0099791973 2014-01-09 Production Wells No Violations I performed a status check inspection of this horizontal well and associated surface facilities. The well is currently shut in and shows 500psi on the tubing. The annulus between the 5.5 production casing and the 9.625 intermediate casing is equipped with a gauge and shows no signs of pressure. ID signage is posted at the wellhead, tank battery, and at the entrance. I found no violations at this time.
0160590012 2012-12-03 Completion Testing No Violations I was on location to check the status of completion operations and hydraulic fracturing of this horizontal well that began this morning. I met with Jeff Wozniak and Jake Smathers (Mountaineer Keystone) to observe the completion operations. A 32 sound wall had been constructed inside the 20 sound barrier on the east and north sides of the location in an attempt to mitigate noise emanating from the equipment on location. Cased Hole Solutions was currently pumping the perforating gun into the hole along the horizontal leg of the well to set the kill plug for the previous stage and perforate the next stage. Pressure and possibly sand in the well bore from the previous treatment was impeding operations at a measured depth of 8691. Mountaineer decided to pull the gun out of the hole and might try installing slotted rings around the perforating gun to give them more push down-hole. Containment was in place around the wellhead and the location was in good condition. No violations.
0552627784 2012-08-15 Drill / Deepen / Reopen No Violations I was on location with inspectors Tom Hill, Bob Roush, John Fleming, Steve Tompkins, and Ryan Channell to witness cementing of the conductor pipe and review drilling plans for Mountaineer Keystone (MK). Others in attendance were Ross Schweitzer, Jim Watson, and Bill Ingalls from (MK), Union Drilling, and other contractors. The plans were reviewed and discussed so that everybody involved was on the same page and understood what was required by the Division of Oil and Gas. For the conductor cement job, Butchs Rat Hole had augered a 30 hole to 63 and set 63 of 20 conductor casing in the hole. Baker Hughes rigged up, established circulation, and cemented the casing with 195 sacks of Type 1 cement. Good cement returns were achieved and held at the surface. 0668323325 2012-12-13 Completion Testing No Violations I was on location with North Region Supervisor Tom Hill to check the completion status of this horizontal well. We met with Jake Smathers (Mountaineer Keystone) in the wireline truck. Cased Hole Solutions was running in the hole to set the kill plug and perforate the final stage of this well prior to fracture stimulation. Containment was down and the location was in good condition. No violations.

Tim Altier (Mountaineer) and Garrettsville and Windham Fire Departments were on location as well. Rockwater Energy was assembling the flow back equipment and Tim led us around the location discussing the testing process and safety measures installed on the equipment. All gas and vapors will be incinerated on location as they are produced during the flow back. Testing will begin Tuesday morning. No violations.

Canton Oil Well Services was on location painting equipment. Pressure on the wellhead was at 1850psi. Mountaineer plans on removing the walls and ditch around the location in the next several weeks and reclaim the west side of the pad. Restoration is progressing. No violations.

1143647987 2012-12-06 Completion Testing No Violations I was on location to check the status of completion operations and hydraulic fracturing of this horizontal well. I met with Tim Altier (Mountaineer Keystone) and he informed me that the wireline had pulled off the perforating gun down-hole. Integrated Production Services (IPS) was on location with a coiled tubing unit and currently tripping out of the hole with the gun. Normal operations will resume after the gun is retrieved. Containment was in place around the wellhead and the location was in good condition. No violations.

1164236179 2013-05-16 Completion Testing No Violations I was on location to check the completion testing status of this well. Completion testing was finished and all temporary flaring and flowback equipment has been removed. There are seven empty frac tanks sitting on containment. The site is clean and the wells are shut-in. 6 steel pipeline has been laid out from the southeast corner of the pad and runs south across the property. No violations.
1232007741 2012-10-12 Drill / Deepen / Reopen No Violations I was on location at 10:15pm to witness the surface casing cement job. Universal Well Services was on location and Union Drilling had just started to run the casing. Union was having trouble threading the 13.375 surface casing. Union ran 10 joints of 13.675 surface casing with 10 centralizers and two baskets. Universal rigged up the cement head, began circulating the hole at 3:30am and started running gel and flake. At 3:45am Universal was asked to shut down so the pump from the half round tank could be unclogged. Universal began circulating the well again at 4:00am and pumped 375 sacks of Type 1 cement with 20 barrels returning and holding at the surface. The plug landed at 4:50am and the pressure was increased from 130psi to 420psi and held for three minutes with no leak-off. See cement/casing ticket for more details on this job. I instructed Union to lower the the light pole in front of the rig at the request of a landowner on Frasier Road. They lowered the pole down to where the light shining into the backyard was below the sound wall. No violations.

