0056124051 2014-06-30 Completion Testing No Violations I was on location to witness the hydraulic fracturing of the Clinton Sandstone in this recently plugged-back well. Universal Well Services conducted the stimulation operations. Pursie Pipes, Perry Pipes, and Mark Smith were on location overseeing the frac job. The formation broke down at 1,084psi and the well was treated with 1,000 gallons of HCL, 74,319 gallons of water, and 7500 pounds of proppant. The job went as designed. 0069414604 2012-07-25 Administrative Inspection No Violations Talked to Mark Smith (PEP Drilling) on 7/20/12 about building the lease road. I granted PEP permission to move the lease road approximately 150-200 east of the original proposed location. The move was approved by Rootstown Zoning and moved the entrance away from a guardrail. I was on location to GPS the new entrance and inspect the apron and road. The apron and road were in good condition and constructed of durable material. No violations. Note: The well status is listed as cancelled and I have a call into Steve Opritza (Division of Oil & Gas) to check the discrepancy as of 7/27/12.
0071174592 2014-06-20 Plug / Plug Back No Violations I was on location to continue with plug-back operations. Mark Smith, Appalachian Well Surveys, and Chipco were on location to conduct operations. Appalachian ran in the 4.5 casing with a 10 perforating string and perforated the casing at the top of the Trenton Limestone from 6,264-6,274. Chipco will run the tubing back in to a depth of 6,274 and a squeeze plug and isolation plug will be set on Monday (6/23/14).
0145223001 2012-08-16 Drill / Deepen / Reopen No Violations I was on location at the request of Mark Smith to discuss changing the rig and pit layout that was decided on during the pre-site review with Bob Worstall on 5/2/2012. After discussion, I granted the proposed change so that the rig would could be oriented north-south and the pits could be placed to the west of the rig due to the presence of a low drainage area and field tile immediately to the east and south. I granted this change on the condition that the excavated soil be placed as a large embankment to the west of the pits to protect the tree line from any gas flares or blow-outs.
0175522428 2012-10-05 Urban Drill / Deepen / Reopen No Violations During this status check inspection, Mark Smith (PEP) was on location with Chipcos service rig attempting to drill out the two stage cementing tool that was run in the well during the production casing cement job. I talked with Mark and he informed me that they were not told about the two stage tool and discovered it when they sat down on it with a perforating gun the previous week. They were attempting to drill out the plug and the mill-tooth bit broke apart so they were trying to get the metal out of the hole with a magnet. They are going to continue and try to drill out the tool for the remainder of the day. No violations.

0545193954 2013-10-17 UrbanProduction Wells No Violations I was on location to follow up on complaint #4504 about a loud popping noise emanating from this location. The pumping unit was operating with a natural gas motor and was equipped with an exhaust that emitted a low frequency popping sound when operating. The exhaust was directed to the northwest and the house of the complainant was approximately 1200 to the west of the location. I went to the house and could hear the exhaust from the pumping unit. I called Mark Smith (PEP) to see if there was a way to reduce the noise and he said he would attach another muffler to the end of the other muffler to reduce the noise. I will check back in several days on the progress.

1184668839 2013-09-05 Completion Testing No Violations I was on location to check the status of this well following completion operations. A Lufkin pumping unit has been installed and is operating with propane being supplied by a temporary 250 gallon steel tank on a skid. The unit is currently pumping and producing to a newly constructed tank battery near the entrance of the lease road. The battery is diked and contains 2-210 barrel tanks and a vertical separator which is not yet plumbed into flow line or tanks. There are charcoal filters on the vent stacks of the tanks. Pressure gauges showed 580psi on the tubing and 445psi on the casing. 2-500 barrel frac tanks are on location and ready to be hauled out. This site still requires preliminary restoration. No violations.
1203173566 2013-07-29 Administrative Inspection No Violations I was on location with Mark Smith and Appalachian Well Surveys to observe the running of the down hole camera into the 4.5 casing. The camera was being run to verify casing integrity following drill out of an obstruction in the two-stage cementing tool. Integrity was previously established with a pressure test after the drill out on 7/8/13. The camera was run and reached the top of the tool collar. There was no apparent damaged to the inside of the casing, at the collars, or inside the tool. Stimulation operations will commence on 7/31/13. No violations.

