During fracing the well circulated and pressure and fluid was observed in the 8.625casing annular space. The effort today is to confirm and locate the breach in the 4.5 casing. Universal Well Service ( UWS ) moved in and rigged up on location at 0800 Hrs. UWS utilized a pump truck and wireline depthometer to pump down a weight flag to the depth of where the fluid was exiting the pipe. Upon initial pumping it took 17 Bbls. of water to circulate the well. The wireline with the flags was pumped down at 4 BPM at 850 PSI until the wireline indicated and stopped at a depth of 1810 G.L.. A total of 65 Bbls. were pumped during this procedure. UWS then rigged down and moved out at 1155 Hrs. Appalachian Well Survey moved in and rigged up at 1230 Hrs. to run a Variable Density Log ( VDL ) and set a Bridge plug, if necessary. The VDL indicated a cement top at 3640 G.L. The Gamma Ray malfunctoned downhole and was not avaialable. Keith Moore decided to set a wireline composite bridge plug in order to effectively shut-in the well. The brigeplug was set at 3700 G.L. in a zone of competent cement bond. AWS was released at 1400 Hrs. Mr. Moore indicated the remedy to the situation would be to cut the casing as deep as possible to insure that as much questionable casing is removed from the wellbore and replace with competent casing. Mr. Moore said that the operations to cut and patch the casing would begin during the week of May 6 2013 pending availibilty of tools and equipment. With the bridge plug in the hole the well is static. |

The annulus between the 8.625 surface casing and the 4.5 production casing had been left open overnight and was plumbed with a 2 steel line to a frac tank and chained down. There was no pressure or circulation from the annulus. Moore Well Services was on location and set a bridge plug at 3700 to isolate the perforations in the Clinton Sandstone. Inspectors Bob Roush and John Fleming arrived on location and Universal Well Services arrived at 8:00am to run a wireline and determine the depth of the parted casing. Universal rigged up to the wellhead, attached twine to the end of the wireline, and placed the wireline tool in the deployment head. They pumped 65 barrels of water ahead, began circulating up the annulus, and released the tool. The twine caught in the parted section of casing and the wireline indicated a depth of 1810 on the counter. Universal rigged down and left location. Moores decided they would use a chemical cutter and cut the free pipe above the Salina Salt and patch the casing with a lead sleeve next week. No violations. |

By the end of the job, the rate was 31.3 BPM and they began flushing the sand out of the casing. Average treatment pressure was 1591psi and instant shut-in pressure was 1160psi. Job totals were 200 MCF of nitrogen, 84,824 gallons of water, and 43,300 lbs of 20/40 sand. No annular pressure was observed during the job. No violations. |

I inspected the lead sleeve and found that it was new and rated to 3750psi by Baker Atlas. The 4.5 casing was new, J-55, 10.5 lbs/ft production casing. Moore Well Services threaded the tool onto the bottom joint and began tripping into the hole with the new casing. The sleeve landed at the cut joint, was rotated onto the casing, and was permanently set with 25,000 lbs of pull on the string from the service rig. Universal arrived on location to conduct a casing pressure test to verify casing integrity. Universal rigged up and placed 3000psi on the casing string at 4:27pm. After 45 minutes, the recording chart indicated that the pressure held steady and did not leak off. Casing integrity has been verified in accordance with OAC 1501:9-1-08(D)(3) and Moore Well Services may continue with completion operations. No violations. |

Keith and Jeff Moore were on location with Universal Well Services. Universal was mixing the frac tanks and rigging up the iron to the wellhead. Universal began pumping freshwater downhole and the formation broke down at 1228psi. Universal then displaced the HCL spotted the previous day with 257 gallons of freshwater and proceeded to drop 100 balls to cover the perforations and shut down for 30 minutes. At this point I left location to inspect an oil spill in Deerfield. At 3:00pm, I received a call from Keith Moore telling me that they had parted the 4.5 casing at approximately 2100psi and that the well was circulating up the annulus of the 8.625 surface casing. He said they were collecting the gel and water in a pit and were going to plumb the surface casing valve into a steel tank. I told him that was acceptable and I would be back to the location as soon as possible. I called North Region Supervisor, Tom Hill, and told him of the situation and he said he would head right out. I arrived back on location and met with Tom Hill, Inspector Bob Roush, and Moore Well Services. Moore was plumbing up the surface valve to the tank and Pander trucking was collecting the flowback fluid from the pit into their water truck. Tom Hill was monitoring the location with a gas detector and did not detect any LEL or H2S while the well was flowing back from 4:00-5:30pm. The annulus stopped circulating around 5:30pm and Moores opened the valve on the 4.5 production casing. The well was not gassing and the casing was loaded with freshwater as a precaution. The annulus was left open and plumbed and chained to the steel tank for the evening. I will return in the morning with the Moores to determine where the hole formed in the casing and what remedial action will be required to properly repair the well. No violations. |