Union Drilling was drilling a 12.25 hole at 2998 measured from the kelly bushing. All drilling equipment is on containment and Veolia is hauling drill cuttings. No violations.
1284576993 2013-08-08 Preliminary Restoration No Violations I was on location to inspect the site for preliminary restoration. The sound walls have been removed and the ditches around the pad have been taken out. The west side of the pad has been reclaimed, graded, and seeded. Vegitation is well established around the perimeter of the location. Preliminary restoration is approved.
1308657592 2012-10-19 Drill / Deepen / Reopen No Violations I was on location at 8:45 P.M. to witness the cementing of the intermediate casing. When I arrived, Universal Well Services was rigging up the cement head. Union Drilling ran 97 joints of 9.625 intermediate casing to a total depth of 4242 at the kelly bushing with 32 centralizers. Universal rigged up and pumped 257 barrels of lead cement and 50 barrels of tail cement with good returns back to surface. The plug landed at 12:00 A.M. and the well was pressured up to 1600psi and held for 5 minutes with no leak off. Bill Ingalls (Mountaineer) told me they were going to install mufflers on the mud pumps and rig motors to try and mitigate some of the noise from the location. I left location at 1:30 A.M. 1309928592 2015-04-29 Production Wells No Violations I was on location to check the status of this Utica Shale well. This well remains shut-in due to pipeline tie-in issues and has only produced during flow back operations conducted in the second quarter of 2013. Gauges indicate 1865psi on the tubing and 980psi on the casing. The wellhead is contained within a liquid tight cellar that has been filled with gravel and the 13.375 surface casing annulus is open to the atmosphere. The well is plumbed to a fire heated, gas production unit and is connected to two (2) 400 barrel tanks inside a containment barrier. Concrete barriers have been positioned around the wellhead for safety. The gate valves on the wellhead are chained and locked and there is a glycol injector on the west side of the wellhead. IDs are posted at the wellhead and the lease road entrance. The lease road is gated and locked. There is a pile of compost filter socks and orange safety fence on the north side of the pad and a pile of plastic liner on the south side of the pad. I will ask Mountaineer Keystone to remove the unused equipment, about their plans for the well, and to consider applying for temporary inactive status (1st year) to comply with Chapter 1509.062 of the Ohio Revised Code. No violations.

