations I was on location to inspect this well pad in regards to complaint #s 4707 & 4708 claiming that the presence of the well pad was causing flooding in their yards and basement. The well is currently under temporary inactive status (1st year) and has never been completed. My inspection found the drainage controls on the pad and around the perimeter functioning properly. All surface drainage from the disturbed area around the pad is being diverted to the north and east and away from the residences. The entire region has experienced above normal precipitation since January and I confirmed this with records from the National Climatic Data Center (NCDC) station records from Kent and Ravenna. More recently, storms and flooding have inundated the region. I found no evidence that the well pad is causing flooding at the complainants homes. I observed a ditch that had been excavated between the two homes was flowing/draining a large volume of water from the northwest and into the road ditch. I will have Engineer Ron Trivisonno review pad construction and conduct a site visit for investigation.

1742966775 2017-05-11 Plug / Plug Back No Violations I was on location with Chesapeake (CHK) company man Blake Riley, Nine Energy Services (NES), and Weatherford to begin plugging operations on this horizontal, Utica-Point Pleasant well. NES rigged up their crane and ran into the 5.5 production casing with a junk basket and gauge ring to 6000 to verify casing drift. They then ran in the casing to 5914 and set a cast iron bridge (CIBP) plug at a depth of 5914 in the curve. NES rigged down and left for the Hosey 6H-X (22420) to shoot off the casing. Weatherford installed a night cap on the wellhead and operations will continue at this location upon plugging completion of the Hosey 6H-X. See plugging report for more details on this job. No violations.
1791787482 2014-05-21 Production Wells No Violations I was on location with Engineer Ron Trivisonno to investigate complaint #s 4707 and 4708 and determine if the well pad was causing flooding issues at the complainants residences to the southwest of the well pad. Mr. Trivisonno concurred with my original assessment that the pad was not diverting surface drainage towards the homes in question and that the drainage was to the north and east away from the homes. He will submit a report of his findings. No violations.
1809716169 2018-11-27 Final Restoration No Violations I was on location to check the status of Final Restoration of this horizontal drilling pad. This well was plugged and abandoned on 5/21/17 and reclamation of the pad was completed in February of 2018. The site was mulched and seeded in the spring of 2018 and a good, stable seed bed was established over the summer. The super silt fence that was installed after reclamation still needs to be removed for Final Restoration to be approved. I called Brian Klink (Chesapeake/Encino) and asked him to have the silt fence removed so the file could be closed. Brian said he would see that the silt fence is removed in a timely manner and would notify me when the work is complete. No violations.
