My inspection today found the lease road gate present and locked. The lease road and apron are in good repair. The pad is fenced and the entrance gate and all man doors on the perimeter of the pad were locked. The pad is becoming inundated with weeds and small diameter trees/brush approximately 1 in diameter and less. The 8H well has never been completed or produced. The wellhead is surrounded by a liquid-tight cellar with a two-piece grate covering the cellar and tubular steel gates surrounding the wellhead. The wellhead consists of a 5.5 (flanged) production casing wellhead with double wing valves on each side. There is no Christmas tree and the top of the wellhead is fitted with a night cap. No gauges are present to check annular and wellhead pressures and the cellar is full of water and frozen. There is no surface equipment or tank battery on the well pad. Chesapeake has applied for, and been issued, a plugging permit for this well that will expire on 5/11/2017. Chesapeake drilled a 398 water well on 2/10/2012 to use for completion operations and is located on the south side of the lease road at coordinates: 41.21888, -81.16999 (NAD83 +/-9). The water well will need to be properly sealed and abandoned under the rules outlined in 3701-28 of the Ohio Administrative Code prior to abandoning the location. I will speak with Chesapeake about their plans to plug and abandon the well. |
0087362419 | 2017-05-21 | Plug / Plug Back | No Violations | I was on location with Chesapeake (CHK) company man Wallace Bossert, Nicks Well Plugging (NWP), and Weatherford to finish plugging this Point Pleasant, horizontal well. The fluid level inside the 9.625 intermediate casing held overnight and NWP ran 24 joints 0f 2.375 tubing to the top of the 5.5 casing stub at 780. They rigged up and circulated 120 barrels of brine at five (5) barrels/minute (bpm) to clean the hole. They then mixed and pumped a 10 barrel sweep consisting of 15 gallons of S-417, 15 gallons of P-32, and 9 gallons of diesel at 2bpm to clean the inside of the casing prior to cementing. They followed the sweep with 65 barrels of water and pulled nine (9) joints to a depth of 489 to set the final plug. The final plug was set in two (2) stages and good cement returns were achieved at the surface. The tubing was pulled and the hole was topped off. I will check the integrity of the top plug tomorrow (5/22/17). See plugging report for more details on this job. No violations. |

I met with Derick from Woodford Construction and he told that they have completed all of the required dirt work on the pad. He told me that between 12,000 and 15,000 tons of limestone was removed from the pad during reclamation. My inspection found that the stone has been removed from the pad and back to the pad entrance as allowed by the landowner waiver (Form 7). The stockpiled topsoil that was located on the southwest side of the pad has been spread and the pad location has been filled and graded. Silt fence has been installed around the reclaimed area of the pad and is backed and supported by chain link fencing and metal poles. The track-hoe was removed from location during my inspection and the two (2) dump trucks and bull dozer will be removed tomorrow. The area will need to be mulched and seeded. As soon as a good seedbed is established, the silt fence may be removed and final restoration will be reviewed for approval. No violations. |

NWP began rigging down and the EES crew began cleaning NWPs circulating tank so that it could be loaded and removed today. I was able to verify the top of the cement plug with a weight and measuring tape at a depth of five (5) feet inside the 9.625 intermediate casing string. There was no observable leakage or gassing and I told NWP that the ID plate may be attached. The well is currently under temporary inactive status (1st year) and has never been completed. My inspection found the drainage controls on the pad and around the perimeter functioning properly. All surface drainage from the disturbed area around the pad is being diverted to the north and east and away from the residences. The entire region has experienced above normal precipitation since January and I confirmed this with records from the National Climatic Data Center (NCDC) station records from Kent and Ravenna. More recently, storms and flooding have inundated the region. I found no evidence that the well pad is causing flooding at the complainants homes. I observed a ditch that had been excavated between the two homes was flowing/draining a large volume of water from the northwest and into the road ditch. I will have Engineer Ron Trivisonno review pad construction and conduct a site visit for investigation. |