1351169966 2012-10-16 Drill / Deepen / Reopen No Violations I was on location to pick up the cement ticket from 10/13/12 and to follow up on a noise complaint I received from the landowner (Beckie Dean) at 10085 Frazier Road. Union Drilling was air/mist drilling the 12.25 intermediate hole at a depth of 3000 measured from the kelly bushing. All drilling equipment is on containment and Veolia is hauling drill cuttings. No violations. I called the landowner at 3:18pm and left a voicemail that I was in the area and would like to meet to investigate the issues. I waited for 20 minutes and then left the location. I spoke to Beckie later in the evening and told her I would meet with her on Thursday (10/18/2012).
1352144883 2012-10-19 Drill / Deepen / Reopen No Violations I was on location to follow up on a complaint at 10085 Frazier Road. Union Drilling had drilled the intermediate hole to a depth of 4270 measured from the kelly bushing and were preparing to run 9.625 surface casing following the test described next. Mountaineer Keystone (MK) was preparing to run a test in an attempt to determine the source of the noise. The Director of Drilling, Jim Watson, had Union Drilling start up every piece of equipment on the drilling rig including the mud pumps and air compressors. After fifteen minutes, Travis Canterbury (MK) and I left and went to the landowners house on Frazier Road to observe and document anything out of the ordinary. I observed that the noise was still noticebly louder in the northwest corner by the chimney than in the front of the house. The subtle resonance I observed the previous day did not manifest itself with all of the equipment running. Union Drilling began shutting down pieces of equipment one at a time to try and isolate what was causing the noise. After the all the equipment was shut off, they began starting and stopping each piece of equipment one at a time. I could not draw any solid conclusions from the test. Travis documented the test and the landowner showed him the damage she thought the rig was causing to her home. I returned to location and informed MK that Mike Mann from the Bureu of Mines would be out here on Monday (10/22/12) to conduct a seismic/acoustic survey and determine if any noise or vibrations from the rig might be causing damage to the house at 10085 Frazier Road.
1352235133 2012-10-22 Drill / Deepen / Reopen No Violations I was on location at 10085 Frazier Road with Mike Mann from the Bureau of Mines to conduct a seismic/acoustic survey to determine what, if any, damaging sound or seismic vibrations might be reaching the home located here. Mike met with the landowner and she showed him what she thought was happening to her house due to the drilling rig. Mike installed a geophone and microphone at the northeast corner of the house and another geophone and microphone outside the southwest corner of the drilling pad in a bean field. I informed Mountaineer Keystone that they could start normal operations just after Noon, and they began running drill pipe into the hole to drill out the cement plug. They began drilling the plug around 4:00 P.M. and Mike decided he would leave the equipment recording overnight and into tomorrow.
1353002537 2012-10-25 Drill / Deepen / Reopen No Violations I was on location to check the drilling status and follow up on complaint from earlier in the day. A landowner at 9915 Silica Sand Road complained about lights shining into the windows of her house. I inspected the location and found a light tower in front of the rig on the north side was telescoped above the sound barrier. I asked Union Drilling if it was necessary or a safety issue to have the lights raised that high. They informed me it was not and lowered the light tower so that it was not shining over the wall. I asked them to be mindful of not raising the lights above the wall. Union Drilling was building curve towards the horizontal leg of the well at a depth of 4970 measaured from the kelly bushing and drilling on fluid. No violations.
1354735393 2012-12-05 Administrative Inspection No Violations These are details of my follow-up to complaint #4123 from 8882 Pierce Road in Nelson Township pertaining to a claim that emissions from the well location were causing asthma symptoms to a resident at the aforementioned address. My initial investigation found that the location of the complaint was 2.2 miles from the location at a bearing of approximately 286 degrees. Prevailing winds would need to be out of the southeast direction for exhaust fumes from the Soinski pad to reach this home. This location is also 2.4 miles north of the Ohio Turnpike. A check of wind direction records from the National Climatic Data Center Youngstown Station indicated a total of 9 days, out of the 72 days the rig was on location, where prevailing winds were out of the southeast. I spoke with Mr. Harkai at 11:28am on 12/5/12 and he said that the asthma symptoms appeared during the past 3-4 weeks. I informed him that the drilling had stopped on 11/7/2012 and he told me was not sure when initial symptoms began. Mountaineer Keystone cemented production casing on 11/7/12 and began rigging down and moving the rig from location for the next week. Following the rig move, completion equipment was moved in and spotted on location for the remainder of November. Completion operations began on 12/3/12. Mr. Harkai also told me he had heard that other landowners were having noise issues at a meeting in Streetsboro and thought maybe the drilling rig emissions could be the source of the asthma symptoms, but he did not know. He was under the assumption that gases were escaping out of the borehole during drilling operations as well and I assured him that was not the case. I told him based on my findings of his location, prevailing wind directions, and activity at the location; I could not substantiate his claim that rig emissions are or were causing the asthma problems without some type of medical or physical evidence.
1359395694 2012-12-21 Administrative Inspection No Violations This inspection is in response to complaint #4150 at 9915 Silica Sand Road about the presence of methane in a water well. I responded to the location to check for the presence of dangerous levels of methane and LEL in the landowners home. My gas detector was malfunctioning and would not reset prior to entering the home. I called Inspector Rick Skidmore and asked if he could meet me at the home with his gas detector. Rick arrived and we entered the home with the gas detector on and funtioning properly. We enetered every room on the main floor checking for gas with the hand pump connected to the detector. A reading of 0.05 methane was detected in the living room but quickly went back to zero. No LELs were detected on the main floor. We entered the basement and I continued to survey for methane and LEL. I checked the entire basement including around the furnace, hot water heater, the well pressure tank, and the sump in the back corner of the basement. I did not record any methane readings or LEL triggers on the lower level. We left the home and I spoke with Kyle Tharp who was on his way up to collect water samples and check the well.
1360694498 2013-01-16 Administrative Inspection No Violations I was on location with Supervisor Tom Hill, Inspectors Ryan Chanell and Kevin Perry, and Tim Altier (Mountaineer Keystone) to inspect the annuli on this well. Mountaineer had removed the pea gravel from the cellar to expose the cut-off casing. The annulus between the intermediate casing and the surface casing showed obvious signs of gas-cut cement and was visibly bubbling when water was placed over the cement. The annulus between the surface casing and the conductor casing had several bubbles, but was not visibly gas-cut around the annulus. The annulus between the production casing and the intermediate casing showed no signs of pressure found the previous day. We requested that the annulus be left open and monitored for pressure. I requested copies of all the cement bond logs for all of the casing strings and Tim said he would get them to me. No violations.
1366383383 2013-04-03 Completion Testing No Violations I was on location with Deputy Chief Bob Worstall to check the completion testing of this horizontal well. Rockwater was flowing the well back and incinerating the methane that was being produced with a large burner system. There was a small amount of liquid being produced to steel tanks and being hauled away by Stallion. Equipment was on containment and safety measures were in place. No violations.

