The gate valve on the 4.5" casing was opened and the valve on the annulus between the 8.625" surface casing and 4.5" was open. Universal began pumping and broke the formation slightly at 1065psi, shut down, and went back in and got a good break at 1201psi and shut down. 0457376956 2013-09-10 Urban Drill / Deepen / Reopen No Violations I was on location to check the rig up status of Poulson Drilling Rig #1. Equipment has been spotted as well as the pipe and casing trailerss in front of the rig. The working pit is dug and lined and the reserve pit has just been excavated. The top 2-3 feet was sandy soil and the rest of the pit is in good clay. No water was encountered during excavation and the pit was then properly lined. I inspected the 11.75 conductor casing and it is new, Range-3, and weighs 32 lbs/ft. The rig will spud the well tomorrow morning. No violations.
0506120362 2013-12-06 Plug / Plug Back No Violations I was on location to witness the plugging of this recently drilled and completed well. Plugging operations were conducted by Hagen Well Services, Petroset Cementing, and J. Hawk water hauling. The bottom plug was bullheaded through the perforations in the 4.5 casing and the casing was ripped at 3500. Cement plugs were set across the Big Lime, Berea Sand & Ohio Shale, and glacial deposits. Good circulation was established prior to setting the plugs and cement circulated and held at the surface on the final plug. See plug report for more details on this job. No violations.
0778329443 2013-09-20 UrbanPreliminary Restoration No Violations I was on location to check the status of the closing of the drill pits. The working pit was in good condition, solidified properly, and being closed. The reserve pit required 3 loads of dust from ChemTech and was mixed well and setting up. I threw a large rock into the pit slurry and it was nearly solidified. The reserve pit can be closed. The wellhead was equipped with a 4x2 swedge and was closed. The valve on the 8.625 casing was plumbed up above grade. All drilling equipment has been removed from location. No violations.

Hagen Well Services had their rig spotted over the hole and were swabbing to a tank. Rods and tubing were spotted and ready to run in the hole. The location was in good condition. No violations.

Refer to the urbanized are permit conditions attached to the permit for details on this inspection. No violations.
1379080475 2013-09-11 Casing String Witnessed No Violations Poulson Drilling Rig #1 drilled a 14.75 hole to a depth of 92 (KB). Bedrock was encountered at 60 (KB) and they ran 2 joints of 11.75 casing into the hole and set at 84 (KB). Petroset rigged up to the cement head, established circulation, then mixed and pumped 125 sacks of Class A cement with 50 pounds of cottonseed hulls in the lead slurry. The hulls circulated to surface and the cement was displaced to 64 with 8 barrels of freshwater. 8 barrels of cement returned to the pits. No violations.
1379363647 2013-09-16 Urban Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Poulson Drilling Rig #1. The rig had encountered a flow zone yesterday around 3800 and began making water. The rig was forced to convert to fluid drilling operations and were currently at a depth of 4218 (KB). They are drilling at a rate of 2 hours/kelly (15/hour) and should reach total depth early tomorrow morning. The pits are in good condition and have plenty of freeboard. No violations.
1379623901 2013-09-17 Casing String Witnessed No Violations Poulson Drilling Rig #1 drilled a 7.875 hole to a depth of 4470 (KB) and ran 108 joints of 4.5 casing to a depth of 4430 (KB). Universal rigged up, established circulation and ran 12 barrels of Unisweep, 7 barrels of calcium, and 7 barrels of Flo-Stop separated by 5 barrel water spacers. They then mixed and pumped 43 barrels of cement slurry and displaced the slurry and plug to 4420 (KB) with 70 barrels of water. The plug landed and the pressure was increased to 1210psi and held for 7 minutes. The float held but the pressure dropped to 1002psi over the seven minute period and equates to a pressure drop of over 17%. OAC 1501:9-1-08(D)(3)(a) allows for a pressure decline of no more than 10% over the 5 minute test period to establish casing integrity. Casing integrity and cement placement shall be further evaluated prior to Cedar Valley continuing with any completion operations and appropriate corrective action shall be taken, if necessary, to verify casing integrity. No violations.
1380300580 2013-09-23 Urban Drill / Deepen / Reopen No Violations I was on location to witness the casing integrity test for this used 4.5 pipe that was set and cemented on 9/17/2013. After landing the plug during the primary cement job, the casing was unable to hold the pressure at or below the 10% leak off allowed by OAC 1501:9-1-08(D)(3)(a) to verify casing integrity. Tim Cosma (Cedar Valley) installed a pressure gauge on the casing and a meter chart to record the test. The hole was loaded with water and the annulus between the 8.625 surface casing and 4.5 production casing was opened. A solvent truck was used to place 2040psi on the 4.5 casing and the test began at 8:20am. At 8:50am the test was complete and the chart and gauge indicated a pressure drop of roughly 20psi which is equal to less than a 1% leak off. Casing integrity has been verified and Cedar Valley may continue with completion operations. No Violations.
1381437663 2013-10-01 Completion Testing No Violations I was on location to witness the hydro frac of this Clinton Well. Universal Well Services was on location to complete the well and Steve Hagens service rig was spotted on the south side of the well. Universal rigged up there iron, tested the lines, and released the pressure. The gate valve on the 4.5 casing was opened and I made sure the annulus between the 8.625 casing and the 4.5 was open. Universal displaced 750 gallons of HCL through the perforations in three stages and the formation broke down at 1201psi. The job was completed with 465 sacks of sand. The job went as designed and there were no pressure deviations during the job. No violations. See stimulation report for more details on this job.
1915180110 2013-10-15 Completion Testing No Violations I was on location to check the status of this recently completed well. The wellhead was shut in and rods and tubing were spotted to the north side of the wellhead. MWR was hauling out the last of the frac tanks. Surface facility construction has not yet commenced. No violations.
2043529816 2014-01-08 Final Restoration No Violations I was on location to check the status of the site following plugging and abandoning the well 1 month ago. All pits are closed and there is no equipment on location. The site and lease road are frozen and rutted badly. Silt fence is still in place along the western edge of the location and topsoil is stockpiled to the east. Restoration work is not possible at this time due to the weather conditions. The site will need to be graded and seeded and the partially built dike and stone for the tank battery will need removed. Final restoration is due in May. No violations.
