0244374702 2013-08-15 Preliminary Restoration No Violations I was on location with Tom Hill and intern Matt Waugh to check the status of this horizontal well pad. All drilling equipment has been removed from location. The wellhead is fenced, chained, and locked. All annuli have pressure gauges and show no signs of pressure. This well has not been stimulated as of this date. The pad is clean and drainage from the pad outside of the northeast and southeast corners is adequate. Vegitation is well established at the drainage areas and around the location. There is no off-site sedimentation or erosion occurring from the pad. Preliminary restoration is approved. No violations.
0841292609 2013-03-25 Drill / Deepen / Reopen No Violations I was on location at 9:00pm to witness the cementing of the intermediate casing for this horizontal well. Sidewinder Drilling drilled a 12.25 hole to a depth of 4360 measured from the kelly bushing (KB) and ran 4345 (KB) of 9.625 casing into the hole with 32 centralizers and a float collar at 4298 (KB). Universal rigged up and began circulating the hole. They ran 50 barrels of gel and Unicele, 5 barrels of water, 20 barrels of Flo-stop, and another 5 barrels of water ahead of the slurry. They then mixed and pumped 315 barrels of cement, dropped the plug, and displaced the plug to the collar with 326 barrels of freshwater. The plug landed at 12:49am at 1500psi and the pressure was increased to 2000psi and held for 5 minutes with no observable leak off. 15-20 barrels of good cement returned to surface. No violations. 1096910369 2013-03-28 Drill / Deepen / Reopen No Violations I was on location with Supervisor Tom Hill to check the drilling status of this horizontal well. We met with Seth Snider (Mountaineer) and reviewed the bond log for the intermediate casing string. We were satisfied with the log. Sidewinder was drilling the curve around 5400 and Tom and I inspected the location. We found several tears in the containment and brought them to the attention of the crew fixing the liner behind the shakers.
1148998388 2013-04-03 Drill / Deepen / Reopen No Violations I was on location with Deputy Chief Bob Worstall to inspect the drilling operations of this horizontal well. Sidewinder Rig #64 was tripping directional tools and drill string into the hole. We met with Bill Ingalls (Mountaineer) and he told us that they had cement from the plugback on the previous hole clogging up the tools. The measured depth of the hole was currently 10,748 (KB). All equipment was on containment and the location was in good condition. No violations.
1364236502 2013-03-17 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the 13.375 surface casing. Sidewinder Drilling drilled a 17.5 hole to a depth of 500 measured from the kelly bushing (KB) and ran 11 joints of 13.375 casing to a depth of 473 (KB). Black Hawk rigged up the rotating cement head and Universal rigged their lines into the tool. Universal began circulating the hole and pumped 20 barrels of gel and flake followed by 5 barrels of freshwater to condition the hole. They then mixed and pumped 89 barrels of cement slurry and displaced the slurry with 68 barrels of freshwater. 30 barrels of good returns came back to the surface. No violations. 1368457851 2013-04-08 Casing String Witnessed No Violations I was on location at 6:50am to witness the cementing of the 5.5 production casing on this horizontal well. Sidewinder Drilling had drilled a 7.875 hole to a total depth of 13,680 measured from the kelly bushing (KB). Sidewinder and Franks Casing rigged up the CRT tool and ran 13,571 (KB) of new, P-110, 5.5 production casing with 180 centralizers and a 94 shoe track on the bottom. Universal rigged up, established circulation, and pumped 50 barrels of Unisweep and 10 barrels of freshwater spacer. They then mixed and pumped 371 barrels of cement slurry, shut down, dropped the plug, and displaced the plug to the float collar with 299 barrels of water. The plug landed at 1910psi and the pressure was increased and held at 2570psi for five minutes. The float held and the well was shut-in. The hole circulated during the entire job. No violations.
1466361150 2015-04-29 Production Wells No Violations I conducted a status check inspection of this Utica Shale well. This horizontal well was drilled and 5.5 production casing was cemented on 04/08/2013. The cased hole was never perforated or stimulated for production purposes. The lease road access gate is locked and the well and company identification are posted at the entrance. The wellhead is contained in a liquid tight cellar that is full of water. The cellar is surrounded by multiple layers of orange safety fencing and concrete barriers are protecting the cellar and wellhead. The rat hole inside the cellar needs to be plugged. There is no surface production equipment on the well pad. I will speak with Mountaineer Keystone about their plans for the well and ask that they apply for temporary inactive status (1st year) to comply with Chapter 1509.062 of the Ohio Revised Code. No violations.
1545486246 2013-04-09 Drill / Deepen / Reopen No Violations I was on location with Deputy Chief Bob Worstall, Engineer Blake Arthur, and North Region Supervisor Tom Hill to inspect drilling operations. The rig had walked from the Soinski #212 which was completed the previous morning. The wellhead and cellar were surrounded by safety fence. The annuli on this well were open and equipped with pressure gauges. The gauges showed no annular pressure. No violations.
1597218879 2012-12-13 Administrative Inspection No Violations I inspected this location to check the status of site construction and to collect new GPS readings of the well location. Mountaineer Keystone is in the process of re-permitting the wells on this pad and have re-staked the well locations 30 to the south of the previous locations and have reversed the numbering of the wells. The entrance and location have been stoned and the topsoil has been placed in a berm around the location and seeded. Compost filter socks have been staked outside the berms to prevent erosion around drainage areas. Site construction appears to be complete. No violations.
1675238295 2013-03-19 Drill / Deepen / Reopen No Violations I was on location to check the status of this horizontal well. I met with Seth Snider and reviewed the cement bond log from the surface casing. Sidewinder was currently changing out rubbers on the stack and preparing to drill out the plug. No violations.
1873044266 2013-05-16 Administrative Inspection No Violations I was on this well pad to check the status of the location since moving the drilling rig from location. All drilling equipment has been removed from the location as well as all housing trailers. Containment is still down on the drilling pad and around the wellhead. The wellhead and cellar are surrounded by orange safety fencing. All annuli on the wellhead are open and equipped with gauges which indicate no annular pressure. No violations.