1384987854 2013-01-15 Administrative Inspection No Violations I was on location with Supervisor Tom Hill, Inspector Kevin Perry, and Mountaineer Keystone (MK) Operations Manager Tim Altier to check for any annular pressure on this well. The only open annular space on this well is located between the 9.625 intermediate casing and the 5.5 production casing. All other casing strings had been cemented to surface. We did not have the proper size gauge or bull plug to check the pressure and would meet later in the afternoon. We returned at 1:30pm with the proper equipment. Tim hooked up the plug and gauge. The valve was opened and the gauge read 500 psi. We decided to bleed off the pressure and recovered 8-10 gallons of drilling mud from the annulus. The pressure bled down to 0 psi and the mud was disposed of in a steel tank. While bleeding the pressure off, Tom used my gas detector to monitor for methane and LEL emanating from the annulus. The detector registered .25-.30 for methane but no LEL was detected. The cellar had been filled with pea gravel and we requested that the gravel be removed to expose the other annuli on the well. Tim said he would have this done tomorrow. No violations.

Universal rigged up and pumped 1688 sacks of Type 1 cement, dropped the plug, and displaced the cement to 11,618. The calculated top of the cement is 1300 into the intermediate casing. 1556644123 2013-06-20 Production Wells No Violations I was on location to check the status of this horizontal well. The well was shut-in at the time of my inspection and all of the flow-back and testing equipment had been removed. Two steel tanks were on containment and being cleaned prior to removal. The 6 pipeline has been installed and buried south of the location and tied into a compressor station that is being constructed. The station is located east of the main building as you enter down the haul on the property. No violations.

1973755287 2012-10-18 Drill / Deepen / Reopen No Violations I met with the landowner at 10085 Frazier Road at 11:30 A.M. this morning to discuss issues pertaining to the drilling of the Soinski 201 and 202 northeast of her home. She told me that the noise every evening is keeping her awake and that she is afraid that the drilling rig is damaging her home. She had me stand inside her front door and listen to the drilling rig and it was a very low humming sound. She then took me back by her fireplace in the northwest corner of the house and the humming from the rig was much more audible. There was a very subtle resonating feeling standing in the corner. I had no explanation why it was louder and seemed to be resonating by her fireplace other than the rig noise was being amplified by her chimney. Union Drilling Rig #48 was currently drilling on air at a depth of 4130. She proceeded to show me hairline cracks in her fireplace that cut across the grouting and through the river rock masonry as well as cracks in her drywall and in the north basement block wall. I took pictures of everything she was pointing out to document the damage she felt the drilling rig was causing to her house. Whether or not this damage was caused by the drilling rig or caused by other forces, I am not of the expertise or knowledge to say. I was concerned about the noise I was hearing through the chimney and if it was having any affect on her house. After I left, I called John Nock with Mountaineer Keystone (MK) and described to him what I had heard and seen at the landowners house. I told him that they needed to try and mitigate the noise from the pumps and motors either with mufflers or try aiming the exhausts in a different direction. I also suggested that they send somebody out from MK to meet with the landowner and document her claims. I called the landowner back and asked if I could bring a representative from MK to her house tomorrow to show them what she thought was happening and she said that would be fine as long as I was present. The well was flowing back and the methane was being incinerated on location. All equipment was on containment and secure. We showed Blake around the well pad to become familiar with pad construction. No violations.
2033023530 2013-01-31 Production Wells No Violations I was on location to check the status of this horizontal well. ROC Well Services was on location constructing the surface facilities and equipment. The tank battery was constructed and contained 12-400 barrel tanks. The spacing between the tanks meets the requirements of at least three feet. The tank battery was diked with a 4 steel wall. The entire dike was lined with a heavy duty liner and was riveted to the top of the steel wall. Pea gravel was placed on top of the liner and the tanks were set on steel rings filled with gravel. The productions units had no been spotted. The wellhead was chained and locked and the cellar was full of water. The face of the gauge that was placed on the annulus between the production and intermediate strings was broken as if it was hit by something. The valve was open and there was no sign of pressure. No violations.