PEP Drilling drilled a 14.75 hole to a total depth of 146 at the kelly bushing (KB +10) and ran 126 (KB) of 11.75 conductor into the hole. Universal Well Services rigged up, established circulation, and cemented the conductor pipe with 85 sacks of Type 1 cement. 1.5 barrels of cement returned to and held at the surface. 1376661826 2013-07-08 Work Over Reconditioning No Violations I was on location to witness a casing integrity pressure test on this Rose Run well following the drill out of an obstruction. Producer Service Corporation was on location with Mark Smith (PEP) and Chipco. A 5000psi rated gate valve was threaded onto the 4.5 casing and was rigged up to the pump truck and the hole was loaded. At the start of the test, the bottom of the gate valve began leaking. The valve was removed and it was determined that the threads on the valve were worn enough that it could not be used. A 4x2 swedge valve was used in place of the gate valve. The pump truck rigged up to the swedge and began the test. 2,737psi was placed on the casing and after 30 minutes the pressure was 2,644psi (a loss of 3.4%). Casing integrity has been verified in accordance with OAC:1501-9-08 (3) and well completion operations may continue. No violations.

I talked with Allen Irwin (toolpusher) and he said they planned to spud the well in the morning. I asked if he had reviewed the permit conditions and discovered that he had been given a copy of the old permit. I told him I would bring him a copy first thing in the morning. A copy of the permit was posted at the entrance off Tallmadge Road. As I was leaving location, a street sweeper was delivered to take care of the mud on Tallmadge Road. No violations.
1416403344 2014-06-23 Plug / Plug Back No Violations I was on location to witness plug-back operations. Chipco spotted 2.375 tubing at 6,274 at the base of the perforations. Petroset rigged up, loaded the hole, and spotted 13 barrels of cement slurry in the 4.5 casing. Chipco pulled 30 joints of tubing, packed in the wellhead, and Petroset rigged up to the tubing and squeezed 6 barrels of cement through the perforations at 1000psi. Chipco pulled the tubing up to 4,850 and Petroset spotted an isolation plug to 4,650. I spoke with Mark Smith (PEP) on 6/25/14 and he tagged the isolation plug at 4,650 and set a mechanical bridge plug at 4,567, 10 above the two-stage cementing tool. Completion operations for the Clinton Sandstone will commence on 6/30/14. See plug report for more details on this job. No violations.

Gas was being sold into a Dominion line through a new Roots Meter. I talked with Mark Smith (PEP) and he is trying to get a bigger dozer out here to reclaim the location before winter. No violations.

PEP Drilling drilled an 11 hole to a total depth of 758 at the kelly bushing (KB +10) and set 738 (KB) of 8.625 surface casing in the hole. Universal rigged up, established circulation, and cemented the surface casing with 195 sacks of tail and lead cement. 3 barrels of cement returned and the cement held at the surface. PEP drilling closed the annular on the drill pipe and pressured up to 500 psi. The pressure was held for 15 minutes with no observable leak-off on the gauge or in the cellar. No violations.
2005629396 2013-07-01 Work Over Reconditioning No Violations I was on location to check the status of this Rose Run well. Mark Smith was on location with Baker Atlas and Chipco. Chipco had been milling on the obstruction most of the morning and tripped out of the hole. The mill bits teeth were worn away and the bit was badly damaged. An impression block was prepared with tar and Chipco ran it into the hole on a sand line. They tagged the obstruction and pulled it out of the hole. Half of the tar pulled off of the block and we were unable to determine the nature of the obstruction. The Baker representative was going to return tomorrow with a new mill bit. No violations.
2092975735 2014-07-30 UrbanPreliminary Restoration No Violations I was on location to check the preliminary restoration status of this well. This well was plugged back to the Clinton Sandstone and re-completed on 6/30/14. The well is equipped with a gas pumping unit and gauges at the wellhead indicate 440psi on the casing and 125psi on the tubing. The belt guard is off of the pumping unit. The tank battery needs to be fenced and is properly diked. The battery contains 2-210 barrel tanks and are equipped with the required charcoal filters. The battery also contains a vertical separator with 125psi on the gauge and a Dominion meter run. The required ID is posted at the tank battery. I spoke with Mark Smith (PEP) about the belt guard and fencing. He told me he would get the belt guard on immediately and that the fencing should be up next week. I will check back next week to see if the work has been done or the well will have to be shut-in until it is completed.
2143362626 2014-06-19 Plug / Plug Back No Violations I was on location to witness plug back operations conducted by Chipco and Petroset Cementing. Company man Mark Smith was overseeing operations. Chipco sat down at 7171 with 2.375 tubing and picked up a joint to 7139 to set the bottom plug. Petroset rigged up and broke circulation. They then mixed and pumped 6.3 barrels of cement slurry and began displacing the slurry to 6828 with salt water. The hole was circulating gas cut oil and water to the tub and I had Chipco shut down their rig motor as a safety precaution. The cement set up in the tubing 1.6 barrels (403) short of displacement. I calculated the top of the cement at just over 200 above the formation top and this satisfies OAC 1501:9-11-08(1) of the plugging requirements. The tubing string was pulled and the cement had filled 23 joints. Squeeze and plug-back operations will continue tomorrow. No violations.
